Does inclusion of neighborhood variables improve clinical risk prediction for advanced prostate cancer in Black and White men?

INTRODUCTION: Black men are diagnosed with high-grade prostate cancer (PCa; Gleason sum ≥7) at greater rates than White men. This persistent disparity has led to mortality rates among Black men that are twice the rate of White men. Risk prediction tools can aid clinical decision making for PCa screening, biopsy, and treatment. However, research has not integrated neighborhood-level risk factors that are associated with rates of high-grade PCa. This study sought to determine whether inclusion of neighborhood-level variables can improve prediction of high-grade PCa over the existing Prostate Can

Disparities in Cytomegalovirus Infection Rates by Race and Ethnicity among Pediatric Allogeneic Hematopoietic Cell Transplantation Recipients at a Single Center.

Previous literature has reported cytomegalovirus (CMV) infection rate disparities among racial/ethnic groups of hematopoietic cell transplantation (HCT) recipients. Because race and ethnicity categorizations are social constructs unlikely to affect biological systems, it is likely there are covariates on the pathway to CMV detection, known as mediators, that can explain the observed disparity. Recent developments in mediation analysis methods enable the analysis of time-to-event outcomes, allowing an investigation of these disparities to also consider the timing of CMV infection detection rela

How the War on Drugs Impacts Social Determinants of Health Beyond the Criminal Legal System.

UNLABELLED: There is a growing recognition in the fields of public health and medicine that social determinants of health (SDOH) play a key role in driving health inequities and disparities among various groups, such that a focus upon individual-level medical interventions will have limited effects without the consideration of the macro-level factors that dictate how effectively individuals can manage their health. While the health impacts of mass incarceration have been explored, less attention has been paid to how the "war on drugs" in the United States exacerbates many of the factors that n

Ethnicity classification systems for public health surveys: experiences from HIV behavioural surveillance among men who have sex with men.

BACKGROUND: Race and ethnicity classification systems have considerable implications for public health, including the potential to reveal or mask inequities. Given increasing "super-diversity" and multiple racial/ethnic identities in many global settings, especially among younger generations, different ethnicity classification systems can underrepresent population heterogeneity and can misallocate and render invisible Indigenous people and ethnic minorities. We investigated three ethnicity classification methods and their relationship to sample size, socio-demographics and sexual health indica

Assessment of Pediatric Residents in the United States by Sex, Ethnoracial and Intersectional Group.

INTRODUCTION: Inequitable assessments in medical education disproportionately affect historically marginalized trainees and undermine development of a diverse physician workforce. With programs around the world incorporating competency-based medical education (CBME), understanding whether assessment disparities exist in specialty-specific competency frameworks is critical. This study examined disparities in program-reported performance assessments of pediatric residents across demographic groups in two pediatric-specific competency frameworks: the General Pediatrics Entrustable Professional Ac

'Maybe you should have a bowl of ice cream': Inequities in patient-clinician interactions among individuals with chronic low back pain.

Prior literature has shown inequities in patient-clinician interactions experienced by individuals with chronic low back pain (CLBP) with underlying pain-related stigmatization and invalidation. Yet, there is a notable gap in understanding how these inequities intersect with multiple systems of oppression, including racism and sexism. This qualitative study examined intersectional perspectives and experiences of patient-clinician interactions among individuals with CLBP. Semi-structured interviews were conducted after the participants engaged in simulated enhanced or limited patient-clinician

Decolonising Healthcare Through Curricula: Integrating Indigenous Knowledges and Person-Centred Practice Through the Integrated Indigenous and Person-Centred Practice Theory and Framework.

AIM AND DESIGN: Indigenous knowledge systems offer enduring, relational and strengths-based frameworks for health and wellbeing. This theory and framework paper critically analyses how colonial legacies embedded in nursing curricula marginalise Indigenous knowledges and perpetuate inequities for Aboriginal and Torres Strait Islander peoples. We argue that current approaches, often fragmented, tokenistic and additive in nature, fail to address the structural and pedagogical roots of unsafe cultural practices and institutional racism in nursing education. RESULTS: In response, we present the In

Harm reduction, methadone maintenance treatment and the root causes of health and social inequities: An intersectional lens in the Canadian context.

BACKGROUND: Using our research findings, we explore Harm Reduction and Methadone Maintenance Treatment (MMT) using an intersectional lens to provide a more complex understanding of Harm Reduction and MMT, particularly how Harm Reduction and MMT are experienced differently by people dependent on how they are positioned. Using the lens of intersectionality, we refine the notion of Harm Reduction by specifying the conditions in which both harm and benefit arise and how experiences of harm are continuous with wider experiences of domination and oppression; METHODS: A qualitative design that uses

Maternal Mortality Among Black Women in Brazil: A Retrospective Cohort Study.

BACKGROUND: Maternal mortality in Brazil remains a critical indicator of social and racial inequalities, reflecting structural failures in access to and quality of obstetric care. Black women, particularly those categorized as black or brown, are at a higher risk of dying during pregnancy, childbirth, or the postpartum period. This is the result of the intersection of institutional racism, poverty, and social vulnerabilities. This study aimed to analyze trends and associated factors of maternal mortality among black women in Brazil from 2000 to 2020. METHODS: This is a retrospective cohort an

A Qualitative Study of the Initial Diagnosis and Treatment Experience of Black Patients with Newly Diagnosed Acute Myeloid Leukemia.

PURPOSE: Black patients with acute myeloid leukemia (AML) experience disparate outcomes, and factors such as structural racism and differential treatment access have been implicated. We sought to understand the experience of Black patients with newly diagnosed AML to gain insight into their unique experience. METHODS: We interviewed Black patients ≥18 years of age with newly diagnosed AML (diagnosed from 7/1/2021 to 7/31/2022) over Zoom. Interviews were conducted by the same interviewer, with concordance in race between the interviewer and interviewees. A rapid qualitative content analysis wa

A Novel Billing-Based Approach to Measuring Receipt of Specialty Palliative Care Consultation.

BACKGROUND: Specialty palliative care improves quality of life, symptoms, and goal-concordant care, with greatest benefits when delivered at "full dose" (appropriate timing, intensity, and clinician expertise). Yet access is inequitable, particularly for racial and ethnic minority patients treated at minority-serving institutions (MSIs). Limited methods for identifying palliative care consultations in national datasets hinder progress in studying disparities. METHODS: We developed a novel billing-based approach to identify specialty palliative care consultations by linking three data sources

Violence perpetration prevalence among Colorado (United States) high school students across gender, racial/ethnic, and sexual identities.

Adolescent violence, including sexual violence, homophobic name-calling, and teen dating violence, are public health problems that cause harm to many adolescents in the United States. Although research on the perpetration of these forms of adolescent violence has increased in recent years, little is known about perpetration rates across gender, race/ethnicity, and sexual orientation. To address this gap, the current study descriptively examined perpetration rates between and across different identities, including self-identified race/ethnicity, sexual identity, and gender identity. In Fall 201

Racial/Ethnic and Sexual Orientation Identity Differences in the Receipt of a Sexual History Assessment from a Health Care Provider among Women in the United States.

BACKGROUND: Differential sexual history assessment, whereby certain groups are more or less likely to be asked questions about their sexual behavior by a health care provider, may lead to differential sexual health care and counseling. METHODS: Using nationally representative data from the 2013 through 2019 waves of the National Survey of Family Growth, we examined racial/ethnic and sexual orientation identity differences in receiving a sexual history assessment from a health care provider in the last 12 months among U.S. women aged 15-44 years (N = 14,019). Perceptions of intersectional stigma among diverse women living with HIV in the United States.

Attitudes and behavior that devalue individuals based upon their HIV status (HIV-related stigma) are barriers to HIV prevention, treatment, and wellbeing among women living with HIV. Other coexisting forms of stigma (e.g., racism, sexism) may worsen the effects of HIV-related stigma, and may contribute to persistent racial and gendered disparities in HIV prevention and treatment. Few studies examine perceptions of intersectional stigma among women living with HIV. From June to December 2015, we conducted 76 qualitative interviews with diverse women living with HIV from varied socioeconomic bac

Overdose response centering inequity and diversity study: a protocol for assessing the population-level and equity impact of the emergency medical services system changes using critical race theory.

BACKGROUND: Structural racism continues to drive racial disparities in opioid-related deaths by creating inequitable access to healthcare, shaping prescription practices, limiting availability of culturally responsive care, and concentrating socioeconomic disadvantage in racial/ethnic minority communities. Emergency Medical Services (EMS) based interventions provide a critical opportunity to address these disparities at the frontlines of care, as minoritized communities often utilize EMS as their usual source of care. In King County, Washington, EMS has begun implementing several system change

Decolonising audiology education: Epistemic barriers and opportunities for Black African students.

BACKGROUND: In South Africa, the Audiology curriculum remains rooted in Eurocentric epistemologies that do not reflect the lived realities, languages and cultural knowledge systems of Black African First-Language Speaking (BAFLS) students. This study responds to national calls for epistemic transformation in health sciences education, including Audiology. OBJECTIVES: This study examined the epistemological challenges encountered by BAFLS Audiology students at the University of Interest (UoI). Our objectives were to explore how the BAFLS students experience accessing and navigating the undergr

Associations between intersectional discrimination, self-efficacy to manage one's health, and health outcomes among Black transgender women.

BACKGROUND: Black transgender women are disproportionately exposed to discrimination and healthcare barriers associated with poor health and delayed treatment-seeking. Consequently, traditional treatment pathways may not be accessible. Thus, health self-management (HSM) may be critical in promoting health among Black transgender women. PURPOSE: The purpose of this study is to examine the relationships among intersectional discrimination, self-efficacy for health management, and mental health outcomes (PTSD symptoms, depressive symptoms, and unhealthy days) among Black transgender women, and t
