Guidance for high-dose vitamin D supplementation for prolonging the honeymoon phase in children and adolescents with new-onset type 1 diabetes.

The publication of our recent randomized controlled trial (RCT) showing that vitamin D could protect the β-cells during the honeymoon phase of type 1 diabetes (T1D) has led to calls for guidance for vitamin D supplementation during the critical phase of type 1 diabetes. Prolonging the partial clinical remission (PR) phase of TID improves glycemic control and reduces long-term complications of T1D. This RCT randomized 36 children and adolescents to either receive vitamin D2 (ergocalciferol, given as 50,000 international units per week for 2 months and then every other week for 10 months) or a p

Hunting for mpox (monkeypox) mimickers: Use of the Biofire meningitis/encephalitis panel on lesion swabs to support alternative viral diagnoses.

BACKGROUND: Mpox (formerly monkeypox) is an emerging zoonotic disease of public health concern that presents as a rash mimicking other common viral exanthems. Unlike traditional testing algorithms relying on several assays, the BioFire FilmArray meningitis/encephalitis (ME) panel simultaneously detects common viruses causing rashes; however, Biofire ME is only licensed for testing on cerebral spinal fluid. OBJECTIVES: This study evaluated use of the Biofire ME panel for detection and discrimination of herpes simplex virus types 1 and 2 (HSV-1 and HSV-2), varicella zoster virus (VZV), human he

Kinetics of human infection with Kyasanur Forest Disease Virus.

BACKGROUND: Kyasanur Forest disease (KFD) was first reported in 1957 and became an emerging tick-borne viral disease of public health importance in India. However, very little is known about the host-virus interaction and pathogenesis of KFD in humans. This study described the presence, duration, and kinetics of KFDV RNA in body fluids in infected human cases. METHODOLOGY: We enrolled 76 laboratory-confirmed KFD individuals and followed them up in the study. We obtained serial samples of blood, throat swabs in viral transport medium (VTM), urine, stool, and semen during the acute and convales

Candida auris Isolates Resistant to Three Classes of Antifungal Medications - New York, 2019.

Candida auris is a globally emerging yeast that causes outbreaks in health care settings and is often resistant to one or more classes of antifungal medications (1). Cases of C. auris with resistance to all three classes of commonly prescribed antifungal drugs (pan-resistance) have been reported in multiple countries (1). C. auris has been identified in the United States since 2016; the largest number (427 of 911 [47%]) of confirmed clinical cases reported as of October 31, 2019, have been reported in New York, where C. auris was first detected in July 2016 (1,2). As of June 28, 2019, a total

Fentanyl and fentanyl analogs in the illicit stimulant supply: Results from U.S. drug seizure data, 2011-2016.

BACKGROUND: U.S. research examining the illicit drug supply remains rare even though the information could help reduce overdoses. Relatively little is known regarding how often opioids are found in stimulants and whether temporal and geographic trends exist. We examined trends in fentanyl-cocaine and fentanyl-methamphetamine combinations in the national illicit drug supply. METHODS: We analysed serial cross-sectional data from the National Forensic Laboratory Information System (NFLIS) collected between January 2011 and December 2016. We restricted the analysis to cocaine (N = 1,389,968) and

Novel antibiotics effective against gram-positive and -negative multi-resistant bacteria with limited resistance.

Antibiotics are a medical wonder, but an increasing frequency of resistance among most human pathogens is rendering them ineffective. If this trend continues, the consequences for public health and for the general community could be catastrophic. The current clinical pipeline, however, is very limited and is dominated by derivatives of established classes, the "me too" compounds. Here, we have exploited our recent identification of a bacterial toxin to transform it into antibiotics active on multidrug-resistant (MDR) gram-positive and -negative bacterial pathogens. We generated a new family of

Provocation studies in the diagnosis of occupational asthma.

Specific and nonspecific provocation studies, although not always essential for diagnosing OA, help confirm the diagnosis and identify the offending agent. Nonspecific bronchial challenge testing is used to detect airway hyperresponsiveness and to clarify the nature of the patient's symptoms. Pharmacologic bronchoconstrictor agents (eg, methacholine, histamine) most commonly are used for the challenge, but nonisotonic aerosols, exercise and hyperventilation also can show airway hyperresponsiveness. Nonspecific challenges usually are done in the laboratory, but can be done at the workplace if e

Association of waist circumference trajectories with incident chronic kidney disease.

BACKGROUND: Chronic kidney disease (CKD) is a major public health burden. Waist circumference (WC) is associated with CKD risk, but the prognostic significance of longitudinal WC change remains uncertain. We examined the association between WC trajectories and incident CKD in Korean adults. METHODS: This retrospective cohort study included 5,429 adults aged 40-69 years from the Korean Genome and Epidemiology Study Ansan-Ansung cohort who were free of CKD at baseline and during the 6-year exposure period. WC trajectories were classified as stable, increasing, or decreasing using Gaussian finit

Lower Extremity Salvage Through Dextranomer Debridement and Evolution Toward a Functional Pincer in a Diabetic Foot: Experience in Rural Surgery.

Necrotizing infections of the diabetic foot associated with peripheral vasculopathy remain a critical challenge in emergency and general surgical practice, particularly in low-resource, second-level rural hospitals that lack access to reconstructive subspecialties. We report the case of a 57-year-old man with type 2 diabetes mellitus and peripheral vascular disease who presented to a rural, second-level public hospital in Linares, Nuevo León, Mexico, with a deep, contaminated surgical wound following a prior amputation of the second and third toes of the left foot performed at another rural fa

Dissemination of antibiotic-resistant enterobacteriaceae from hospital wastewater in Gabon: implications for the one health strategy.

INTRODUCTION: The proliferation of Enterobacteriaceae exhibiting a multidrug-resistant phenotype in aquatic ecosystems is a major concern for global public health. In Gabon, the lack of adequate pre-treatment systems means that hospital wastewater acts as a critical reservoir associated with the spread of these pathogens into surface waters. This study aimed to characterise the bacterial load and phenotypic resistance profiles of Enterobacteriaceae isolated from the effluents of eleven Gabonese hospitals, whilst assessing the environmental factors associated with their presence. METHODOLOGY:

Intimate Partner Violence and Mental Well-Being Among Married Women Attending Psychiatric Outpatient Clinics: The Roles of Childhood Trauma and Self-Compassion.

Background/Objectives: Intimate partner violence (IPV) is a public health problem associated with adverse mental health outcomes among women. Although prior research has focused on psychological distress (PD), mental well-being (MWB) is a distinct dimension of mental health that has received less attention in IPV research, particularly in clinical samples. Childhood trauma (CT) and self-compassion (SC) may contribute to variability in IPV-related outcomes. This study examined associations between IPV and MWB, considering SC, PD, and CT within an integrated framework. Methods: In this cross-sec

Stent indwell duration and adverse events in endoscopic ultrasound-guided lumen-apposing metal stent drainage of peripancreatic collections: systematic review and meta-analysis.

BACKGROUND: In the drainage of peripancreatic fluid collections (PFCs), current practice recommends the removal of lumen-apposing metal stents (LAMSs) at 3-4 weeks, despite limited evidence. Many patients require longer indwell times (e.g. for serial necrosectomies or owing to logistical constraints). This study aimed to evaluate the incidence of adverse events (AEs) in patients undergoing PFC drainage with a LAMS, focusing on the timing of these events relative to the date of stent placement. METHOD: A systematic review with meta-analysis of prospective studies evaluating the use of LAMSs in

Independent association between acute kidney injury and kidney function trajectory.

INTRODUCTION: It is now universally accepted that acute kidney injury (AKI) is not a limited and entirely reversible condition as many previously thought. However, the independent association between mild to moderate AKI and subsequent kidney function trajectory remains poorly characterized because prior studies have not adequately controlled for pre-existing differences between patients who did or did not develop AKI. METHODS: Here, we employed linear mixed-effects regression models to examine the association between an episode of AKI and subsequent trajectory of estimated glomerular filtrat

Insertion sequences accelerate genomic convergence of multidrug resistance and hypervirulence in Klebsiella pneumoniae via capsular phase variation.

BACKGROUND: The convergence of resistance and hypervirulence in Klebsiella pneumoniae represents a significant public health threat, driven by the horizontal transfer of plasmids. Understanding factors affecting plasmid transfer efficiency is essential to elucidate mechanisms behind emergence of these formidable pathogens. METHODS: Hypermucoviscous K. pneumoniae strains were serially passaged in LB medium to investigate capsule-deficient phenotypes. Capsule-deficient mutants were analyzed using genetic sequencing to identify the types and insertion sites of insertion sequences (IS). Bioinform
