Risk factors associated with surgical site infection after internal fixation of ballistic diaphyseal fractures.

OBJECTIVES: Management of civilian gunshot wound (GSW) fractures is controversial, with limited data on infection risk and controversy regarding treatment. While lower-energy GSWs are considered lower risk than other open fractures, complication rates remain high. This study aimed to identify risk factors for infection in operatively treated ballistic fractures. METHODS: After institutional review board approval, we identified 2136 GSW-related fractures from 01/01/2012 to 12/31/2021 at our level one trauma center. After excluding articular, hand, foot, injuries through viscera and pre-existin

Modified Elastic Stable Intramedullary Nailing: A New Approach for Metaphyseal-Diaphyseal Fractures of the Forearm in Children.

Elastic Stable Intramedullary Nailing (ESIN) is the treatment of choice for forearm fractures in children. However, fractures occurring at the distal metaphyseal-diaphyseal junction (DMDJ) of the radius are notoriously difficult to treat. This paper presents a modified ESIN method to treat these fractures. Surgical intervention was considered for fractures with angulation >10 ° in the coronal plane, 20 ° in the sagittal plane, >1 cm of bayoneting, and instability after reduction. A radial approach is used. Once the nail is at the biceps tuberosity, it is retracted by 4 cm and at bent 90 °, and

Plates for the treatment of long bone metaphyseal and diaphyseal fracture and deformity in osteogenesis imperfecta: A scoping review.

PURPOSE: Osteogenesis imperfecta is a rare disorder characterized by bone fragility. The current gold standard treatment for long bone fractures and deformities is telescopic intramedullary rods. Although the use of plates as a standalone implant has been discouraged, recent research has investigated their use as an adjunct to intramedullary fixation. This scoping review aims to assess the current literature on plates for the treatment of long bone metaphyseal and diaphyseal fractures and deformities in osteogenesis imperfecta. METHODS: The MEDLINE, Embase, CENTRAL, and CDSR databases were se

Elastic intramedullary nailing: a viable solution to stabilise adult humeral diaphyseal fractures? A systematic review.

PURPOSE: Conservatively managed diaphyseal humeral fractures have demonstrated higher non-union rates than previously reported and there are drawbacks to functional bracing. There is a growing trend towards operative management of these injuries, yet there are several complications associated with current operative methods. This systematic review aimed to evaluate the role of elastic stable intramedullary nailing (ESIN) as a solution to mitigate these risks. METHODS: A systematic review and meta-analysis was performed using PubMed, EMBASE, MEDLINE and Cochrane databases. Methodology followed

Femoral diaphyseal fatigue stress fracture in an older adult male with normal bone mineral density.

A healthy man in his early 70s taking no regular medications presented to a tertiary hospital emergency department three times in 4 weeks with the isolated symptom of moderate-to-severe non-traumatic unilateral distal right thigh pain on weightbearing. Plain radiographs of the femur and knee were performed on each occasion. Imaging reports of the first and second occasions made a comment of mild degenerative changes of the tibiofemoral joint with no other abnormalities, resulting in the patient being discharged with a diagnosis of knee osteoarthritis, provision of analgesia and advice to follo

Diaphyseal fractures of the humerus with radial nerve injury: how to manage both fracture and the nerve injury.

In this article, data of nerve and bone recovery were reported in a series of patients treated with open reduction and internal fixation (ORIF) with plate in humeral shaft fractures (HSF) associated with radial nerve palsy (RNP). The authors highlight the role of early surgical intervention for optimal nerve repair and patient's recovery, and to avoid subsequent and more complex surgery to patients. This is a retrospective study of 24 of 31 patients with HSF with RNP (10,1%) out of 308 patients with HSF, treated surgically between 2012 and 2023 in a level I Trauma Center. The mean age was 57 y

A Comparative Study Between Plate Osteosynthesis and Intramedullary Nailing for Diaphyseal Fracture of Radius and Ulna in Adults.

INTRODUCTION: In this era of active living, industrial growth, increasing automobile accidents, and athletic activities, fractures of the forearm bones are becoming more frequent. The incidence of diaphyseal fractures of both bone forearms is reported to be approximately 10 per 10,000 persons per year, although rates may vary according to age and sex. If not properly treated, a fracture of the forearm bones might cause a serious loss of function. Therefore, to restore function, these fractures require adequate anatomical reduction and internal fixation. The majority of forearm fractures in adu

Epidemiology of proximal and diaphyseal humeral fractures in children: an observational study from the Swedish Fracture Register.

BACKGROUND: Most fractures in children are fractures of the upper extremity. Proximal and diaphyseal humeral fractures account for a minority of these fractures. To our knowledge, few previous reports address these fractures. This study aimed to describe the epidemiology and current treatment of proximal and diaphyseal humeral fractures by using the Swedish Fracture Register (SFR). METHODS: In this nationwide observational study from the SFR we analysed data on patient characteristics, injury mechanism, fracture classification and treatment. We included patients aged < 16 years at time of inj

The effect of single plate and double plate application on union and functional results in humeral diaphyseal nonunions.

In this study, we assessed and compared the outcomes of two different fixation techniques for humeral diaphyseal fracture nonunions. A retrospective evaluation of 22 patients who underwent single-plate and double-plate fixation due to humeral diaphyseal nonunions was conducted. Union rates, union times, and functional outcomes of the patients were assessed. There was no significant difference between single- plate and double-plate fixation in terms of union rates or union times. The double-plate fixation group achieved significantly better functional outcomes. Nerve damage or surgical site inf

Osteosynthesis of diaphyseal tibia fracture with locking compression plates: A numerical investigation using Taguchi and ANOVA.

Performance of the locking compression plate (LCP) is a multifactorial function. The control parameters of plating, such as geometries, material properties, and physical constraints of the LCP components, affect basic functions associated with the bone fixation, including the extent of stress shielding and subsequent bone remodeling, strength and stability of the bone-LCP construct, and performance of secondary bone healing. The main objectives of this research were as follows: (1) to find the appropriate values of control parameters of an LCP construct to achieve the optimized performance thr

Less Invasive Surgery With Autologous Bone Grafting for Proximal Fifth Metatarsal Diaphyseal Stress Fractures.

Surgery with autologous bone grafting for proximal fifth metatarsal diaphyseal stress fracture has a potential to decrease nonunion, but it is not performed widely as the primary surgery because of donor-site morbidity. We have devised and performed a less invasive surgical procedure with autologous bone grafting and aimed to investigate the clinical and radiologic outcomes of this procedure. The data for 73 patients who underwent primary intramedullary screw fixation with autologous bone grafting from the fifth metatarsal base for proximal fifth metatarsal diaphyseal stress fractures were inv

Comparison of contact surface areas of metatarsal diaphyseal osteotomies for correction of hallux valgus: Experimental study.

OBJECTIVE: This study aimed to determine, pre-correction, the potential change in the osteotomy-site bony contact surface area that would occur during standard metatarsal diaphyseal procedures with the Baran-Unal modification of Mau osteotomy and then to compare it, post-correction, with the actual osteotomy-site bony contact surface area changes for a standard degree of deformity correction. METHODS: A total of 30 standard, same sized, biomechanically equivalent, left first metatarsal sawbones were included in this experimental study. They were divided equally into five groups for each of th
