Detection and boundary identification of phonocardiogram sounds using an expert frequency-energy based metric.

This paper presents a new method to detect and to delineate phonocardiogram (PCG) sounds. Toward this objective, after preprocessing the PCG signal, two windows were moved on the preprocessed signal, and in each analysis window, two frequency-and amplitude-based features were calculated from the excerpted segment. Then, a synthetic decision making basis was devised by combining these two features for being used as an efficient detection-delineation decision statistic, (DS). Next, local extremums and locations of minimum slopes of the DS were determined by conducting forward-backward local inve

A framework for automatic heart sound analysis without segmentation.

BACKGROUND: A new framework for heart sound analysis is proposed. One of the most difficult processes in heart sound analysis is segmentation, due to interference form murmurs. METHOD: Equal number of cardiac cycles were extracted from heart sounds with different heart rates using information from envelopes of autocorrelation functions without the need to label individual fundamental heart sounds (FHS). The complete method consists of envelope detection, calculation of cardiac cycle lengths using auto-correlation of envelope signals, features extraction using discrete wavelet transform, princ

Discrete wavelet-aided delineation of PCG signal events via analysis of an area curve length-based decision statistic.

The aim of this study is to describe a robust unified framework for segmentation of the phonocardiogram (PCG) signal sounds based on the false-alarm probability (FAP) bounded segmentation of a properly calculated detection measure. To this end, first the original PCG signal is appropriately pre-processed and then, a fixed sample size sliding window is moved on the pre-processed signal. In each slid, the area under the excerpted segment is multiplied by its curve-length to generate the Area Curve Length (ACL) metric to be used as the segmentation decision statistic (DS). Afterwards, histogram p

Factors influencing cardiac auscultation proficiency in physician trainees.

INTRODUCTION: We assessed the accuracy of physician trainees in identifying different cardiac sounds and examined the factors influencing their cardiac auscultation proficiency. METHODS: A total of 106 physicians in the Family Medicine Training Programme were asked to identify 10 cardiac sounds played sequentially on the Littmann electronic stethoscope, which functioned as a surrogate patient. Their auscultation accuracy was scored numerically out of a maximum of 10. Demographical data of the physicians was collected prospectively. RESULTS: The mean (+/-SD) auscultation proficiency score of

Haemodynamic effects of acetazolamide in patients with cardiovascular disorders: correlation with calculated cerebral perfusion reserve.

Individual responses of cerebral blood flow to acetazolamide are highly variable. We hypothesized that this may be due to interactions with cardiovascular diseases and medication. Therefore, we measured the haemodynamic effects of acetazolamide in patients with various cardiovascular disorders and evaluated whether these had any correlation with calculated cerebral perfusion reserve. Ten male patients aged 58 +/- 9 years (mean +/- S.D.) were studied with first-pass radionuclide ventriculography and echocardiography before and 20 min after the administration of 1 g acetazolamide. Systolic blood

A comparison of the random-zero and standard mercury sphygmomanometers.

Both the standard mercury sphygmomanometer and the random-zero sphygmomanometer have been used in epidemiological studies and clinical trials. Problems arise in comparing studies since, in addition to other methodological differences, the readings obtained with the random-zero sphygmomanometer have been found to be lower than those obtained with the standard mercury sphygmomanometer. In the present study, blood pressures were measured in 66 subjects to examine the comparability of findings with the two instruments. Trained observers measured blood pressures simultaneously using a double-headed

[Aortic flow measurement by transesophageal Doppler effect].

Continuous measurement of cardiac output by thermodilution is invasive, impractical and unpleasant for the patient. We propose to measure descending aortic blood flow with a specially designed intra-oesophageal Doppler echo probe. The apparatus is composed of two main parts. First an A scan system makes possible the measurement of the diameter of the vessel, second a continuous wave velocimeter is used to measure the spatial mean velocity of the blood. An output calculator determines the descending aortic blood flow. The oesophageal catheter contains three ultrasonic transducers at its tip mou

Resting heart rate and pressure-rate product of children in a total biracial community: the Bogalusa Heart Study.

Resting heart rates were ascertained during the 1978-1979 school year in 3590 children aged 5-17 years in the biracial geographic population of Bogalusa, Louisiana. These rates were found to be relatively independent of the method of measurement, whether supine by physician's stethoscope or dressed sitting radial pulse taken by a nurse, and in either case adhering to a strict measurement protocol. Apart from the known influences of age and sex, the authors found a small but consistent racial influence, with whites having 3-4 beats/min higher rates than blacks. Controlling for age, the authors

Silica nanoparticles with encapsulated DNA (SPED) to trace the spread of pathogens in healthcare.

BACKGROUND: To establish effective infection control protocols, understanding pathogen transmission pathways is essential. Non-infectious surrogate tracers may safely explore these pathways and challenge pre-existing assumptions. We used silica nanoparticles with encapsulated DNA (SPED) for the first time in a real-life hospital setting to investigate potential transmission routes of vancomycin-resistant enterococci in the context of a prolonged outbreak. METHODS: The two study experiments took place in the 900-bed University Hospital Zurich, Switzerland. A three-run 'Patient experiment' inve

Capacity of Health Facilities to Manage Hypertension in Mukono and Buikwe Districts in Uganda: Challenges and Recommendations.

BACKGROUND: The burden of chronic diseases is increasing in both low- and middle-income countries. However, healthcare systems in low-income countries are inadequately equipped to deal with the growing disease burden, which requires chronic care for patients. The aim of this study was to assess the capacity of health facilities to manage hypertension in two districts in Uganda. METHODS: In a cross-sectional study conducted between June and October 2012, we surveyed 126 health facilities (6 hospitals, 4 Health Center IV (HCIV), 23 Health Center III (HCIII), 41 Health Center II (HCII) and 52 pr

Use of respiratory signal analysis to assess severity of Brachycephalic Obstructive Airway Syndrome (BOAS) in dogs.

Brachycephalic Obstructive Airway Syndrome (BOAS) is a potentially life-threatening condition that can be challenging to diagnose and grade objectively. The aim of this study was to investigate the use of respiratory signal analysis to assess severity of BOAS in dogs. Hundred and seventeen client-owned dogs of brachycephalic and non-brachycephalic breeds were enrolled. Respiratory sounds were recorded using an electronic stethoscope before and after a 3-minute exercise test (ET). Dogs were assigned a BOAS severity grade (BOAS 0-3) using a validated respiratory functional grading scheme. Signal

The application of blood flow sound contrastive learning to predict arteriovenous graft stenosis of patients with hemodialysis.

End-stage kidney disease (ESKD) presents a significant public health challenge, with hemodialysis (HD) remaining one of the most prevalent kidney replacement therapies. Ensuring the longevity and functionality of arteriovenous accesses is challenging for HD patients. Blood flow sound, which contains valuable information, has often been neglected in the past. However, machine learning offers a new approach, leveraging data non-invasively and learning autonomously to match the experience of healthcare professionas. This study aimed to devise a model for detecting arteriovenous grafts (AVGs) sten

Diagnosis of Coexisting Valvular Heart Diseases Using Image-to-Sequence Translation of Contact Microphone Recordings.

OBJECTIVE: Development of a contact microphone-driven screening framework for the diagnosis of coexisting valvular heart diseases (VHDs). METHODS: A sensitive accelerometer contact microphone (ACM) is employed to capture heart-induced acoustic components on the chest wall. Inspired by the human auditory system, ACM recordings are initially transformed into Mel-frequency cepstral coefficients (MFCCs) and their first and second derivatives, resulting in 3-channel images. An image-to-sequence translation network based on the convolution-meets-transformer (CMT) architecture is then applied to eac

Analysis of pulmonary sounds for the diagnosis of interstitial lung diseases secondary to rheumatoid arthritis.

The diagnosis of interstitial lung diseases in patients affected by rheumatoid arthritis is fundamental to improving their survival rate. In particular, the average survival time of patients affected by rheumatoid arthritis with pulmonary implications is approximately 3 years. The gold standard for confirming the diagnosis of this disease is computer tomography. However, it is very difficult to raise diagnosis suspicion because the symptoms of the disease are extremely common in elderly people. The detection of the so-called velcro crackle in lung sounds can effectively raise the suspicion of
