[Pattern of injuries to the surgical team during ophthalmosurgical interventions].

BACKGROUND: Accidental sharp injuries among the health-care workers are frequent, especially in the surgical disciplines. Because there are only limited data available about accidental sharp injuries in small surgical disciplines, we investigated the occurrence of sharp injuries among ophthalmosurgeons in Switzerland. METHODS: An anonymous questionnaire was sent to all 500 members of the Swiss Ophthalmological Association. We asked how often the surgeons or any member of the surgical team suffered injury during an ophthalmic surgery within the last 2 years and which instrument had caused the

Transmission of hepatitis B virus from an orthopedic surgeon with a high viral load.

BACKGROUND: During the evaluation of a needle-stick injury, an orthopedic surgeon was found to be unknowingly infected with hepatitis B virus (HBV) (viral load >17.9 million IU/mL). He had previously completed two 3-dose series of hepatitis B vaccine without achieving a protective level of surface antibody. We investigated whether any surgical patients had acquired HBV infection while under his care. METHODS: A retrospective cohort study of all patients who underwent surgery by the surgeon was conducted. Patients were notified of their potential exposure and need for testing, and samples with

[Occupational exposure to HIV in health care workers, Silesia voivodeship].

BACKGROUND: Occupational exposure to HIV is defined as a contact of health care workers with potentially infectious material. The risk of occupational transmission is not high (0.09-0.3%), but it increases in case of percutaneous injuries caused by tools contaminated with infected blood, deep needle stick or direct contact of an infected needle with artery or vein. MATERIAL AND METHODS: The aim of the study was to determine the epidemiology of HIV infections among health care workers in the Silesian voivodeship, in the years 1999-2006 and the conditions of occupational exposure. Data on occup

Induction of pneumoperitoneum in morbidly obese patients.

BACKGROUND: Induction of pneumoperitoneum can be a difficult, time-consuming, and occasionally hazardous task in a morbidly obese patient. METHODS: We have induced pneumoperitoneum in 600 consecutive morbidly obese patients using a 120 mm Veress needle inserted <1 mm beneath the left costal margin, between the mid-clavicular and anterior axillary lines. Absolute muscular relaxation was necessary. RESULTS: A distinct "pop" was felt on entering the peritoneal cavity. The expected intraperitoneal pressure was 7-14 mmHg. A pressure >20 mmHg indicated that the Veress needle was in the abdominal w

Epidemiology of reported sharps injuries in a tertiary care hospital.

Over a 12-month period 233 puncture wounds were reported in a 1112-bed tertiary medical care centre. Accident forms were reviewed to determine the epidemiology of puncture injuries. The nursing department accounted for the majority of all puncture injuries (68%). For nurses, medical and surgical units represented those clinical areas with the greatest number of puncture injuries. Areas with the highest incidence rates, however, included the clinical research centre, the emergency room, the surgical intensive care unit and areas where the intravenous team operated. Activities involving direct c

Comparison of routes for achieving parenteral access with a focus on the management of patients with Ebola virus disease.

BACKGROUND: Dehydration is an important cause of death in patients with Ebola virus disease (EVD). Parenteral fluids are often required in patients with fluid requirements in excess of their oral intake. The peripheral intravenous route is the most commonly used method of parenteral access, but inserting and maintaining an intravenous line can be challenging in the context of EVD. Therefore it is important to consider the advantages and disadvantages of different routes for achieving parenteral access (e.g. intravenous, intraosseous, subcutaneous and intraperitoneal). OBJECTIVES: To compare t

Prevalence and correlates of needle-stick injuries among active duty police officers in Tijuana, Mexico.

INTRODUCTION: Police officers are at an elevated risk for needle-stick injuries (NSI), which pose a serious and costly occupational health risk for HIV and viral hepatitis. However, research on NSIs among police officers is limited, especially in low- and middle-income countries. Despite the legality of syringe possession in Mexico, half of people who inject drugs (PWID) in Tijuana report extrajudicial syringe-related arrests and confiscation by police, which has been associated with needle-sharing and HIV infection. We assessed the prevalence and correlates of NSIs among Tijuana police office

Inadvertent intraocular injection of botulinum toxin A.

This report describes an inadvertent perforation of the globe while injecting botulinum toxin A into the medial rectus muscle without electromyography guidance. Botulinum toxin A does not appear to have toxic effects on the human retina. Close observation and necessary measures to treat any retinal breaks and control increased intraocular pressure should be undertaken.

The effect of music-reinforced nonnutritive sucking on state of preterm, low birthweight infants experiencing heelstick.

This study examined the physiologic and behavioral effects of music-reinforced nonnutritive sucking (NNS) for preterm, low birthweight (LBW) infants experiencing heelstick. Subjects were 60 infants, age 32 to 37 weeks post conceptional age in a neonatal intensive care unit. Infants were randomly assigned to one of three treatment groups: pacifier-activated lullaby (PAL), pacifier-only, and no-contact. Experimental infants were provided the Sondrex PAL System, which plays music contingent on infant sucking. Pacifier-only infants did not receive music reinforcement for sucking, and no-contact in
