Here, we report a case of hereditary hyperhomocysteinemia presenting as dilated cardiomyopathy which was successfully diagnosed using a combination of point-of-care ultrasonography (POCUS) and echocardiogram (ECHO). A 39-year-old Caucasian male with a family history of homocystinuria and early deaths in adult male members from cardiovascular disease presented with complaints of purplish discoloration and 4/10 pain in bilateral feet along with severe nausea/vomiting for the last two days. Physical examination was significant for tachycardia, low normal mean arterial pressures, dry mucous membra

In spite of the growing interest in rodents with subterranean activity in general and the spalacids (Spalacidae) in particular, little is known about the biology of most members of this clade, such as the Chinese bamboo rat (Rhizomys sinensis). Here, we analyzed the ear morphology of R. sinensis with respect to hearing specialization for subterranean or aboveground modes of communication. It is well-known that ecology and style of life of a particular species can be reflected in morphology of its ear, its hearing and vocalization, so we expect that such information could provide us insight int

This review covers the basic anatomy and physiology of the olivocochlear reflexes and the use of otoacoustic emissions (OAEs) in humans to monitor the effects of one group, the medial olivocochlear (MOC) efferents. MOC fibers synapse on outer hair cells (OHCs), and activation of these fibers inhibits basilar membrane responses to low-level sounds. This MOC-induced decrease in the gain of the cochlear amplifier is reflected in changes in OAEs. Any OAE can be used to monitor MOC effects on the cochlear amplifier. Each OAE type has its own advantages and disadvantages. The most straightforward te

To examine the relationship between hearing and changes in the inner ear, we investigated human temporal bone specimens from 2 patients with noise-induced hearing loss and prepared audio-cytocochleograms as described by Schuknecht et al. Patient 1 was a 50-year-old male who died of thyroid cancer and had worked at a printing house for 38 years. Patient 2 was a 58-year old male who died of maxillary sinus cancer and had worked in construction for 22 years. A pure-tone audiogram showed high-tone sensorineural hearing loss with c5-dip-type hearing disorder in both ears in Patient 1, and a high-to

Low-frequency sounds presented at high nontraumatizing levels induce temporary hyperacusis in humans and animals. One explanation of this finding is that the basilar membrane operating point may be disturbed by an endolymph volume change. This possibility was investigated using volume and flow markers iontophoresed into the endolymphatic space of guinea pigs. Marker concentrations were measured with ion-selective microelectrodes placed apically and basally to the iontophoresis site during exposure of the ear to low-frequency tones. Concentration changes were interpreted quantitatively using a

The indication for external ventricular drainage after clipping of ruptured aneurysms in the acute stage is evaluated. Since 1990, 234 patients who presented with subarachnoid hemorrhage (SAH) and underwent aneurysmal clipping in the acute stage were evaluated retrospectively. Patients with ventricular dilatation had intraoperative placement of a ventriculostomy to obtain intraoperative brain relaxation, but the ventricular catheter was removed at the end of the surgery. All these patients were managed postoperatively in a similar fashion with no ventricular drainage, avoidance of dehydration

Prolonged vasospasm was produced in the canine basilar artery by injection of 0.75 ml/kg of fresh autologous arterial blood into the cisterna magna (subarachnoid hemorrhage, SAH group) or by subarachnoid application of oxyhemoglobin (oxyHb induced group). Lipoperoxide contents of the arterial wall was measured by thiobarbituric acid test. Prostaglandin I2 (PGI2) and thromboxane A2 (TXA2) biosynthetic activity of arterial wall was measured by radioimmunoassay as their stable metabolites, 6-keto-prostaglandin F1 alpha, thromboxane B2, respectively. Ultrastructual examination was carried out to r

Tone complexes with positive (m+) and negative (m-) Schroeder phase show large differences in masking efficiency. This study investigated whether the different phase characteristics also affect loudness. Loudness matches between m+ and m- complexes were measured as a function of (1) the fundamental frequency (f0) for different frequency bands in normal-hearing and hearing-impaired subjects, and (2) intensity level in normal-hearing subjects. In normal-hearing subjects, the level of the m+ stimulus was up to 10 dB higher than that of the corresponding m- stimulus at the point of equal loudness.

BACKGROUND: The inner ear responds to acute impact of high-intensity noise with a temporary threshold shift (TTS), which represents a reaction of the outer hair cells. That is why TTS should be detectable as changes of distortion-product otoacoustic emissions (DPOAE).
METHODS: DPOAE were measured in 102 ears in 59 test subjects with normal hearing before and after definitive noise exposure (20 min of "white noise" at 90 dB HL). We also registrated the TTS (4kHz) immediately after noise and measured DP changes during a 30-min recovery period.
RESULTS: Typically we found a reduction of DP ampl

Hair cell stereocilia are composed of packed actin filaments, oriented such that the preferred end for the addition of actin monomers is at the tips of the sterocilia. It has therefore been suggested that when stereocilia grow, they do so from their tips (Tilney and DeRosier, 1986, Dev. Biol. 116, 119-129). In order to test the hypothesis, radiolabelled amino acids were applied to the air-sac of chicken eggs at day 17 of incubation, i.e., at the beginning of a phase in which the stereocilia have achieved their mature width, but are growing rapidly in length. Incorporation of radiolabel was stu

Background: It is assumed that speech comprehension deficits in background noise are caused by age-related or acquired hearing loss. Methods: We examined young, middle-aged, and older individuals with and without hearing threshold loss using pure-tone (PT) audiometry, short-pulsed distortion-product otoacoustic emissions (pDPOAEs), auditory brainstem responses (ABRs), auditory steady-state responses (ASSRs), speech comprehension (OLSA), and syllable discrimination in quiet and noise. Results: A noticeable decline of hearing sensitivity in extended high-frequency regions and its influence on lo

OBJECTIVE: To determine if decreased Cu/Zn superoxide dismutase (SOD1) levels in inner pillar cells is associated and diminished inner hair cell ribbon synapse plasticity in presbycusis.
METHODS: We evaluated the auditory brainstem responses (ABRs) of 2-, 5-, 6-, and 7-month-old C57BL/6J mice. ABRs were obtained using clicks and 4-, 12-, and 32-kHz tone bursts. Cochleae were collected immediately after audiometric assessment for Western blot analysis. The inner and outer hair cells and the inner hair cell ribbon synapses were separately counted. Frozen tissue sections were exposed to immunofl

BACKGROUND AND PURPOSE: The vasoconstrictor endothelin-1 (ET-1) has been implicated in the pathogenesis of cerebral vasospasm following subarachnoid hemorrhage (SAH). Previous results showed that CGS 26303, an endothelin converting enzyme (ECE) inhibitor, effectively prevented and reversed arterial narrowing in animal models of SAH. In the present study, we assessed the effect of CGS 26303 on neurological deficits in SAH rats. The involvement of vasoactive pathways downstream of ET-1 signaling in SAH was also investigated.
METHODS: Sprague-Dawley rats were divided into five groups (n = 6/grou

BACKGROUND: In histologic studies, the volumetric status of the intralabyrinthine fluids is judged by the position of the endolymphatic membranes. Bulging of the membranes, commonly known as endolymphatic hydrops, is assumed to be caused by excess of endolymph. The opposite situation, retraction of the membranes is, however, only incidentally described and relatively little attention has been paid to its significance. Almost one hundred years ago Wittmaack described retraction of the endolymphatic membranes, which has since been considered to be preparation artifact--a concept that essentially

BACKGROUND: Studies have demonstrated that autophagy pathways are activated in the brain after experimental subarachnoid hemorrhage (SAH) and this may play a protective role in early brain injury. However, the contribution of autophagy in the pathogenesis of cerebral vasospasm (CVS) following SAH, and whether up-regulated autophagy may contribute to aggravate or release CVS, remain unknown. Cystatin C (CysC) is a cysteine protease inhibitor that induces autophagy under conditions of neuronal challenge. This study investigated the expression of autophagy proteins in the walls of basilar arterie

A study was made on human temporal bones to elucidate the effects of aging on the auditory system. Ninety-six ears from 68 individuals, varying in age from infancy to 84 years, served as 'normal ears'. None of these cases had clinical or pathological evidence of specific ear diseases. 'Pathological ears' consisted of 89 ears from 58 individuals which exhibited slowly progressive hearing loss, and which had no specific known cause for the loss. Two aging phenomena were found to affect the human ear. The first is physiological, and has been called 'auditory senility'. It involves all the differe

Anatomical middle and inner ear parameters are often used to predict hearing sensitivities of mammalian species. Given that ear morphology is substantially affected both by phylogeny and body size, it is interesting to consider whether the relatively small anatomical differences expected in related species of similar size have a noticeable impact on hearing. We present a detailed anatomical description of the middle and inner ears of the red fox Vulpes vulpes, a widespread, wild carnivore for which a behavioural audiogram is available. We compare fox ears to those of the well-studied and simil

For the most part, the coiled shape of the cochlea has been shown to have only minor importance for air-conducted hearing. It is hypothesized, however, that this coiled shape may play a more significant role for the bone-conducted (BC) route of hearing, through inertial forces exerted by the middle ear and cochlear fluid, and that this can be tested by comparing the results of applying BC stimuli in a variety of different directions. A three-dimensional finite element model of a human middle ear coupled to the inner ear was formulated. BC excitations were simulated by applying rigid-body vibra

Epidermoid tumors comprise about 1% of all intracranial tumors. They are congenital lesions that arise from paramedian cisterns within the posterior fossa. These tumors present as heterogeneous hyperintense lesions on FLAIR and homogenous hyperintense lesions on DWI. Surgical resection remains the most accepted form of therapy, but epidermoid tumors may recur. These tumors are well exposed through a traditional retrosigmoid approach. The tumor can be removed relatively easily as it is avascular. However, the propensity of this tumor type to fill the small spaces within basal cisterns and attac
