Cefepime-Zidebactam: Leveraging β-Lactam Enhancement Against Difficult-to-Treat and Metallo-β-Lactamase-Producing Gram-Negative Pathogens.

Carbapenem resistance among Gram-negative bacteria remains a serious public health threat, with metallo-β-lactamase (MBL)-producing organisms increasing substantially in prevalence and few therapeutic options available to adequately address these difficult-to-treat pathogens. Cefepime-zidebactam (FEP-ZID) is a novel β-lactam/β-lactam enhancer combination that offers a mechanistically distinct approach to overcoming resistance in Gram-negative organisms. Zidebactam contributes three complementary mechanisms: inhibition of Class A and C β-lactamases, direct antibacterial activity via PBP2 inhibi

Impact of initial disease severity and metabolic stress on mortality in patients with suspected external ventricular drain-associated infection.

External ventricular drainage (EVD) remains indispensable for managing intracranial pressure and cerebrospinal fluid (CSF) disorders. Despite advances in neurosurgical care and infection prevention, EVD-related infections still contribute substantially to morbidity and mortality. This study aimed to identify predictors of mortality and evaluate the clinical relevance of infection-related and metabolic parameters in patients with suspected EVD-associated infection undergoing EVD placement. We retrospectively analyzed all adult patients who underwent EVD insertion and were clinically suspected o

RND efflux pump inhibitor-like natural products as antibiotic adjuvants against multidrug-resistant gram-negative bacteria: a graded-evidence review.

Multidrug-resistant (MDR) Gram-negative bacteria-particularly carbapenem-resistant Enterobacterales, Pseudomonas aeruginosa and Acinetobacter baumannii-remain among the hardest infections to treat, and Resistance-Nodulation-Division (RND) efflux pumps are central to that resistance, making them a recurring target for antibiotic-adjuvant strategies. Natural products offer chemically diverse starting points for efflux-modulating adjuvants, but a fall in MIC is hard to attribute to pump inhibition: membrane permeabilisation, proton-motive-force (PMF) dissipation, efflux-gene downregulation, intri

Microbiota-driven therapeutic efficacy of Hyperoside in ulcerative colitis and associated anxiety.

BACKGROUND: Ulcerative colitis (UC) is subtype of inflammatory bowel disease that is frequently comorbid with anxiety disorders. However, effective dual-targeting therapies are still lacking. Hyperoside (HYP), a natural flavonoid, exhibits anti-inflammatory and neuroprotective properties, yet its potential therapeutic effects on UC and associated anxiety, as well as the underlying mechanisms, remain largely unexplored. METHODS: A murine model of DSS-induced colitis was established and treated with HYP. Disease activity was assessed through body weight, colon length, and histopathology. Anxiet

[New antimicrobial substances to combat increasing resistance].

Increasing antimicrobial resistance (AMR) is one of the greatest threats to global health. In 2021, 4.71 million deaths worldwide were closely associated with AMR, and 1.14 million deaths could be directly attributed to infections caused by multidrug-resistant organisms (MDROs), particularly multidrug-resistant Gram-negative bacteria (MDRGN). Enterobacterales (e.g., Escherichia coli and Klebsiella spp.) resistant to third-generation cephalosporins and carbapenems, multidrug-resistant Pseudomonas aeruginosa, and carbapenem-resistant Acinetobacter baumannii (CRAB) have been identified by the Wor

Multifaceted antimicrobial mechanisms of NCR147-derived peptides from Medicago truncatula.

INTRODUCTION: Antimicrobial peptides (AMPs), key components of innate immunity, offer broad-spectrum activity against diverse pathogens. In Medicago truncatula, over 700 nodule-specific cysteine-rich (NCR) peptides with highly diverse sequences and physicochemical properties are produced in the symbiotic cells of root nodules, where cationic members block bacterial cell division and display potent antimicrobial activity in vitro. In contrast, anionic NCRs typically lack antimicrobial effects, and NCR147-a neutral peptide-is the only known non-cationic NCR that shows weak bactericidal activity.

Quantitative interpretation models for targeted next-generation sequencing in lower respiratory tract infections: a multicenter prospective study.

BACKGROUND: Lower respiratory tract infections (LRTIs) represent a significant global health burden. While targeted next-generation sequencing (tNGS) offers potential advantages for pathogen detection, its clinical implementation is hindered by the absence of validated quantitative interpretation criteria for pathogen discrimination. METHODS: We conducted a multicenter prospective study of 631 patients with suspected LRTIs across five intensive care units in eastern China from January 2022 to March 2025. Bronchoalveolar lavage fluid specimens underwent concurrent tNGS and conventional microbi

Real-world outcomes and safety of colistin therapy in children with multidrug-resistant gram-negative infections: a nine-year experience.

Antimicrobial resistance (AMR) among Gram-negative (GN) pathogens has emerged as a critical global threat, particularly in low- and middle-income countries where access to novel antimicrobials is limited. In Türkiye, colistin (CST) remains a last-resort therapy against multidrug-resistant (MDR) and carbapenem-resistant Gram-negative bacteria (CR-GNB) despite its nephrotoxic potential. This study aimed to evaluate the clinical characteristics, microbiological outcomes, and adverse events associated with CST therapy in pediatric patients over a nine-year period. We conducted a single-center retr

Comparative analysis of gut and ovarian microbiota in Dactylopius opuntiae (Hemiptera: Dactylopiidae) from distinct Moroccan regions.

INTRODUCTION: Investigating the hidden microbial world within insect hosts yields new insights into their biology, ecology, and adaptation. Dactylopius opuntiae (Cockerell), a major invasive pest of the cactus Opuntia ficus-indica, causes severe economic and ecological damage in Morocco and other arid regions. However, little is known about its associated bacterial microbiota and how it varies across environmental gradients. This study explored the bacterial diversity of D. opuntiae and evaluated how it varies across tissue types, developmental stages, and contrasting environmental regions. M

Analysis and Diagnostic Significance of Cerebrospinal Fluid Laboratory Parameters in Patients with Post-Craniotomy Meningitis.

BACKGROUND: Post-craniotomy meningitis (PCM) is a common and serious infectious complication in neurosurgical practice, often associated with considerable morbidity and healthcare burden. Due to the lack of specificity in its clinical manifestations, early diagnosis remains a significant challenge. Although cerebrospinal fluid (CSF) culture is currently regarded as the diagnostic gold standard, its effectiveness is limited by stringent pathogen growth requirements and a relatively low positivity rate, rendering it insufficient to meet clinical needs. Therefore, this study aimed to evaluate the
