[Diagnostics and therapy of dysphonia suitable for the ages and developmental stages of children and adolescents (part 2)].

The incidence of dysphonia among children and adolescents is evaluated internationally at 6 % to 25 %. Nonetheless, hoarseness as a leading symptom among children is often insufficiently recognized by their parents/attachment figures, by the young patients themselves, and even by physicians. In an overview, the hereditary and acquired organic and functional causes - including secondary organic lesions of the vocal folds - their pathomechanisms and the symptoms typical for this age group are presented. For diagnostics suitable for these age groups and developmental stages, modern methods of lar

High-speed video analysis of the phonation onset, with an application to the diagnosis of functional dysphonias.

An objective method for the diagnosis of functional dysphonias is presented. The mathematically motivated approach was evaluated on 71 female subjects with normal voice or functional dysphonia. Using digital high-speed recordings, the phonation onset process was recorded in real-time for 8-10 different sound pressure levels for each subject. From these recordings two parameters were mathematically estimated, reflecting the phonation onset dynamics. The growth of the vocal fold amplitudes during the phonation onset process was described by a parameter a for which its lower threshold value a(th)

Vocal fold augmentation by autologous fat injection with lipostructure procedure.

BACKGROUND: Fat is theoretically the ideal implant for vocal fold augmentation because it is soft, easily available and biocompatible. However, reabsorption is a frequent cause of long-term failure. OBJECTIVE: We prospectively tested Coleman's "lipostructure" technique used for facial recontouring in the treatment of glottic incompetence. STUDY DESIGN AND SETTING: Fourteen patients (aged 18-74 years, mean 48) with breathy dysphonia secondary to laryngeal hemiplegia (7) or anatomical defects (7) underwent vocal fold lipoinjection. Fat harvested by liposuction was centrifuged, and the fat cell

Characterization of Laryngopharyngeal Reflux in the Elderly Population.

INTRODUCTION: Laryngopharyngeal reflux (LPR) often is not diagnosed due to its presentation without heartburn. The aim of this study was to analyze the signs and symptoms of elderly patients undergoing treatment for LPR using subjective as well as objective measures including strobovideolaryngoscopies, 24-hour pH studies, and manometry to highlight appropriate considerations for diagnosis and treatment. METHODS: This was a retrospective review of patients with laryngopharyngeal reflux who were seen from January 2010 to February 2021 in the practice of the senior author (Robert T. Sataloff) wh

Vertical Phase Difference and Glottal Efficiency in Musical Theater and Opera Singers.

OBJECTIVE: The objectives of this study were to (1) determine the relationship between vertical phase difference (VPD) and glottal efficiency (GE) in singers and (2) to compare VPD and GE between musical theater and opera singing styles. METHODS: Five opera and three musical theater singers (Mean age = 25.3, 100% male) performed a series of vocal tasks at two pitches (low pitch C#3, 133 ± 5 Hz; high pitch C#4, 277 ± 5 Hz) for three levels of loudness (soft, baseline, and loud), while undergoing flexible distal chip videostroboscopy. Aerodynamic and acoustic information for each vocal task was

Severe Transient Pharyngeal Paralysis Following C2 Fracture Repair.

OBJECTIVES: We report a case of isolated pharyngeal plexus injury following posterior plating of a C2 fracture in an 84-year-old white male. METHODS: Methods include a case report with literature review. RESULTS: The patient presented with dense pharyngeal paralysis and inability to initiate swallowing but maintained true vocal cord movement. The patient required gastrostomy tube feeding for 3 months but eventually recovered his swallowing function. Prior literature on acute pharyngeal paralysis following upper cervical spine fracture repairs is reviewed with recommendations for care of this

Comparison of voice outcome after vocal fold augmentation with fat or calcium hydroxylapatite.

OBJECTIVES/HYPOTHESIS: To evaluate the short-term voice outcomes of vocal fold augmentation using fat compared to calcium hydroxylapatite product Radiesse Voice (Merz Aesthetics). STUDY DESIGN: Retrospective study design. METHODS: Sixty-six consecutive patients with vocal fold insufficiency were recruited, including the final 33 patients treated with fat and all 33 patients treated with calcium hydroxylapatite product. Before April 2011, fat augmentation was the standard treatment; from April 2011, only calcium hydroxylapatite product was used. For all subjects, videolaryngostroboscopic eval

[The effect of anchor voices and visible speech in training in the GRABS scale of perceptual evaluation of dysphonia].

INTRODUCTION: Perceptual evaluation of voice quality remains a key standard for judgment of vocal impairment. The GRABS method has become a commonly-used scale for rating severity of dysphonia, but it has no published, standardised protocol to follow. Training is important for reaching good interrater agreement for its parameters; however, the references most often cited for the GRABS provide no guidelines for clinical administration, speech material or rating calibration. This study investigated the effect of anchors (standard reference voices) and visible speech (narrow band spectrogram) in

Voice therapy for laryngeal hemiplegia: the role of timing of initiation of therapy.

OBJECTIVE: Laryngeal hemiplegia, also known as vocal fold paralysis, causes severe communicative disability. Although voice therapy is commonly considered to be beneficial for improving the voice quality in several voice disorders, there are only a few papers that present scientific evidence of the effectiveness of voice therapy in treating the disabilities of laryngeal hemiplegia. The aim of this study was to evaluate the outcomes of voice therapy in patients with laryngeal hemiplegia and to evaluate the role of the time gap between onset of laryngeal hemiplegia and initiation of therapy. Instability of voice in adolescence: pathologic condition or normal developmental variation?

OBJECTIVE: Pediatricians often send adolescents with dysphonia to the otorhinolaryngologist's office to find the reason for their hoarseness. The aim of this study was to identify the main characteristics of adolescent voice and to determine which characteristic (variable of voice analysis) can distinguish normal variations of voice development from pathologic disorders. STUDY DESIGN: On the basis of history, indirect laryngoscopy, and stroboscopy, 51 adolescents (22 boys, 29 girls) from age 10 to 17 years were divided into four subgroups: candidates for singing lessons without voice problems

Voice disorders in children.

The pediatric otolaryngologist has an especially important role in the differential diagnosis and treatment of two voice disorders; these are the voice quality problems (dysphony) and the resonance problems (rhinophony). The first step in the examination is to preclude the organic causes. The functional dysphonia is mostly related to voice abuse/misuse, but may be present on a psychosomatic basis; environmental factors can also play a role in the etiology and the personality structure has been found to be very relevant. The perceptual evaluation of voice is of obvious importance. Endoscopy wit

Two trajectories of functional recovery in thyroid surgery related unilateral vocal cord paralysis.

BACKGROUND: Iatrogenic unilateral vocal fold paralysis caused by thyroid surgery induces profound physical and psychosocial distress in patients. The natural course of functional recovery over time differs substantially across subjects, but the mechanisms underlying this difference remain unclear. In this study, we examined whether the anatomic site of the lesion affected the trajectory of recovery. METHODS: In this prospective case series study in a single medical center, patients with thyroid surgery-related unilateral vocal fold paralysis were evaluated using quantitative laryngeal electro

Medialization laryngoplasty for odynophonia.

IMPORTANCE: Medialization laryngoplasty (ML) remains the gold standard for the long-term treatment of hoarseness related to glottal insufficiency. Odynophonia is a less common manifestation of glottal insufficiency related to vocal fold motion impairment (VFMI), and ML is hypothesized to relieve pain associated with this condition. OBJECTIVE: To evaluate whether patients with VFMI and a chief complaint of odynophonia may achieve pain relief with ML, regardless of the degree of vocal impairment. DESIGN, SETTING, AND PARTICIPANTS: We conducted a retrospective review of medical records for 8 pa

The ram sign: detecting previously undiagnosed congenital laryngeal clefts in adults.

OBJECTIVES: We present the clinical characteristics of a case series of adult patients with type III laryngeal clefts according to the classification of Benjamin and Inglis, in an attempt to make practitioners aware of the "ram sign," a clinical finding associated with laryngeal clefts in adults. Laryngeal clefts are uncommon defects that are nearly universally identified during infancy as a result of persistent aspiration and pneumonia. Undiagnosed laryngeal clefts in adults are extremely rare. METHODS: Three type III laryngeal clefts were identified in adults (29, 48, and 60 years of age) f

Injection of cultured autologous fibroblasts for human vocal fold scars.

OBJECTIVES: This study evaluated the safety and effectiveness of injection of autologous fibroblasts into the lamina propria layer to treat vocal fold scarring. STUDY DESIGN: A prospective, open-label, single arm, pilot study at a single tertiary care center. METHODS: Autologous fibroblasts were expanded in cell culture from punch biopsies of the buccal mucosa of five human subjects with vocal fold scarring. Three doses of 1-2 × 10(7) cells/mL were injected into the superficial lamina propria layer of each scarred vocal fold at four-week intervals. The primary efficacy measure was an objecti

Prolonged ulcerative laryngitis: a new disease entity.

Over the last decade, a new disease entity, prolonged ulcerative laryngitis (PUL), with unique clinical presentation and prolonged disease course, has been recognized. Until now, very few studies dealing with this disease have been reported in the literature. From 1999 to 2008, we analyzed clinical data from a series of 39 PUL patients who were treated with an observational approach without implementing specific treatments. This disease affects adults, predominantly females. The age of patients in our series ranged from 26 to 76 years with a median of 49.5 years. This disease is characterized
