Biomechanical Comparison of 2 Common Techniques of Minimally Invasive Hallux Valgus Correction.

BACKGROUND: Hallux valgus is one of the most common surgically corrected forefoot deformities. Compared to open procedures, minimally invasive (MIS) treatment of hallux valgus has resulted in decreased operative time, reduced complication rates, and greater patient satisfaction. Historically, distal chevron osteotomies are the standard for moderate hallux valgus correction. To our knowledge, no studies have evaluated biomechanical strength of transverse and chevron distal first metatarsal osteotomy (DMO) constructs. The purpose of this study was to evaluate the biomechanical strength of these

Weightbearing CT Evaluation of Contact and Distance Mapping of the First Tarsometatarsal and Metatarsophalangeal Joints Following MIS Hallux Valgus Correction.

BackgroundThree-dimensional distance and coverage mapping (DM and CM) generated through weightbearing CT (WBCT) can aid in understanding the complex articular relationship in hallux valgus (HV). Minimally invasive (MIS) hallux valgus correction has become popular in correcting this deformity. Prior biomechanical studies have demonstrated that alterations occur at the first metatarsophalangeal (MTP) and tarsometatarsal (TMT) articulations following correction. The purpose of this study is to evaluate DM and CM characteristics at the first MTP and TMT joint as well as the medial metatarsosesamoi

Surgical Treatment of Moderate Hallux Valgus: A Comparison of Distal Chevron Metatarsal Osteotomy With and Without Lateral Soft-Tissue Release.

PURPOSE: The purpose of the study was to determine whether lateral soft-tissue release (LSTR) has a beneficial or detrimental effect on the outcome of distal Chevron first metatarsal osteotomy (DCMO) in the treatment of moderate hallux valgus (HV). METHODS: We compared the effect of different surgical treatments in 2 groups of patients: group I (23 patients, 25 feet, average age of 55 [from 43 to 77] years) was subjected to DCMO only, whereas group II (18 patients, 23 feet, average age of 59 [from 52 to 70] years]) was subjected to DCMO with LSTR. Complex Dorsal Dislocation of the Metacarpophalangeal Joint of the Fifth Finger Treated by Open Reduction With Volar Approach: A Case Report.

Complex dorsal dislocations of the metacarpophalangeal (MCP) joint are rare, with involvement of the fifth finger being exceedingly uncommon. These injuries typically result from hyperextension forces that entrap the volar plate, precluding successful closed reduction and necessitating surgical intervention. In this report, we present the case of a 20-year-old male patient who sustained multiple injuries, including craniofacial trauma and an open dorsal dislocation of the left fifth MCP joint, following a road traffic accident. Initial attempts at closed reduction under local anesthesia were u

Factors Associated With Hallux Valgus Progression in Patients With Rheumatoid Arthritis: Insights From Non-Weight-Bearing Radiographs in the KURAMA Cohort.

OBJECTIVES: This longitudinal study investigated the factors associated with hallux valgus (HV) progression in patients with rheumatoid arthritis (RA). METHODS: We included 316 patients with RA who underwent foot radiography in 2015 and more than four years later. The HV angle (HVA), first-second intermetatarsal angle (IMA), first-fifth intermetatarsal angle (M1/M5 angle), and calcaneal pitch (CP) angle were measured. Sesamoid position on a non-weight-bearing radiograph was divided into seven grades with reference to the Hardy classification. Factors of HV progression were examined. RESULTS:

Total sesamoidectomy for painful hallux rigidus: a medium-term outcome study.

BACKGROUND: The nature of the sesamoid complex in the development of hallux rigidus or limitus (HL) has been poorly characterized and the role of the sesamoids in the surgical management of this condition has not been explored. Previous surgical approaches in younger active patients unsuited to destructive procedures have been limited. MATERIALS AND METHODS: Thirty-three patients (36 procedures) were reviewed between 2 and 4 years following total sesamoidectomy for the management of hallux rigidus/limitus. The American Orthopedic Foot & Ankle Society hallux (AOFAS) clinical rating system was

Stress fractures of the foot - current evidence on management.

Stress fractures are a consequence of repeated submaximal loads with inadequate time for recovery and biologic repair or remodelling. The foot and ankle complex (FAC) represents a common site for development of stress fractures. Whilst the overall incidence of stress fractures is low, they are prevalent in athletes and military personnel causing significant time away from sports or work. Within these populations, certain stress fractures directly correlate to specific activities. Factors that commonly influence these fractures include an acute increase in new repetitive physical activity combi

Weightbearing CT 3-D Mapping of First Metatarsophalangeal and Sesamoid Joint Interactions in Hallux Valgus: A Case-Control Study.

BACKGROUND: Traditional methods of evaluating Hallux Valgus (HV) have limitations in assessing 3D deformity elements such as first metatarsal pronation and sesamoid subluxation. This study aimed to use coverage mapping (CM) to analyze surface interactions of the first metatarsophalangeal joint (MTPJ) and metatarsosesamoid joints in HV and control patients, correlating findings with visual analog scale (VAS) pain scores. We hypothesized that patients with HV would show decreased joint coverage, reflective of joint subluxation, and that these changes would correlate with higher VAS scores. METH

Sesamoidectomy for the treatment of chronic neuropathic ulcerations.

Neuropathic ulcerations in diabetic patients are frequent causes of hospitalizations and morbidity. The plantar aspect of the first metatarsophalangeal joint is a common location for these ulcerations, because of the significant weightbearing forces generated through this joint and the presence of sensory and motor neuropathy. The authors describe 24 cases in which excision of the tibial sesamoid, fibular sesamoid, or both, was performed to resolve these lesions.

Radiographic Results and Return to Activity After Sesamoidectomy for Fracture.

BACKGROUND: Limited data are available comparing the results of lateral sesamoidectomy and medial sesamoidectomy for the treatment of fractures recalcitrant to nonoperative treatment interventions. The hypothesis of this study was that sesamoidectomy for either lateral or medial sesamoid fractures would not change radiographic alignment of the first ray given the use of identical reconstruction of the plantar plate, intersesamoid ligament, and plantar ligament complex at the time of surgery. METHODS: This retrospective cohort study compared the outcomes of 46 consecutive patients treated with

Biomechanics of the first ray. Part II: Metatarsus primus varus as a cause of hypermobility. A three-dimensional kinematic analysis in a cadaver model.

Variation in functional stability of the first metatarsocuneiform joint was analyzed between transverse plane deviated (adducted) and corrected first metatarsal positions in a closed kinetic chain model. Six fresh frozen cadaver specimens with intact ankles and feet were fitted with a custom fabricated titanium metatarsal jig, which allowed for manipulation of the first metatarsal in the transverse plane. Specimens were mounted into a custom-made acrylic load frame and axially loaded to 400 N. Radiowave three-dimensional tracking transducers were attached to the following osseous segments: fir

Proximal oblique-domed osteotomy of the first metatarsal for the treatment of hallux valgus associate with flat foot: effect to the correction of the longitudinal arch of the foot.

INTRODUCTION: Flat foot and/or metatarsal primus varus are the major causes of hallux valgus, and it is important to correct these deformities in order to prevent the recurrence of this condition. We demonstrate the clinical and radiological assessment of the correction of hallux valgus, metatarsal primus varus, and flat foot after proximal oblique-domed osteotomy of the metatarsus with distal soft tissue reconstruction. MATERIALS AND METHODS: Twenty-seven feet of 22 patients with moderate or severe hallux valgus who had undergone proximal oblique-domed osteotomy were studied. After the adduc
