Use of analgesic, anesthetic, and sedative medications during pediatric hospitalizations in the United States 2008.

BACKGROUND: The wide need for analgesia, anesthesia, and sedation in children and the lack of pediatric labeling leads to widespread off-label use of medications for pain and sedation in children. Any attempt to address the lack of labeling will require national estimates of the numbers of children using each medication, their ages, and other factors, to understand the overall use of these medications. We describe use of analgesics, anesthetics, and sedatives in pediatric inpatients by result of conducting a statistical analysis of medication data from >800,000 pediatric hospitalizations in th

Transdermal buprenorphine controls central neuropathic pain.

A 53-year-old male with peripheral sensorimotor neuropathy suffered an intracerebral hemorrhage resulting in right hemiparesis and hemisensory loss. Three months later, he developed constant and burning pain within the entire right side of his body. He was diagnosed with central pain syndrome and treated with antiepileptics and tricyclic antidepressants. Minimal analgesia was achieved, which was limited by intractable sedation and drowsiness. Patient was then treated with oral opioids (morphine and hydrocodone with acetaminophen) in escalating doses that produced cognitive impairment. After an

Assessment of Codispensing Patterns of Mirabegron and Prespecified CYP2D6 Substrates in Patients with Overactive Bladder.

BACKGROUND: Patients with overactive bladder (OAB) experience sudden, intense urges to urinate, which may include urge urinary incontinence and nocturia. Pharmacotherapy includes β3-adrenergic receptor agonists such as mirabegron; however, mirabegron contains a label warning for cytochrome P450 (CYP) 2D6 inhibition, making coadministration with CYP2D6 substrates require monitoring and dose adjustment to avoid unintended increases in substrate concentration. OBJECTIVE: To understand the codispensing patterns of mirabegron among patients using ten predefined CYP2D6 substrates with and before mi

Two decades of prescription opioid-related cardiovascular safety signals: a FAERS-based pharmacovigilance study using machine learning.

INTRODUCTION: Prescription opioids are essential for pain management but have been increasingly linked to cardiovascular adverse events. Understanding these risks is critical for informed prescribing opioid drugs. METHODS: Approximately 18 million adverse event reports from 2004 Quarter 1 to 2024 Quarter 3 in the U.S. FDA Adverse Event Reporting System (FAERS) were downloaded and processed. RxNorm was used to normalize prescription opioid drug names. Empirical Bayes Geometric Mean analysis was then performed to detect signals of disproportionate reporting. Cardiovascular-related Preferred Ter

Effect of Florida's Prescription Drug Monitoring Program and Pill Mill Laws on Opioid Prescribing and Use.

IMPORTANCE: Prescription Drug Monitoring Program (PDMP) and pill mill laws are among the principal means states use to reduce prescription drug abuse and diversion, yet little high-quality evidence exists regarding their effect. OBJECTIVE: To quantify the effect of Florida's PDMP and pill mill laws on overall and high-risk opioid prescribing and use. DESIGN, SETTING, AND PARTICIPANTS: We applied comparative interrupted time-series analyses to IMS Health LifeLink LRx data to characterize the effect of PDMP and pill mill law implementation on a closed cohort of prescribers, retail pharmacies,

Restorative neuromodulation for chronic lumbar spine pain secondary to a long construct spinal fusion: a case report.

BACKGROUND: Persistent low back pain is a known complication of long construct fusions, often resulting in long term impacts on patients' physical function. While available treatment options manage patients' pain, there are few options available that also work to improve the patients' physical function. Prior studies have demonstrated the benefit of restorative stimulation of the multifidus muscles on patients' pain, function, and disability levels. This case hypothesizes that medial branch restorative neuromodulation at the L2 segment may be therapeutic for the patient's chronic lumbar spine

Detection of Drugs in Nails: Three Year Experience.

Nails (fingernails and toenails) are made of keratin. As the nail grows, substances incorporate into the keratin fibers where they can be detected 3-6 months after use. Samples are collected by clipping of 2-3 mm of nail from all fingers (100 mg). We present drug testing results from 10,349 nail samples collected from high-risk cases during a 3-year period of time. Samples were analyzed by validated analytical methods. The initial testing was performed mostly using enzyme-linked immunosorbent assay, but by liquid chromatography-tandem mass spectrometry (LC-MS-MS) as well. Presumptive positive

Crowdsourcing black market prices for prescription opioids.

BACKGROUND: Prescription opioid diversion and abuse are major public health issues in the United States and internationally. Street prices of diverted prescription opioids can provide an indicator of drug availability, demand, and abuse potential, but these data can be difficult to collect. Crowdsourcing is a rapid and cost-effective way to gather information about sales transactions. We sought to determine whether crowdsourcing can provide accurate measurements of the street price of diverted prescription opioid medications. OBJECTIVE: To assess the possibility of crowdsourcing black market

Gabapentin as a potential option for treatment of sciatica.

Gabapentin has been approved in the United States for the treatment of epilepsy and postherpetic neuralgia. Gabapentin has also demonstrated proven efficacy for the treatment of diabetic peripheral neuropathy and trigeminal neuralgia, although these represent off-label uses of the drug. However, to our knowledge, no data have been published regarding the efficacy of gabapentin for treating sciatica. We describe two patients with sciatica who were successfully treated with gabapentin. The first was a 32-year-old man with severe shooting pain in his left leg that was later diagnosed as sciatica

Declining but Pronounced State-Level Disparities in Prescription Opioid Distribution in the United States.

The United States (US) opioid epidemic is a persistent and pervasive public health emergency that claims the lives of over 80,000 Americans per year as of 2021. There have been sustained efforts to reverse this crisis over the past decade, including a number of measures designed to decrease the use of prescription opioids for the treatment of pain. This study analyzed the changes in federal production quotas for prescription opioids and the distribution of prescription opioids for pain and identified state-level differences between 2010 and 2019. Data (in grams) on opioid production quotas and
