Radiographic changes in the first metatarsal head after distal chevron osteotomy combined with lateral release through a plantar approach.

The purpose of this study was to evaluate the development of clinically significant avascular necrosis of the head of the first metatarsal after: (1) distal metatarsal osteotomy of the chevron configuration beginning apically at the center of the metatarsal head and extending into the head metatarsal neck junction and (2) release of the adductor hallucis muscle, the lateral capsulosesamoid ligament, and the lateral head of the flexor hallucis brevis via fibular sesamoidectomy in the majority of procedures (71/77). Although there were initial radiographic findings suspicious of avascular necros

MP I joint giving way--a case study.

A case history of a 26 year old international class female 400 m hurdle sprinter is presented. While sprinting she felt a sudden and very intensive pain at her left hallux. After this she was unable to run and had episodes of giving way in the MP I joint elicited by minor activity. Operative investigation revealed a broad disruption of the MP I medial collateral ligament. After periosteal flap repair and early functional aftertreatment she returned to full high level sports ability.

Automated skeletal maturity staging from handwrist radiographs using deep learning models: A decision support system based on Fishman's indicators.

OBJECTIVE: This study aimed to develop an automated system for skeletal maturity staging using deep learning (DL) models of hand-wrist radiographs based on Fishman's method. METHODS: In total, 2,318 hand-wrist radiographs of patients aged 8-19 years were retrospectively analyzed. Each radiograph was labeled by experts according to Fishman's method. This assessment was based on 11 skeletal maturity indicators (SMIs) across six anatomical regions, including the sesamoid bone, third and fifth fingers, and radius. Five different DL models (Faster R-CNN, FCOS, RetinaNet, SSD, and YOLOv7) were trai

Morphological characteristics of the flexor hallucis longus groove and tendon quality in patients with hallux rigidus: A CT-based study.

BACKGROUND: The etiology of hallux rigidus is still unknown and elucidating targeted factors to prevent hallux rigidus or improve treatment outcomes is essential. An association between flexor hallucis longus (FHL) tendon tightness and hallux rigidus development has been suspected; however, the characteristics of the FHL tendon in the hallux rigidus remain unclear. This study aimed to explore the quality of the FHL tendon and the morphological characteristics of the FHL tendon groove at the talus and sustentaculum tali in the pathogenesis of hallux rigidus. METHODS: Twenty-one feet with hallu

Analysis of Shortening and Elevation of the First Ray With Instrumented Triplane First Tarsometatarsal Arthrodesis.

BACKGROUND: The Lapidus procedure using planar saw resection has often been criticized for complications related to excessive shortening and elevation of the first ray. The goal of this study was to assess the amount of shortening that occurs when using a cutting guide for controlled saw resection of the first tarsometatarsal (TMT) joint surfaces, along with assessment of deformity correction in all 3 anatomic planes. METHODS: A prospective multicenter study with IRB approval included 35 hallux valgus subjects evaluated at baseline and 6 months following instrumented triplane first TMT arthro

Short term results of osteotomy-sparing technique in management of moderate hallux valgus using TightRope system.

BACKGROUND: Currently, correction of the first intermetatarsal angle associated with hallux valgus is done using two main categories of surgeries either a distal or a proximal first metatarsal osteotomy. The osteotomy-sparing technique using TigthRope ®device was developed to manage hallux valgus avoiding osteotomy complications. Our hypothesis was that osteotomy sparing technique using TightRope® implant would be suitable for moderate hallux valgus in young adult patients with strong larger bones giving good results with minimal complications. METHODS: 26 male patients (30 feet) with age ran

Uncommon causes of knee pain in the athlete.

There are many causes of knee pain in the athlete. Clearly, common things, such as meniscal tears, ligament sprains, contusions, and patellofemoral dysfunction, are common. The infrequency of other entities, however, should not make the physician complacent. A thorough history and physical examination will, in most cases, elicit inconsistencies in the occasional athlete presenting with one of these uncommon causes of knee pain (Table 1). Diagnosing these uncommon causes, when they do present to the health care provider, allows for early and correct treatment with a more rapid return to sports.

Surgical treatment of iatrogenic hallux varus.

Iatrogenic hallux varus is a dreaded complication of hallux valgus surgery, consisting in 1st-ray deformity in the form of medial malalignment of the 1st phalanx with respect to the metatarsal axis. Such over-correction results from imbalance between excessive medial capsule retraction or tensioning and excessive lateral laxity or soft-tissue release. There may be loss of medial stability of bone origin due to excessive "exostosectomy" or excessive intermetatarsal angle closure. Following excessive lateral release, the imbalance gradually induces a varus deformity of the 1st phalanx due to tra

Sesamoid malalignment in hallux valgus: Radiographic and MRI measurements and their correlation with internal derangement findings of the first metatarsophalangeal joint.

OBJECTIVE: Sesamoid displacement (SD) and rotation are important components in the preoperative assessment of hallux valgus (HV). To date, Inter reader reliability (IRR) of SD on X-rays and MRI, correlations with hallux valgus angle (HVA), and qualitative changes of the hallux-sesamoid complex (HSC) on MRI have not been studied. The aim of this study was to correlate sesamoid malalignment with HV severity and findings of internal joint derangement. METHODS: Two readers analyzed a series of 56 consecutive patients who had X-rays and MRI performed on the same foot within 3 months of each other.

Cumulative effects of lifelong systemic excess growth hormone on postcranial skeletal morphology in adult mice.

Many developmental and metabolic effects of growth hormone (GH) on vertebrate life history traits have been widely studied in biological and biomedical contexts. The scope of alterations in GH/GH receptor (GHR) interactions has generally focused on molecular, cellular, histological, and physiological framing, leaving a gap in understanding of how the cumulative organism- and lifespan-scale manifestations of morphological changes attributable to GH/GHR perturbations may both be influenced by and also influence smaller-scale study results. The pilot study conducted herein used micro-computed tom

Turf toe repair: a technical note.

Turf toe injury typically describes an injury to the metatarsosesamoid complex of the hallux generally caused by a hyperextension force to the great toe. This injury may be accompanied by pain, deformity, and decreased athletic performance. Operative treatment to repair the damaged tissue can be difficult, and we present a technique description that may help simplify the surgical reconstruction.

Morphology and deformity of the distal phalanx in hallux valgus.

BACKGROUND: Studies have shown that the first metatarsal contributes to hallux valgus. The proximal phalanx, another factor that defines the hallux valgus angle, also contributes to the development of hallux valgus. There have been no reports on the use of computed tomography to evaluate bone morphology of the proximal phalanx. The purpose of this study was to analyze the morphology and deformity of the proximal phalanx and its relationship to hallux valgus using computed tomography, and to consider the indications for proximal phalanx surgery in hallux valgus. METHODS: Patients who consulted

Functional sequelae after ulnar nerve repair. Study of forty-nine cases.

The full picture of functional ulnar deficiency, following suture or grafting, was studied and analyzed by two of the authors, who followed this series of patients on a long-term basis. The patients were tested using sphygmometric rubber bulbs in other to determine the overall closing force of the hand (making a fist), and the force of the pinch between the thumb and index ("energetic pinch"). Moreover, radionuclide vascular tests were used in order to find an eventual etiology of the "cold" disease, present in 2/3 of these injured hands. The principal functional deficiencies which bothered th

Arthroscopy of the First Metatarsophalangeal Joint.

Arthroscopic treatments for the first metatarsophalangeal joint have not spread fully owing to a number of factors. First, in the common arthroscopic view, the joint is observed from above, which makes the structures difficult to grasp and the dorsal aspect of the joint difficult to visualize and treat. Second, techniques for widening the view have not yet been reported. Third, the portals necessary for observation and treatment with hallux sesamoids have not yet been established. Finally, techniques for detecting the sesamoids have not been previously reported. We have developed a method for

Appraising the Welfare of Thoroughbred Racehorses in Training in Queensland, Australia: The Incidence and Type of Musculoskeletal Injuries Vary between Two-Year-Old and Older Thoroughbred Racehorses.

Musculoskeletal injuries (MSI) remain a concerning cause of racehorse morbidity and mortality with important ethical and welfare consequences. Previous research examining risk factors for MSI report inconsistent findings. Age is thought to affect MSI risk, but, to date, there have been no prospective studies comparing MSI in two-year-old versus older horses. This study aimed to: (1) determine the incidence of MSI for two-year-old and older horses, and whether this was affected by training track, season, or rainfall, and (2) determine the types of MSI affecting two-year-old and older horses, an
