Unexpected soft tissue changes in response to root coverage using an acellular dermal matrix allograft: 12–year follow up.

A woman undergoing orthodontic treatment presented with recession and reduced keratinized gingiva on teeth 31 and 41. The patient declined creation of a donor site for conventional autogenous connective soft tissue grafting and opted for an acellular dermal matrix soft tissue substitute for root coverage. Orthodontic treatment followed, and the patient returned for orthognatic surgery after 12 years. Long-term follow up revealed that root coverage remained stable over time and creeping attachment on both teeth was observed. Unexpectedly, an increase in the width of keratinized gingiva was obse

Penile reconstruction with dermal template and vacuum therapy in severe skin and soft tissue defects caused by Fournier's gangrene and hidradenitis suppurativa.

The aim of this article is to improve the treatment of patients with complete skin loss of the penile shaft after Fournier's gangrene or hidradenitis suppurativa using modern biomatrices and topical negative pressure therapy. From January 2010 to December 2011, three patients with Fournier's gangrene or hidradenitis suppurativa were treated. After initial radical debridements, topical negative pressure therapy was applied for wound stabilisation. After that dermal templates (acellular dermal matrix) were used to achieve early healing and topical negative pressure-dressing in a special setting

Comparative evaluation of the effectiveness of coronally positioned flap with or without acellular dermal matrix allograft in the treatment of multiple marginal gingival recession defects.

Gingival recession remains an important problem in dental esthetics. Various surgical techniques have been proposed for treating multiple gingival recessions. The objective of this study was to evaluate and compare the effectiveness of a coronally positioned flap (CPF) with or without acellular dermal matrix allograft (ADMA) in the treatment of multiple marginal tissue recession. Twenty patients with a mean age of 31.6 years presented with 43 buccal/labial multiple recession defects (Miller Class I/II). Ten patients each were randomly assigned to one of two treatment groups: group 1 (ADMA) or

The use of human acellular dermal matrix in the prevention of infra-auricular depressed deformities and Frey's syndrome following total parotidectomy.

BACKGROUND: The aim of the study was to explore whether human acellular dermal matrix (ADM) implantation could prevent infra-auricular depressed deformities and Frey's syndrome following total parotidectomy. METHODS: We analyzed 41 cases of total parotidectomy (control group) and 29 cases of simultaneous ADM implantation following total parotidectomy (ADM group) for infra-auricular depressed deformities and the presence of gustatory flushing or sweating. The follow-up periods ranged from 6 months to 7 years. RESULTS: Facial contours and bilateral symmetry improved after surgery in the ADM gr

Longitudinal Assessment of Facial Scars after Acellular Dermal Matrix Grafting following Skin Cancer Resection: Focus on Color and Contraction.

BACKGROUND: Skin cancer commonly affects the face, presenting significant reconstructive challenges, particularly for older patients, who may benefit from minimally invasive techniques. The authors' team used acellular dermal matrix (ADM), as this technique provides a relatively simple alternative that reduces the complexity associated with autologous graft surgery. The objective of this study was to assess changes in color matching and the extent of scar contraction over time with this method. METHODS: This retrospective study assessed 153 ADM grafts applied after skin cancer resection on th

What did we catch? Predictors of infection after tissue expander-based breast reconstruction in a safety-net system.

BACKGROUND: Infection is a common complication following tissue expander (TE)-based breast reconstruction. Few studies have examined risk factors specifically in the unique populations encountered at safety-net hospitals. The purpose of this study was to identify predictors of TE infection at Harris Health safety-net hospitals, which serve the third most populous county in the United States. METHODS: A retrospective review was performed to evaluate women over the age of 18 years who underwent TE placement at two safety-net hospitals from October 2015 to November 2022. Demographic information,

Comparison of clinical efficacy between reconstruction of the superior acromioclavicular ligament with acellular dermal allografts and clavicular hook plate in acromioclavicular dislocations.

PURPOSES: We aimed to compare the clinical efficacy of superior acromioclavicular ligament reconstruction (SALR) using acellular dermal allograft with that of clavicular hook plate fixation (HP) in patients with acromioclavicular (AC) dislocations. We hypothesized that the SALR could provide more stability than hook plate. METHODS: Twenty-two cases of acute AC joint dislocation between November 2021 to December 2023 were retrospectively reviewed. All patients were divided into 2 groups based on the treatment with SALR (12 cases) or HP (10 cases). Patients were evaluated radiologically and cli

Long-Term Outcomes following Hybrid Breast Reconstruction.

BACKGROUND: Hybrid breast reconstruction combines free tissue transfer with implant placement. Various mesh products have been successfully used to secure the implant position in these reconstructions. In this study, the authors investigate the impact of mesh type on long-term outcomes following hybrid breast reconstruction. METHODS: A retrospective analysis of all patients with at least 24 months of follow-up after immediate bilateral prepectoral hybrid breast reconstruction was performed. Univariate and multivariable regression analyses were used to evaluate long-term outcomes and compare b

Decreased hernia recurrence using autologous platelet-rich plasma (PRP) with Strattice™ mesh in a rodent ventral hernia model.

BACKGROUND: Recurrence after ventral hernia repair (VHR) remains a multifactorial problem still plaguing surgeons today. Some of the many contributing factors include mechanical strain, poor tissue-mesh integration, and degradation of matrices. The high recurrence rate witnessed with the use of acellular dermal matrices (ADM) for definitive hernia repair has reduced their use largely to bridging repair and breast reconstruction. Modalities that improve classic cellular metrics of successful VHR could theoretically result in improved rates of hernia recurrence; autologous platelet-rich plasma (

Autologous skin substitute for hard-to-heal ulcers: retrospective analysis on safety, applicability, and efficacy in an outpatient and hospitalized setting.

Chronic ulcers ((arterio)venous, decubitus, or postoperative) have no tendency to heal within a period of at least 3 months despite optimal therapy according to internationally accepted guidelines. This retrospective study evaluates the safety and efficacy of an autologous, dermal-epidermal skin substitute (SS) for treating ulcers of various origins. Ulcers were treated within 7 Dutch centers over 5 years. Sixty-six ulcers (size: 0.75-150 cm²; duration: 0.25-32 years) with a follow-up time of 24 weeks after a single-skin substitute application were assessed. Wound-bed preparation consisted of

Decellularization of porcine dermis via supercritical CO2-assisted co-solvent system for cutaneous wound healing.

Efficient generation of bioactive and structurally preserved decellularized extracellular matrix (dECM) is critical for regenerative medicine applications. Conventional decellularization techniques, however, frequently utilize detergents such as sodium dodecyl sulfate and Triton X-100, which may compromise ECM integrity, diminish retention of bioactive components and impair mechanical properties. To address these limitations, we present a supercritical CO2 (scCO2)-assisted co-solvent strategy for efficient decellularization of porcine dermis, yielding a bioactive acellular dermal matrix (ADM)

Human dermal allograft augmentation in primary and revision arthroscopic rotator cuff repair: a retrospective controlled study including patient outcomes and ultrasound evaluation of tendon healing.

PURPOSE: Rotator cuff (RC) tears are common in older adults, often leading to muscle atrophy. Standard arthroscopic repair has high re-tear rates, prompting the use of biological patches for augmentation. This study assessed differences in range of motion, strength, and tendon healing using ultrasound in primary and revision RC repairs using acellular dermal allograft augmentation. METHODS: Forty-eight patients undergoing arthroscopic RC repair with dermal allograft augmentation were assessed, with 42 completing a median follow-up of 32.4 months. Twenty patients had primary repairs and 22 had

Closing the gap: Healing acute complex wounds using an acellular dermal substitute - a prospective case series.

BACKGROUND: Although various dermal substitutes are available, their use in the acute setting or for extensive surface areas is often constrained by high costs and labor-intensive application procedures. Despite these challenges, they have proven effective in treating different skin and soft tissue defects. METHODS: In this prospective case series, 26 adults with acute complex deep soft tissue defects resulting from different etiologies were treated with Glyaderm. Glyaderm is a low-cost acellular dermal substitute and can be applied in either a one- or two-stage procedure with a split skin gr

Combined use of composite mesh and acellular dermal matrix graft for abdominal wall repair following tumour resection.

BACKGROUND: Surgeries for sarcomas in the abdominal wall require wide resections, often radical en bloc resections, which generate major defects involving a very complex repair. The combined use of porcine dermal xenografts, together with composite meshes, may assist in the repair of these defects with minimal complications. METHOD: We present a series of 19 patients (10 males and 9 females), with a mean age of 53.2 years (range: 11-86 years) treated in the Sarcoma Unit of the Virgen de la Arrixaca University Hospital from January 2015 to December 2021.
