Lane heading difference: An innovative model for drowsy driving detection using retrospective analysis around curves.

Driving while sleepy is a serious contributor to automobile accidents. Previous research has shown that drowsy drivers produce systematic errors (variability) in vehicle behavior which are detectable using vehicle monitoring technology. The current study developed a new methodological approach using a vehicle heading difference metric to detect drowsy driving more effectively than other more commonly used methods. Twenty participants completed a driving scenario as well as several measures of fatigue in five testing sessions across a night of sleep deprivation. Each simulated highway driving s

Driving after stroke: A trichotomous logistic regression model to support decision making in uncertain cases.

BACKGROUND: Assessing fitness to drive after stroke is a complex clinical task, as even mild cognitive deficits can undermine safety. Due to the substantial overlap in cognitive test results between safe and unsafe drivers, binary classification models inevitably carry a risk of misclassification. This study aimed to develop and validate a logistic regression model that introduces a third, indeterminate category - leaving room for clinicians to withhold judgment in uncertain cases and thereby support more cautious, evidence-based decisions. METHODS: A total of 115 stroke survivors underwent a

Forty- versus 20-minute trials of the maintenance of wakefulness test regimen for licensing of drivers.

STUDY OBJECTIVES: Objective assessment of the ability to maintain wakefulness, although very important, is still equivocal. A recent study from our lab has shown that the Maintenance of Wakefulness Test (MWT), when performed with the 20-minute protocol (MWT20), is unreliable in assessing patients who are highly motivated to maintain wakefulness. In this study, we sought to examine whether the 40-minute protocol (MWT40) is a better tool in assessing such individuals. METHODS: One hundred sixty-four consecutive subjects referred to our sleep lab by the Medical Institute for Driving Safety were

Assessment and policy for commercial driver license referrals.

This report describes experiences, subsequent action, and policy recommendations regarding sleep disorders assessment of veterans in relation to a commercial driver medical examiner referral. A series of 6 veterans were seen in our sleep clinic, presenting with an order from a commercial driver medical examiner (CDME) for polysomnography and/or Multiple Sleep Latency Testing (PSG/MSLT). We searched the literature for an evidence-based justification for handling this referral, and we concluded that there is neither federal policy nor current evidence to suggest that any current diagnostic test,

[Treatment of adult patients with epilepsy in the children's hospital].

During one year from November 2002 to October 2003, 810 patients with epilepsy were examined in Shiga Medical Center for Children, and 186 patients (23%) were over 18 years old. Among them, 162 adult patients (male; 52%, female; 48%) were regularly examined at the outpatient-clinic. Seizures persisted in 56% of these adult patients. Over 80% of the patients were classified as symptomatic epilepsy. Seventy-eight percent of the cases were combined with physical or mental disability. Six percent of the patients had psychological disorders and 11% of the patients had internal diseases. Fifteen per

Does license disqualification reduce reoffence rates?

A review was conducted of the subsequent driving records of over 25,000 Queensland drivers cited for a drink driving offence in 1988 who received at least one subsequent license restriction. The interval of follow-up was at least 3 years, average 3.9 years. Periods of driving disqualification were identified and, for each driver, the total amount of time during restricted and unrestricted driving was computed; the numbers of events, i.e. crashes and traffic offences, recorded during these periods were counted. Rates under disqualification and during legal driving, expressed per thousand person

Longitudinal Outcomes of Cumulative Impact Exposure on Oculomotor Functioning in Professional Motorsport Drivers.

IMPORTANCE: Professional motorsport drivers are regularly exposed to biomechanical forces comparable with those experienced by contact and collision sport athletes, and little is known about the potential short-term and long-term neurologic sequelae. OBJECTIVE: To determine whether cumulative impact exposure is associated with oculomotor functioning in motorsport drivers from the INDYCAR professional open-wheel automobile racing series. DESIGN, SETTING, AND PARTICIPANTS: This is a longitudinal retrospective cohort study conducted across 3 racing seasons (2017-2019). Statistical analyses were

Evidence from a cohort of able bodied adults to support the need for driver training for motorized scooters before community participation.

OBJECTIVE: This study sought to utilize the implementation of a new competency test in order to define skills required to safely drive a motorized scooter. This test endeavours to reduce the number of driving and pedestrian related accidents, by determining an acceptable level of driver skill and awareness. METHODS: Healthy subjects, who might at some time use a motorized scooter for mobility, were recruited from the local community. Each undertook a driver competency test including basic driving skills, traffic and multiple tasks. Ten subjects repeated the test three times to determine pract

Older drivers and cataract: driving habits and crash risk.

BACKGROUND: Cataract is a leading cause of vision impairment in older adults, affecting almost half of those over age 75 years. Driving is a highly visual task and, as with other age groups, older adults rely on the personal automobile for travel. The purpose of this study was to examine the role of cataract in driving. METHODS: Older adults (aged 55-85 years) with cataract (n = 279) and those without cataract (n = 105) who were legally licensed to drive were recruited from eye clinics to participate in a driving habits interview to assess driving status, exposure, difficulty, and "space" (th

Persistence of effects of the Checkpoints program on parental restrictions of teen driving privileges.

OBJECTIVES: We describe intervention effects on parent limits on novice teenage driving. METHODS: We recruited parents and their 16-year-old children (n = 469) with learner's permits and randomized them from August 2000 to March 2003. Intervention families received persuasive newsletters related to high-risk teenage driving and a parent-teenager driving agreement; comparison families received standard information on driver safety. We conducted interviews when the adolescents obtained a learner's permit, upon licensure, and at 3, 6, and 12 months postlicensure. RESULTS: Intervention parents a

[The mechanism of injuring of front-seat passengers in head-on motor vehicle collisions: forensic issues].

Forensic pathologists sometimes need to determine seating positions of automobile occupants after road traffic accidents as accurately as possible. That could be of essential significance particularly in regard to the question of guilt both in the criminal and civil law. So far, medical surveys have implied the specific injury which would undoubtedly point at the allocation of the seating positions of fatally injured car occupant. Some of the injuries could occur by both direct and indirect force action. Same type of injury of the specific body region in both drivers and front seat passengers

[Medicolegal and compensation scientific approach to automobile accident].

Multiple injuries are frequently observed over the whole body of traffic victims in medico-legal autopsy cases. The assessment of a traffic casualty must include not only the victim but also the vehicle and the circumstances of the accident. Only consideration of all available data permits a better assessment of the mechanism of the crash and causation of injuries. J. M. Thevenet drove the first car carried from France to Japan on February 6, 1898. On October 28th, 1905, the first death by a road traffic accident occurred in Osaka. We performed a retrospective analysis of 279 traffic fatalitie

Occupational and nonoccupational correlates of alcohol consumption in urban transit operators.

BACKGROUND: The influence of occupation and the worksite has emerged as an important area of study in research on alcohol consumption. Occupational and nonoccupational factors were studied in relation to alcohol consumption using data from a 1983-1985 cross-sectional study of transit operators. METHODS: A total of 1.853 operators underwent a medical examination for driver's license renewal (including information on age, ethnicity, gender, education). Of these operators, 1,448 completed a questionnaire about occupational (e.g., time of shift, job stressors) and nonoccupational (e.g., personali

Implications of vision testing for older driver licensing.

OBJECTIVES: A range of medical conditions can lead to visual impairments either through effects on the eye or the visual pathways and brain. The prevalence of visual impairment increases with age. Furthermore, research evidence has shown that aging is related to a number of other processes that can lead to changes in cognitive functioning and sensory perception that may adversely affect driving (Marottoli and Drickamer, 1993; Stelmach and Nahom, 1992). This combination of factors is thought to contribute to older driver crash risk (Hakamies-Blomqvist, 1993; Stutts et al., 1998). Licensing auth

Characteristics of 167 consecutive traffic accident victims with special reference to alcohol intoxication: a prospective emergency room study.

This prospective study examined the characteristics of 167 consecutive traffic accident victims admitted to the emergency room of a major Swiss hospital with particular attention to the presence of a detectable blood alcohol concentration (BAC). The majority of the study population were male (71%), 16-29 years of age (56%) and occupants of an automobile or motorcycle (70%). Most patients were injured during the daytime, with nighttime accidents increasing towards the end of the week. Seventy-five percent of the injured were drivers of the crash vehicle and the majority (56%) were involved in m

Driver licensing: descriptive epidemiology of a social determinant of Aboriginal and Torres Strait Islander health.

OBJECTIVE: Education, employment and equitable access to services are commonly accepted as important underlying social determinants of health. For most Australians, access to health, education and other services is facilitated by private transport and a driver licence. This study aimed to examine licensing rates and predictors of licensing in a sample of Aboriginal and Torres Strait Islander people, as these have previously been poorly described. METHODS: Interviewer-administered surveys were conducted with 625 people 16 years or older in four Aboriginal Community Controlled Health Services i
