Long-term outcomes and sex differences after restorative proctocolectomy in pediatric patients with ulcerative colitis.

BACKGROUND: Restorative proctocolectomy (RPC) for ulcerative colitis (UC) could result in a higher patient quality of life, avoiding frequent disease flares; however, pouch failures and pouch-related complications (PRCs) can develop. PURPOSE: No cohort studies have examined pouch failure and the differences between adult and pediatric patients or the sex differences in pediatric UC. Therefore, the pouch failure rates were compared between adults and pediatric patients, and pouch failure and PRCs in pediatric UC were evaluated. METHODS: UC patients who underwent RPC between January 1987 and J

Colorectal cancer and inflammatory bowel disease: epidemiology, risk factors, mechanisms of carcinogenesis and prevention strategies.

Patients with long-standing ulcerative colitis and Crohn's disease have an increased risk of developing colorectal cancer and patients with small intestinal Crohn's disease are at increased risk of small bowel adenocarcinoma. Colorectal cancer appearing on the ground of inflammatory bowel disease is the result of a process which is believed to begin from no dysplasia progressing to indefinite dysplasia, low-grade dysplasia, high-grade dysplasia and finally to invasive adenocarcinoma, although colorectal cancer can arise without proceeding through each of these steps. Ulcerative colitis patient

Inulin and oligofructose in chronic inflammatory bowel disease.

Crohn's disease and ulcerative colitis, also called chronic inflammatory bowel diseases (IBD), affect up to 500 per 100,000 persons in the Western world. Recent studies in the etiology of IBD suggest that these diseases are caused by a combination of genetic, environmental, and immunological factors. Results from humans and especially animal models of colitis reported by our group and others have indicated that these diseases result from a lack of tolerance to resident intestinal bacteria in genetically susceptible hosts. Probiotic bacteria have health-promoting effects for the host when inges

Therapy of ulcerative colitis.

PURPOSE OF REVIEW: The past year has brought forth several controversies regarding the treatment of ulcerative colitis along with several potentially important clinical advances. The aim of this review is to provide the practicing gastroenterologist with a summation of these recent developments, with particular emphasis on advances that have already moved toward acceptance into clinical practice. RECENT FINDINGS: Controversies remain regarding the optimal formulations and doses of aminosalicylates. At the same time, the potential for chemoprotective effects in patients using maintenance thera

Adherence to advanced therapies in inflammatory bowel diseases: Real-world rates and predictors of non-adherence.

BACKGROUND AND AIM: Biologics and small molecules are key treatments for inflammatory bowel diseases (IBD), but long-term adherence remains challenging. This study assessed adherence to biological and small-molecule therapies and identified predictors of non-adherence in IBD patients. METHODS: Treatment schedules (January-October 2024) were collected for all patients receiving biologics or small molecules. Infusion dates were retrieved from administrative databases, while patients on subcutaneous therapy were contacted by phone. For oral agents, adherence was measured by pill counts at resupp

Outcome of restorative proctocolectomy when the diagnosis is suggestive of Crohn's disease.

Twenty of 81 patients treated by restorative proctocolectomy for presumed ulcerative colitis had some features of Crohn's disease: 10 were classified as definite Crohn's disease and 10 as indeterminate colitis. These pathological features were first apparent during synchronous colectomy and pouch construction in 10 of 11 cases. In the remainder, histological features of possible Crohn's disease were first identified during rectal excision (n = 6), preliminary subtotal colectomy (n = 2), and after pouch excision (= 2). Complications were marginally more common in patients with features of possi

[Restorative proctocolectomy with an ileoanal pouch. Postoperative course and long-term functional results].

INTRODUCTION: Over the last 25 years restorative proctocolectomy with an ileoanal pouch has been the gold standard in the surgical treatment of ulcerative colitis and in selected patients with familial adenomatous polyposis. We present a study of the course, complications and long-term functional results. MATERIALS AND METHODS: A prospective cohort analysis and a questionnaire in 178 consecutive patients operated since 1987 in Hvidovre Hospital. RESULTS: Postoperative complications were seen in 38 patients (21%), but only few were serious: anastomotic leakage in 2 (1%), pelvic abscess in 4 (

Restorative proctocolectomy--a nine year experience at the King Faisal specialist hospital.

INTRODUCTION: Inflammatory bowel disease and Familial Adenomatous Polyposis (FAP) are relatively uncommon in Gulf Arabs. Restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA) is a surgical method for treating patients with mucosal ulcerative colitis (MUC) and FAP. This paper documents a small experience with this operation in Saudi Arabia. METHODS: The charts of all patients who had either MUC or FAP and in whom an IPAA had been performed were identified and were examined in order to determine patient demographics, timing and staging of operation, operative and long term morbid

Backwash ileitis does not affect pouch outcome in patients with ulcerative colitis with restorative proctocolectomy.

BACKGROUND & AIMS: There has been controversy over the significance of active inflammation of the terminal ileum (also known as backwash ileitis) in patients with ulcerative colitis (UC) and idiopathic inflammatory bowel disease of indeterminate type for diagnosis and pouch construction. We investigated the impact of backwash ileitis on pouch outcome after restorative proctocolectomy with ileoanal pouch anastomosis. METHODS: Data from patients with backwash ileitis (n = 132) were compared with those from 132 matched controls without ileal inflammation for age, sex, and type of proctocolectomi

Short- and long-term outcomes of IPAA for ulcerative colitis in Latin America: A retrospective, multicentre assessment.

INTRODUCTION: Data on ileal pouch-anal anastomosis (IPAA) outcomes in ulcerative colitis (UC) patients from resource-constrained settings, such as Latin America, are limited. This study aimed to evaluate demographic characteristics, perioperative outcomes and long-term results of UC patients undergoing IPAA in Latin America. METHODS: A retrospective analysis was conducted on UC patients who underwent IPAA at 11 academic centres across Argentina, Brazil, Chile and Colombia between 2012 and 2022. Main outcome was 30-day postoperative complications. RESULTS: A total of 273 patients underwent IP

Ileal Paneth Cell Phenotype is a Cellular Biomarker for Pouch Complications in Ulcerative Colitis.

BACKGROUND & AIMS: Biomarkers that integrate genetic and environmental factors and predict outcome in complex immune diseases such as inflammatory bowel disease (IBD; including Crohn's disease [CD] and ulcerative colitis [UC]) are needed. We showed that morphologic patterns of ileal Paneth cells (Paneth cell phenotype [PCP]; a surrogate for PC function) is one such cellular biomarker for CD. Given the shared features between CD and UC, we hypothesized that PCP is also associated with molecular/genetic features and outcome in UC. Because PC density is highest in the ileum, we further hypothesiz

Assessment of the ileoanal pouch for the colorectal surgeon.

INTRODUCTION: Many pouch complications following ileoanal pouch surgery have an inflammatory or mechanical nature, and specialist colorectal surgeons are required to assess the anatomy of the ileoanal pouch in multiple settings. In this study, we report our stepwise clinical and endoscopic assessment of the patient with an ileoanal pouch. METHODS: The most common configuration of the ileoanal pouch is a J-pouch, and the stapled anastomosis is more frequently performed than a handsewn post-mucosectomy. A structured clinical and endoscopic assessment of the ileoanal pouch must provide informati
