Juvenile Nasopharyngeal Angiofibroma in an Adult Patient: A Rare Presentation with Fahr Syndrome and Multiple Comorbidities-A Case Report and Literature Review.

Background and Clinical Significance: Juvenile nasopharyngeal angiofibroma (JNA) is a benign but locally aggressive vascular tumor, classically affecting adolescent males. Diagnosis in adulthood is exceptionally uncommon and may mimic other vascular or malignant nasopharyngeal lesions. This patient also had chronic hypocalcemia with Fahr-like intracranial calcifications secondary to long-standing postoperative hypoparathyroidism after thyroid carcinoma treatment. To our knowledge, this coexistence has not been previously reported. Case Presentation: A 34-year-old Caucasian male with papillary

A Case in Which the Endoscopic Denker's Approach Was Useful in the Diagnosis of IgG4-Related Ophthalmic Disease.

IgG4-related disease (IgG4-RD) is a chronic inflammatory condition characterized by elevated serum IgG4 levels, infiltration of IgG4-positive plasma cells, and fibrosis in various organs. We report the case of a 76-year-old man who presented with left-sided proptosis. Computed tomography revealed a mass lesion in the left orbit. An initial biopsy via a transnasal approach under local anesthesia was inconclusive. Although endoscopic sinus surgery was performed under general anesthesia, a definitive diagnosis could not be obtained. The lesion continued to enlarge, and subsequent ophthalmologic e

Radiological differentiation between bacterial orbital cellulitis and invasive fungal sino-orbital infections.

PURPOSE: Invasive fungal orbital infections (IFOI) may be difficult to differentiate from sinogenic bacterial orbital cellulitis (OC). This study investigates the features differentiating OC from IFOI on magnetic resonance imaging (MRI). METHODS: Retrospective study of adult patients with sinogenic OC and IFOI with pre-intervention MRI. Patients without post-septal involvement, non-sinogenic OC (e.g.: secondary to trauma) and poor-quality scans were excluded. Independent Sample's t test and Fisher's exact test were conducted with p < 0.05 deemed statistically significant. RESULTS: Eleven cas

Modified Transpterygoid Approach to Sphenoid Meningoencephaloceles: A Shorter Run for a Longer Slide.

OBJECTIVES: Cerebrospinal fluid (CSF) leaks and meningoencephaloceles originating in the lateral recess of the sphenoid sinus can be challenging. The traditional transpterygoid approach through the pterygopalatine fossa (PPF) is time consuming and places important structures at risk, which can lead to significant morbidity. We report a multi-institutional experience using a simplified, endoscopic modified transpterygoid approach (MTPA), which spares the PPF contents in the management of lateral sphenoid sinus meningoencephaloceles and CSF leaks. STUDY DESIGN: Multi-Institutional, Retrospectiv

Endoscopic transpterygoidal repair of a large cranial defect with cerebrospinal fluid leak in a patient with extensive osteoradionecrosis of the skull base: case report and technical note.

BACKGROUND: Endoscopic endonasal techniques have recently become the method of choice in dealing with cerebrospinal fluid leak involving the anterior cranial fossa. However, most surgeons prefer an intracranial approach when leaks involve the middle cranial fossa. This case report illustrates the possibilities of using endoscopic techniques for cerebrospinal fluid leaks involving the middle fossa. CASE REPORT: A 37-year-old male patient presented with multiple areas of cranial defect with cerebrospinal fluid leak due to osteoradionecrosis following radiation for nasopharyngeal carcinoma 4 yea

Midfacial Protraction With Skeletal Anchorage After Pterygomaxillary Separation.

The present article reports the treatment of a 7-year-old girl with maxillary hypoplasia associated with multiple tooth agenesis through maxillary protraction with skeletal anchorage and pterygomaxillary separation. Two titanium mini-plates were placed in the lateral region of the nasal cavity and used as anchorage for maxillary protraction with a reverse-pull facemask. Pterygomaxillary separation was also performed to enhance the effects of maxillary protraction. One week after surgery, 300 g of force was applied on each side to protract the maxilla. Active treatment time was 4 months, with 1

Transpalatal greater palatine canal injection: Radioanatomic analysis of where to bend the needle for pediatric sinus surgery.

BACKGROUND: The greater palatine canal (GPC) local injection is used to limit posterior bleeding during sinus surgery in adults. Given the potential for causing iatrogenic damage to the intraorbital contents, this procedure is not commonly used in the pediatric population. No studies have described the anatomic development of the GPC during facial growth. By using age-stratified radioanatomic analysis, the dimensions of the GPC and the clinical implications are described for pediatric patients. An age-stratified radioanatomic study was performed. METHODS: High-resolution computed tomography m

Ultrasound-guided coronoid approach to the trigeminal nerve block in a dog undergoing orbital exenteration.

An ultrasound-guided trigeminal nerve block using a coronoid approach was performed in a male neutered Jack Russell Terrier, aged 12 years, undergoing orbital exenteration for a retrobulbar mass. The previously reported temporal approach for this block was contraindicated as the location of the mass risked intersecting the tumour with the needle. With the dog anaesthetized and in left lateral recumbency, a microconvex transducer was placed caudal to the orbital ligament to obtain a transverse view of the pterygopalatine fossa. The angle of the probe was adjusted until the frontal bone and sphe

Sinonasal and Orbital Imaging Findings in COVID-Associated Rhino-Orbito-Cerebral Mucormycosis During the Second Wave of COVID-19: A Retrospective Cohort Study in a Tertiary Hospital in Central India.

Background Mucormycosis is a consequence of the angioinvasive disease caused by filamentous fungi that belong to the order Mucorales, particularly Mucor, Rhizopus, and Rhizomucor. Rhizopus oryzae is the most prevalent form. The invading hyphae lead to damage of blood vessels leading to thrombosis and consequent tissue necrosis. The incidence of this disease entity witnessed a significant rise during the second wave of the coronavirus disease 2019 (COVID-19) pandemic. Timely diagnosis and prompt treatment are crucial to diminish both the mortality and morbidity associated with this disease. Ima

Peripheral trigeminal nerve branch lesions: CT and MRI findings in a pathologically confirmed case series.

PURPOSE: The peripheral branches of the trigeminal nerve (V1, V2, and V3) follow an intricate and tortuous course through various skull base foramina and soft tissue spaces. Due to the rarity of lesions specifically involving these distal segments, radiological diagnosis remains a significant challenge. This study aims to summarize the characteristic imaging patterns of diverse pathologically confirmed lesions affecting the peripheral trigeminal branches using CT and MRI. METHODS: We retrospectively reviewed 18 patients (2008-2022) with pathologically proven peripheral trigeminal lesions. The
