Diffuse neuroinflammation and immature neuron loss in fetal Rhesus macaques after short-term intrauterine infection.

Chorioamnionitis or infection of the amniotic fluid and membranes surrounding the fetus can cause fetal systemic inflammation and adverse neurological outcomes. To investigate the pathogenesis of chorioamnionitis-induced neurological injury, we developed a non-human primate model by infusion of live Escherichia coli. A micro-osmotic pump was placed in pregnant Rhesus macaques at ~ 85% term gestation via hysterotomy. Live E. coli (105 CFU/h, 1µL/h, n = 7) or sterile broth (Controls, 1µL/h, n = 7) infusions were given over 72 h followed by necropsy. A third group of pregnant macaques received mi

Lower Uterine Segment Corrugated Sutures in Hemorrhage during Cesarean Section because Previal and/or Placenta Accreta Spectrum: Case Reports Series and Literature Review.

OBJECTIVES: We present the original technique of compression hemostatic sutures on the lower uterine segment due to early postpartum hemorrhage during cesarean section, with a literature review. METHODS: A retrospective clinical case study was conducted at the tertiary perinatal center. Twelve patients had nine planned and three urgent cesarean sections due to antenatally verified placenta previa and/or placenta accreta spectrum and defined early postpartum hemorrhage > 1000 mL during cesarean section. As the use of uterotonics failed to produce any effect and hemorrhage persisted, compressio

Serial plasma metabolites following hypoxic-ischemic encephalopathy in a nonhuman primate model.

Biomarkers that indicate the severity of hypoxic-ischemic brain injury and response to treatment and that predict neurodevelopmental outcomes are urgently needed to improve the care of affected neonates. We hypothesize that sequentially obtained plasma metabolomes will provide indicators of brain injury and repair, allowing for the prediction of neurodevelopmental outcomes. A total of 33 Macaca nemestrina underwent 0, 15 or 18 min of in utero umbilical cord occlusion (UCO) to induce hypoxic-ischemic encephalopathy and were then delivered by hysterotomy, resuscitated and stabilized. Serial bloo

Intrauterine device insertion during the postpartum period: a systematic review.

BACKGROUND: Insertion of an intrauterine device (IUD) at different times or by different routes during the postpartum period may increase the risk of complications. METHODS: We searched Medline, Lilacs and Cochrane Collaboration databases for articles in any language, between database inception until December 2008, which compared outcomes of postpartum IUD insertion time intervals. Search terms included postpartum, puerperium, postcesarean delivery, cesarean section, IUD(s), IUCD(s), intrauterine device(s) and insertion. RESULTS: From 297 articles, we identified 15 for inclusion in this revi

Enhancing sealing of fetal membrane defects using tissue engineered native amniotic scaffolds in the rabbit model.

OBJECTIVE: The purpose of this study was to compare the efficacy of native engineered amniotic scaffolds (AS) and polyesterurethane scaffolds (DegraPol) and document wound healing response when sealing iatrogenic fetal membrane defects in the rabbit model. STUDY DESIGN: Native AS were engineered from freshly harvested membranes of 23 days' gestational age (GA; term = 31-2 d). Acellularity of AS was assessed by histology, light and scanning electron microscopy. Fetal membrane defects were created by 14 gauge-needle puncture at GA 23 days and primarily closed with AS (n = 10) or DegraPol (n = 1

Circulatory changes following premature delivery in a baboon model of hyaline membrane disease.

The premature baboon delivered by hysterotomy at 140 +/- 2 days (75%) gestation develops hyaline membrane disease (HMD) and left-to-right (L-R) shunting through the patent ductus arteriosus (PDA). To characterize hemodynamic changes that follow premature delivery, we measured systemic and organ blood flow, oxygen transport, and systemic vascular resistance over the first 96 h of life. We compared these measurements with those from more mature animals of the same species. Radiolabeled microspheres were used to measure organ blood flow (in ml.min-1.g-1) at 3 (n = 18), 23 (n = 17), and 96 h (n =

Trauma in pregnancy: assessment, management, and prevention.

Trauma complicates one in 12 pregnancies, and is the leading nonobstetric cause of death among pregnant women. The most common traumatic injuries are motor vehicle crashes, assaults, falls, and intimate partner violence. Nine out of 10 traumatic injuries during pregnancy are classified as minor, yet 60% to 70% of fetal losses after trauma are a result of minor injuries. In minor trauma, four to 24 hours of tocodynamometric monitoring is recommended. Ultrasonography has low sensitivity, but high specificity, for placental abruption. The Kleihauer-Betke test should be performed after major traum

The Intrauterine Treatment of Open Spinal Dysraphism.

BACKGROUND: Open spinal dysraphism is a congenital malformation that causes major morbidity. Its consequences include sensory and motor impairment as well as bladder- and bowel dysfunction. It is often also associated with prenatal ventriculomegaly, which, in turn, necessitates postnatal treatment with a ventriculoperitoneal shunt in approximately 80% of cases. Prenatal therapy with coverage of neural tube defect can reduce the shunt rate and preserve motor function. In this review, we describe the different surgical procedures and their outcomes. METHODS: This review is based on publications

Management of Cesarean Section Uterine Scar Ectopic Pregnancy Type 2 Using Methotrexate: Insights from a Case Report.

BACKGROUND Cesarean section uterine scar ectopic pregnancies (CSEP) are rare occurrences in which an embryo implants along uterine scar tissue from previous hysterotomies. These cases carry high morbidity and mortality, including risk of significant hemorrhage and uterine rupture. Society for Maternal-Fetal Medicine guidelines suggest either medical or surgical termination of these pregnancies; however, there are no current definitive recommendations for the management of these cases. Here, we present the case of a 36-year-old patient with a first-trimester CSEP who was managed with local intr

Amniotic Membrane Interposition Graft for Open Fetal Myelomeningocele Repair.

BACKGROUND AND OBJECTIVES: Myelomeningocele (MMC) is a congenital anomaly frequently leading to motor deficits, urological dysfunction, and hydrocephalus. Fetal surgical repair improves motor function and reduces the need for cerebrospinal fluid diversion for hydrocephalus. One complication of MMC repair is spinal cord tethering at the site of surgical repair. Surgical techniques to reduce symptomatic tethering and achieve optimal motor function are an area of ongoing research. This study's objective is to evaluate a technique for interposing an amniotic membrane graft between the pia of the c

Role of Imaging in Obstetric Interventions: Criteria, Considerations, and Complications.

US has an established role in the prenatal detection of congenital and developmental disorders. Many pregnant women undergo US at 18-20 weeks of gestation for assessment of fetal anatomy and detection of structural anomalies. With advances in fetoscopy and minimally invasive procedures, in utero fetal interventions can be offered to address some of the detected structural and physiologic fetal abnormalities. Most interventions are reserved for conditions that, if left untreated, often cause in utero death or a substantially compromised neonatal outcome. US is crucial for preprocedural evaluati

Vaccination of neonatal calves with Mycobacterium bovis BCG induces protection against intranasal challenge with virulent M. bovis.

Vaccination of neonates with Mycobacterium bovis bacillus Calmette-Guerin (BCG) may be a strategy that overcomes reduced vaccine efficacy associated with exposure to environmental mycobacteria in humans and cattle. Preliminary comparisons indicated that 2-week-old calves produced an immune response to vaccination at least as intense as that observed in adults. Subsequently, five gnotobiotic hysterotomy derived calves aged 1 day were inoculated with BCG and 3 months later were challenged intranasally with virulent M. bovis. The number of tissues with lesions and the pathological extent of these

Operative fetoscopy: new perspective in fetal therapy?

In a limited number of conditions in utero surgery may be life-saving, such as some cases of congenital diaphragmatic hernia, cystic adenomatoid malformation of the lung, sacrococcygeal teratoma and lower urinary tract obstruction. Postoperative premature labour and its extreme invasiveness have been major drawbacks for open fetal surgery. More recently the merger of fetoscopy and advanced video-endoscopic surgery has been the basis of the concept of endoscopic fetal surgery. In order to evaluate the opportunities of 'fetendo' surgery, animal models have been developed to test the safety of th
