Degenerative and traumatic ligament lesions of the carpometacarpal joints frequently occur at the thumb ray, whereas the carpometacarpal amphiarthrosis of other finger rays are rarely affected. The metacarpophalangeal and interphalangeal joints of the thumb and fingers are stabilized by bilaterally running collateral ligaments and palmar plates. At the base of the metacarpophalangeal joints, several ligaments of the extensor hoods guide the extensor tendons and coordinate the fine motoric skills of phalangeal flexing and extending. Several annular and cruciform ligaments hold the flexor tendon

CASE: An exceptional event occurred during gradual extension with an external fixator for recurrent small finger Dupuytren flexion deformity. A massive proximal interphalangeal joint distraction arose, possibly because of the center of rotation misalignment and a hasty patient-controlled correction. The device was removed, and a plaster splint protected the finger for 5 days to prevent dislocation of the unstable joint. It relocated with an acceptable clinical result.
CONCLUSION: Gradual extension of Dupuytren contractures with an external fixator may induce severe joint distraction by eccent

Surgery is still the main treatment for congenital polydactyly, and the aim of surgical reconstruction is to obtain a thumb with excellent function and appearance. A systematic assessment of polydactyly is required prior to surgery, including bone stress lines, joint deviation, joint activity and joint instability, size and development of finger and nail. Bone shape, joint incongruency, and abnormal tendon insertions must be corrected completely, in order to obtain good function and to avoide secondary surgery. Bilhault-Cloquet procedure can reconstruct the size of the finger and nails. Fine m

Robotic surgery started nearly 30 years ago. It has achieved telepresence and the performance of repetitive, precise, and accurate tasks. The "master-slave" robotic system allows control of manipulators by surgeon at distant site. Robotic surgical fingers were developed to allow surgeons to move them with accuracy through sensors fixed on surgeon's hand. Also, haptic sensors were developed to allow transmission of sensation from robotic finger to surgeon's finger. A complete system of a, 3D printed by a stereolithography (SLA) 3D printer, robotic surgical finger with haptic feedback system is

PURPOSE: Pediatric finger flexion contractures can have wide transverse contracted volar tissue resulting in a pterygium or web of tissue extending across the affected joints. Here, we establish the double opposing stiletto (DOS) flap technique for releasing the skin of the flexion joint contracture transversely and then recruiting flap tissue from both the radial and ulnar aspects of the pterygium to cover the volar defect and with primary closure of flap donor sites.
METHODS: The study includes pediatric patients with flexion joint contractures resulting from burn scar or camptodactyly etio

The intrinsic muscles of the hand consist of seven interossei and four lumbrical muscles. With the extrinsic long extensors, the intrinsic muscles act via the dorsal aponeurosis to control finger motion. The interossei also control finger abduction and adduction, and flexion of the metacarpophalangeal joints. The lumbricals are the main extensors of the interphalangeal joints. The complex structure of the dorsal aponeurosis allows coordination and individual joint motion. The muscles of the hypothenar and thenar eminences also insert into the dorsal aponeurosis and the skeleton of the small fi

BACKGROUND: Reconstructive hand surgery is well established for the management of patients with rheumatoid arthritis; however, with the advent of biologic drugs and methotrexate, disease activity, including the development of hand deformities, is well controlled. Nonetheless, many patients still need personalized surgery.
CASE: A 61-year-old woman with a 35-year history of rheumatoid arthritis presented with right hand deformity with unstable ulnar deviation of the metacarpophalangeal joints from the index to the little finger and hyperextension of the thumb interphalangeal joint. Her hobby w

Understanding the transformation of tendon forces into joint torques would greatly aid in the investigation of the complex temporal and spatial coordination of multiple muscles in finger movements. In this study, the effects of the finger posture on the tendon force transmission within the finger extensor apparatus were investigated. In five cadaver specimens, a constant force was applied sequentially to the two extrinsic extensor tendons in the index finger, extensor digitorum communis and extensor indicis proprius. The responses to this loading, i.e., fingertip force/moment and regional stra

PURPOSE: To conduct kinematic analyses of both intact and sectioned terminal tendon (TT) of multiple fingers in the hand.
METHODS: The TTs of 36 fresh-frozen cadaveric digits were used in this study. TT excursion was assessed along with the influence on proximal joint motion. The influence of TT lengthening and shortening on distal interphalangeal (DIP) joint motion were investigated.
RESULTS: TT excursion averaged 1 mm at the DIP joint and was influenced by the proximal interphalangeal (PIP) joint but not the position of other joints in the hand and wrist. The greatest degree of DIP joint m

The patient was a 37-year-old male who visited us for the chief complaints of severely keratinized elastic soft nodules with normal skin color or slightly glossy pale brown color at the distal interphalangeal joints of the bilateral second, third, fourth and fifth fingers associated with bilateral clubbed fingers. The nodules were histopathologically diagnosed as knuckle pads. Clubbed fingers were also noted, but no cardiopulmonary disorder was observed. Knuckle pads are circumscribed fibromatous thickenings of the skin overlying the finger joints. External factors are considered to be the cau

Surgical treatment of bony mallet fingers is frequently recommended, but the evidence is sparse. This randomized clinical trial aimed to compare nonoperative splinting versus extension-block pinning of bony mallet fingers with involvement of more than one-third of the joint surface but without primary joint subluxation. Thirty-two patients were randomized and 28 fulfilled the protocol. At 6 months follow-up, there were no significant differences in active extension lag in the distal interphalangeal joint (the primary outcome) or in patient-reported function and pain scores. Flexion and active

INTRODUCTION: The efficiency of collagenase of Clostridium histolyticum (CCH; Xiapex) in the treatment of Dupuytren's contracture has been proved in phase III studies. This retrospective study aims to evaluate our clinical results after the use of CCH.
PATIENTS AND METHODS: The study included 40 Dupuytren's contractures in 37 patients. There were 32 male and 5 female patients; their average age was 66 years. The most affected finger was the ring finger (55%; 22/40), followed by the little finger (30%; 12/40) and the middle finger (15; 6/40). 14 fingers (35%) presented isolated contractures of

BACKGROUND: This study aimed to demonstrate the clinical application of the dorsoproximal interphalangeal island flap as an alternative approach to skin graft or cross-finger flap to repair lesions at the ventral site at the proximal interphalangeal (PIP) finger joint.
METHODS: Fifteen patients received flaps (11 men and four women, n = 25 flaps). The repair of volar contracture in finger sequelae after burn injuries was the main indication. Five patients underwent two or more flaps during the same surgical session. The mean patient age was 18 years (range, 7-56 years). Most patients presente

The metacarpal-like hand is a severe hand injury, never addressed before. It describes a hand that has lost a significant degree of prehension through a wide array of amputations, involving all digits proximal to the functional length except in one finger or in two digits, including the thumb. The thumb condition can be used to differentiate between two types. In type I, the thumb is intact or amputated at or distal to the interphalangeal joint (functional length); therefore, the reconstruction is focused mainly on fingers. In type II, the thumb is amputated proximal to the interphalangeal joi

Neck/shoulder pain has previously been linked to repetitive work and muscle fatigue. We have shown that asymptomatic people performing repetitive upper limb tasks display signs of shoulder fatigue and of whole-body compensatory strategies. However, the role played by the proprioceptive system in the production of these compensatory strategies has not been studied. A group of asymptomatic adults (n = 18) performed a repetitive pointing task at shoulder height to fatigue. Before and after fatigue, they performed two position sense tasks, eyes closed: a single-joint task where they abducted their

Osteoarthritis is the most common joint disease. In addition to known risk factors e.g. genetics, age and hormonal status it has been suggested that chronic-repetitive micro- and substantial (macro-) injury may play an important pathogenetic role. In a longitudinal case-study we examined Judo-players for clinical and radiological changes of the finger joints over the course of 16 years. All examined 8 players demonstrated soft tissue changes including Heberden nodes and radiological changes typical for osteoarthritis of the finger joints. Changes were symmetrical and were not restricted to joi

A 13-year-old female patient visited our hospital with a painful distal interphalangeal joint and deformity on her right middle finger. Initial plain radiographs uncovered ulnar-sided subluxation of the distal interphalangeal joint of the right middle finger, and an osteochondral defect was also noted at the articular surface of the distal and middle phalanges. We decided to attempt distraction arthroplasty with an external fixator. Finally postoperative 12-month follow-up plain radiographs showed complete osteochondral regeneration and phalangeal alignment. To our knowledge, this is the first

INTRODUCTION: The aim of this study was to evaluate different grip positions as a contributing factor for primary periphyseal stress injuries of the finger phalanges in climbing.
METHODS: Ultrasound imaging of the proximal interphalangeal joint was performed on 37 asymptomatic adolescent climbers. Longitudinal images were obtained of middle and ring fingers of both hands in different grip positions (open, half-open, and crimp), unloaded and loaded. The translation between the dorsal head of the proximal phalanx and the shaft of the middle phalanx was measured in an unloaded and loaded situati

This study aimed to evaluate the sensitivity and specificity of the new clinical diagnostic and classification criteria for Kashin-Beck disease (KBD) using six clinical markers: flexion of the distal part of fingers, deformed fingers, enlarged finger joints, shortened fingers, squat down, and dwarfism. One-third of the total population in Linyou County was sampled by stratified random sampling. The survey included baseline characteristics and clinical diagnoses, and the sensitivity and specificity of the new criteria was evaluated. We identified 3,459 KBD patients, of which 69 had early stage
