Laparoscopic ureterocalicostomy: development of a technique simplified by application of Nitinol clips and a wet monopolar electrosurgery device.

PURPOSE: We developed a technique for laparoscopic ureterocalicostomy with the use of intracorporeal suturing and subsequently simplified the technique by application of experimental Nitinol clips. MATERIALS AND METHODS: We performed laparoscopic ureterocalicostomy on 16 domestic swine divided into four groups of four animals each. The kidney was exposed laparoscopically, and the renal artery was atraumatically clamped. The lower pole of the kidney was amputated to expose a lower-pole calix, and hemostasis of the cut renal surface was obtained with a wet monopolar electrosurgical device (Floa

Access to the upper calyx in supine position: breaking a myth.

The feasibility and safety of upper calyx access in supine percutaneous nephrolithotomy (PCNL) remains controversial. We aimed to detail our technique and describe the outcomes in effectivity as well as safety in a large cohort. The authors analyzed patients with renal and proximal ureteral stones who underwent supine PCNL using upper calyx access at Rio Hortega University Hospital Valladolid (Spain) between 2013 and 2023. Patients were placed in the Galdakao-modified supine Valdivia position. Transpapillary percutaneous access to the upper calyx was guided by a combination of ultrasound (US)

Percutaneous Nephrolithotomy for De Novo Urolithiasis After Kidney Transplantation: A Systematic Review of the Literature.

Introduction and Objective: Renal transplantation is the treatment for end-stage renal disease that offers better quality of life and survival. Among the possible complications that might affect allografts, urolithiasis might have severe consequences, causing acute kidney injury (AKI) or septic events in immunocompromised patients. Allograft stones might be treated with percutaneous nephrolithotomy (PCNL). The aim of this Cochrane style review was to assess the safety and efficacy of PCNL in patients with renal transplant. Methods: A comprehensive search in the literature was performed includi

Role of aztreonam in urinary tract infections.

Bacterial colonization is the necessary first step in the etiology of all urinary tract infections. The hospital setting provides a pool of pathogenic bacteria which may lead to colonization in the hospitalized patient. The nosocomial infections which may follow and require treatment most often follow catheterization. Some host factors often lead to persistence of colonization and invasive symptomatic disease. These might include stone, caliceal diverticula, a hydronephrotic kidney, vesicoureteral reflux, bladder neck obstruction with residual urine, urethral diverticula, and infection of the

False-positive radioiodine uptake in the abdomen and the pelvis: radioiodine retention in the kidneys and review of the literature.

Because the kidneys are usually not visualized on radioiodine whole-body scans, the renal uptake can be mistaken for a thyroid cancer metastasis. The authors report the prevalence and characteristics of radioiodine retention in the kidneys and review the reported causes of false-positive radioiodine uptake in the abdomen and pelvic areas. Radioiodine uptake in the renal bed was noted on 9 of 400 (2.2%) I-123 diagnostic whole-body scans performed over a 7-month period in our center. The uptake was noted more clearly on posterior views, cleared on delayed images after further hydration, and was

Comparison of propofol-alfentanil and propofol-remifentanil anaesthesia in percutaneous nephrolithotripsy.

BACKGROUND AND OBJECTIVE: Percutaneous nephrolithotripsy (PCNL) is used for the fragmentation and removal of stones from the renal pelvis and renal calyceal systems. We compared the effects of propofol-alfentanil or propofol-remifentanil anaesthesia on haemodynamics, recovery characteristics and postoperative analgesic requirements during percutaneous nephrolithotripsy. METHODS: Thirty non-premedicated patients were randomly allocated to receive either propofol-alfentanil (Group A) or propofol-remifentanil (Group R). The loading dose of the study drug was administered over 60 s (alfentanil 10

Renal transplantation update.

Various factors affect the outcome of renal transplants in humans. Matching for HLA-A, -B, and -DR histocompatibility antigens improves survival rates for renal allografts from first cadaver donors. Zero-HLA-A- and -B-antigen-mismatched grafts and two-HLA-DR-antigen-matched grafts do better, although results differ depending on the recipient's primary renal disease. Pretransplant third-party blood transfusions significantly improve survival rates of cadaver donor allografts. The mechanism of this beneficial effect has not been identified; however, blood transfusions probably do not "select out

Urologic complications in renal transplantation.

The current overall reported incidence of major urologic complications following renal transplantation is 5 per cent. The presence of such a complication increases the likelihood of patient mortality by a factor of three. Standard utilization of postoperative radionuclide scanning is very useful in early diagnosis. Vesical fistulas generally result from improper bladder closure. The incidence of bladder complications increases with secondary and tertiary grafts. Ureteral complications result when the blood supply of the ureter is impaired. These include fistula formation, necrosis, and obstruc

Comparative performance of five nephrolithometry scoring systems and a novel nomogram for predicting stone-free rate following pediatric percutaneous nephrolithotomy.

BACKGROUND: Percutaneous nephrolithotomy (PCNL) is the first-line treatment for complex pediatric renal stones, yet accurately predicting postoperative outcomes remains a clinical challenge. This study aimed to systematically compare the performance of five established nephrolithometry scoring systems-the S.T.O.N.E. (size, tract length, obstruction, number of involved calices, and essence) score, Guy's Stone Score (GSS), Seoul National University Renal Stone Complexity (S-ReSC) score, Clinical Research Office of the Endourological Society (CROES) nomogram, and Stone Kidney Size (SKS) score-and

Forgotten Perinephric Drain Anchored in the Tissues: Lessons to Be Learned.

Background: Drainage of any deep seated abscess often requires placement of catheters along with other conservative measures. These catheters are removed when the drainage volume reduces to clinically insignificant levels. However, if left in situ, there are potential complications. One such complication necessitating additional surgical procedure is highlighted in this case report. Malecot's catheter and children may be more at risk for such a complication. Case Presentation: A 7-year-old girl presented with recurrent episodes of right flank pain associated with high-grade fever with chills f

[Multiple endoscopy for the treatment of upper urinary tract calculi following ileal conduit: a case report and literature review].

Upper urinary tract calculi with infection is a quite difficult acute urologic emergency. And what is more, upper urinary tract obstruction after radical cystectomy following urinary diversion may be fatal in the vulnerable patients with this kind of situation. Postoperative anatomy and other factors affect the upper urinary tract calculi, and urinary tract infection greatly increased the risk. But it is particularly difficult to handle with patients with poor general condition and septic shock treatment, so how to optimize the selection program is worth further studying. A 65-year-old man was

Minimally invasive percutaneous ablation of parapelvic renal cysts and caliceal diverticula using bipolar energy.

BACKGROUND AND PURPOSE: The use of bipolar electrocautery has proven advantages over monopolar energy during transurethral surgery by limiting hyponatremia and its clinical sequelae. Percutaneous ablation of caliceal diverticula and parapelvic renal cysts has been shown to be an effective surgical approach for the management of these conditions when clinically indicated. We present single center results of percutaneous ablation of renal cysts and caliceal diverticula using a bipolar energy technique and compare the results with a cohort of patients undergoing the procedure using monopolar ener
