Clinical Performance of New Enhanced Monofocal Intraocular Lenses: Comparison of Hydrophobic C-loop and Hydrophilic Plate-Haptic Platform.

INTRODUCTION: Enhanced monofocal intraocular lenses (IOLs) represent a new type of lens, which should lead to a very good distance vision similar to monofocal IOLs and an improved intermediate vision without increasing the risk for photic phenomena. METHODS: The aim of this clinical observation/registry study was to directly compare two different IOL platforms (hydrophilic acrylic L-333 (group A) vs hydrophobic acrylic AN6Q (group B)) with the same enhanced monofocal optic principle but different material and haptic design in clinical routine. A total of 102 cataract cases (51:51) were includ

Digital Visual Acuity and Reading Performance with Positive and Negative Display Polarities After Bilateral Diffractive Multifocal Intraocular Lens Implantation: A Longitudinal Observational Study.

INTRODUCTION: Traditional acuity charts may not fully capture visual performance during everyday digital device use. Digital screen-based assessments may better reflect contemporary visual demands, and display polarity may influence reading performance in patients with multifocal intraocular lenses (MF-IOLs). Furthermore, display polarities differ in luminance and contrast characteristics, which may influence pupil size and reading performance in patients with MF-IOLs. We aimed to compare digital visual acuity (VA) between positive (black-on-white) and negative (white-on-black) display polarit

Assessment of the impact of cataract surgery on subjective quality of vision across different intraocular lens type using the portuguese-validated QoV questionnaire.

BACKGROUND: Investigating cataract surgery outcomes is essential to capture patients' subjective perception of visual quality. This study evaluated quality of vision (QoV) before and after cataract surgery by comparing different intraocular lens (IOL) models using a Rasch-validated Brazilian Portuguese version of the QoV questionnaire. METHODS: This prospective study recruited patients from the Cataract Department of the Hospital Oftalmológico de Brasília. Participants were ≥ 50 years old, underwent bilateral phacoemulsification with implantation of the same IOL model, and had preoperative be

Polypseudophakia: from "Piggyback" to supplementary sulcus-fixated IOLs.

Polypseudophakia, the concept of using a second intraocular lens (IOL) to supplement an IOL that has already been placed in the capsular bag, was first used as a corrective measure where the power requirement was higher than that of available single IOLs. Subsequently, the technique was modified to compensate for post-operative residual refractive errors. In these early cases, an IOL designed for the capsular bag would be implanted in the sulcus.  Although these approaches were less than ideal, alternative means of correcting residual refractive errors were not without their limitations: IOL e

Preoperative Corneal Irregular Astigmatism and Long-Term Visual Outcomes After Multifocal Intraocular Lens Implantation.

PURPOSE: Multifocal intraocular lenses (MIOLs) are disadvised among patients with irregular corneas, but the role of subclinical corneal irregular astigmatism on long-term visual outcomes remains poorly elucidated. SETTING: Keye Eye Center, Seoul, Korea. DESIGN: Retrospective single-center clinical cohort study. METHODS: Bilateral implantation of PanOptix MIOL with 3 years follow-up. Eyes were stratified based on total corneal irregular astigmatism (TCIA) in Scheimpflug tomography into low (≤0.15 µm), moderate (>0.15 to <0.30 µm), and high (≥0.30 µm) TCIA groups. RESULTS: In the cohort of

Incremental cost effectiveness evaluation in clinical research.

OBJECTIVE: The health economic evaluation of therapeutic and diagnostic strategies is of increasing importance in clinical research. Therefore also clinical trialists have to involve health economic aspects more frequently. However, whereas they are quite familiar with classical effect measures in clinical trials, the corresponding parameters in health economic evaluation of therapeutic and diagnostic procedures are still not this common. METHODS: The concepts of incremental cost effectiveness ratios (ICERs) and incremental net health benefit (INHB) will be illustrated and contrasted along th

Accommodative intraocular lens versus standard monofocal intraocular lens implantation in cataract surgery.

BACKGROUND: Following cataract surgery and intraocular lens (IOL) implantation, loss of accommodation or postoperative presbyopia occurs and remains a challenge. Standard monofocal IOLs correct only distance vision; patients require spectacles for near vision. Accommodative IOLs have been designed to overcome loss of accommodation after cataract surgery. OBJECTIVES: To define (a) the extent to which accommodative IOLs improve unaided near visual function, in comparison with monofocal IOLs; (b) the extent of compromise to unaided distance visual acuity; c) whether a higher rate of additional c

Ocular parameters associated with visual performance of enhanced monofocal intraocular lens.

BACKGROUND: An enhanced monofocal intraocular lenses (IOLs) (Tecnis Eyhance ICB00 and Tecnis Eyhance Toric DIU) has been developed to enhance intermediate vision while avoiding the disadvantages of multifocal IOLs. Although many studies have demonstrated the improvement of intermediate visual acuity with enhanced monofocal IOLs, it is not known specifically for which patients these IOLs should be recommended or avoided. In this study, we aim to find out which ocular parameters affect vision performance and photic phenomenon of ICB00 or DIU at different distances. METHODS: Patients who underwe

Trifocal intraocular lenses versus bifocal intraocular lenses after cataract extraction among participants with presbyopia.

BACKGROUND: Presbyopia occurs when the lens of the eyes loses its elasticity leading to loss of accommodation. The lens may also progress to develop cataract, affecting visual acuity and contrast sensitivity. One option of care for individuals with presbyopia and cataract is the use of multifocal or extended depth of focus intraocular lens (IOL) after cataract surgery. Although trifocal and bifocal IOLs are designed to restore three and two focal points respectively, trifocal lens may be preferable because it restores near, intermediate, and far vision, and may also provide a greater range of

Manifestations of systemic disease in the retina and fundus of cats and dogs.

The fundus is unique in that it is the only part of the body that allows for a noninvasive and uninterrupted view of vasculature and nervous tissue. Utilization of this can be a powerful tool in uncovering salient incidental findings which point to underlying systemic diseases, and for monitoring response to therapy. Retinal venules and arterioles allow the clinician to assess changes in vascular color, diameter, outline, and tortuosity. The retina and optic nerve may exhibit changes associated with increased or decreased thickness, inflammatory infiltrates, hemorrhages, and detachments. While

Drug-Induced Periocular and Ocular Surface Disorders: An EAACI Position Paper.

Various systemic and topical medications can induce ocular and periocular cutaneous adverse effects (AEs), ranging from mild to severe. These AEs may lead to ocular surface (OS) damage and, in some cases, life-threatening complications. Drug-induced ocular adverse reactions are generally classified into two primary categories: toxic reactions and/or allergic hypersensitivity reactions, which can be IgE or non-IgE-mediated. Systemic antibiotics, antivirals, and anticonvulsants can trigger adverse reactions that may involve the OS. Drugs like antihistamines, beta-blockers, antipsychotics, antide

Urgent and emergent radiotherapy for hematologic malignancies of the central nervous system: a review of the literature and practical approach.

IntroductionHematologic malignancies, including leukemias, lymphomas, and myeloma, can involve the central nervous system (CNS) at the time of diagnosis or later in relapse. CNS involvement can lead to acute neurologic symptoms or signs that need prompt evaluation and treatment. Radiotherapy (RT) can lead to quick disease response, but how it can best be incorporated early into multi-modality treatment in the urgent clinical setting is often unclear.MethodsHere, we outline a practical approach to planning and incorporating urgent RT in patients with hematologic malignancies involving the CNS.
