Giant cell reparative granuloma of the temporal bone.

Giant cell reparative granuloma (GCRG) is an uncommon non-neoplastic reactive tumor that occurs almost exclusively within the mandible and maxilla and can be locally aggressive. Only sporadic cases involving the skull base have been reported. However, this lesion is probably underappreciated because it might be unrecognized or misdiagnosed. We present a case of GCRG of the left temporal bone that was treated surgically via a combined transtemporal-subtemporal approach. A short literature review about diagnosis, clinical behavior and treatment of this tumor entity is given.

Transient facial palsy in two cases of benign, very rare middle ear tumors (carcinoid tumor and myxoma).

OBJECTIVE: Presentation of the clinical features of 2 very rare middle ear tumors in which the guiding symptom was facial palsy. MATERIAL AND METHODS: Illustrative case reports about a myxoma and a carcinoid tumor of the middle ear associated with peripheral facial palsy. RESULTS: The facial palsy was transient in either case, and its pathomechanism is open for discussion. In both cases, the initial symptoms were typical for an inflammatory process. Moreover, both tumor entities are typically found in organs other than the ear; if located in the middle ear, those neoplasms grow rather superf

Endolymphatic sac enhancement: reversal of pathogenesis.

As this article and many other publications have indicated, many thousands of patients with incapacitating intractable progressive Meniere's disease have had their lives restored through endolymphatic sac enhancement surgery. This is the only nondestructive procedure for Meniere's disease that has stood the test of time. Destructive procedures such as labyrinthectomy (chemical or physical) and vestibular nerve section are reserved for rare cases of failure or recurrence after sac enhancement, despite revision of the enhancement. There is much evidence to support the concept that endolymphatic

Arachnoid cysts and pneumosinus dilatans.

The association of intracranial arachnoid cysts and pneumosinus dilatans is considered to be relatively rare, and has previously been described in only a limited number of cases. In this study the CT and/or MRI examinations of a total of 30 cases with middle cranial fossa arachnoid cysts were retrospectively evaluated in an attempt to note any association with pneumosinus dilatans of the paranasal sinuses. In four patients with surgically treated, large, middle cranial fossa arachnoid cysts, obvious paranasal sinus changes, consistent with pneumosinus dilatans, were noted. The frequency of the

Efficacy of tympanomastoid surgery for control of infection in active chronic otitis media.

The efficacy of surgery in controlling infection in 272 tympanomastoidectomy procedures for chronic otitis media (COM) was assessed by means of a four-point rating scale that incorporated both symptoms and signs, such as the presence or absence of otorrhea and granulation tissue. Of the 272 procedures, 170 were performed for COM with cholesteatoma and 102 were for active COM with granulation tissue but no cholesteatoma. Forty-seven percent were primary procedures, and 53% were revisions. Minimum follow-up was 12 months for all cases, with a mean of 30 months. Adequate control of infection occu

The role of surgery in necrotizing otitis externa.

This retrospective case review describes a subset of 5 patients with necrotizing otitis externa (NOE) with a refractory disease course who underwent surgery as part of their management plan between 2008 and 2013. Surgery promoted the cure of 4 of the 5 patients, and a fungal pathogen was recovered in 4 of 5 surgical samples. We conclude that surgery may be a necessary diagnostic and treatment adjunct in selective cases of NOE, especially in patients with a refractory disease course or with a suspected fungal etiology.

Does Checking the Placement of Ossicular Prostheses via the Posterior Tympanotomy Improve Hearing Results After Cholesteatoma Surgery?

INTRODUCTION: Posterior tympanotomy (PT) is often performed during the surgical management of middle ear cholesteatoma with extension in the retrotympanum area. This PT can also be used to control the right position of the ossicular prosthesis masked by the tympanic membrane reconstruction. OBJECTIVE: To compare audiologic results after ossiculoplasty performed via the outer ear canal and via the PT for patients with cholesteatoma. MATERIALS AND METHODS: Retrospective chart reviews were performed for 68 patients (68 ears) with cholesteatoma who underwent titanium ossicular prosthesis surgery

Bilateral implant-retained auricular prosthesis for a patient with congenitally missing ears. A clinical report.

Microtia is a major congenital anomaly of the external ear. It includes a spectrum of deformities from a grossly normal but small ear to the absence of the entire external ear. These deformities account for three in every 10,000 births, with bilaterally missing ears seen in fewer than 10% of all cases. Congenital abnormalities of the ear are unlikely to result in the complete absence of the ears, but the patient presented in this article had bilateral congenitally missing ears. There was loss of anatomic landmarks and alteration of normal bony architecture. Minimal tissue was available for ret

Robot-assisted placement of craniofacial implants.

PURPOSE: The purpose of this study was to improve and accelerate the rehabilitation process for patients with severe ear microtia with an implant-anchored auricular prosthesis. A medically approved robot system was used to place the craniofacial implants and a new process was developed for preoperative fabrication of the prosthesis using a rapid prototyping technique. MATERIALS AND METHODS: Preoperatively, after computerized tomography, the implant positions were determined in a planning tool according to bone availability and esthetic considerations. Intraoperatively, the robot showed the su

Cranial base approaches to posterior circulation aneurysms.

Aneurysms arising from the posterior circulation, especially when they are large and complex, continue to present a technical challenge. The development of cranial base strategies and principles has added to surgical management options. The authors used one of four cranial base approaches for the treatment of 30 patients with large and/or complex aneurysms arising from the vertebrobasilar circulation. These approaches included the extradural temporopolar, combined petrosal, retrolabyrinthine-transsigmoid, and the extreme-lateral inferior transtubercular exposure. The indications, technique, an

Vestibular evoked myogenic potentials in the sternomastoid muscle are not of lateral canal origin.

We studied vestibular evoked myogenic potentials in 6 patients after unilateral vestibular neurectomy and in 22 patients after unilateral vestibular neuritis and unilateral absent caloric responses. We found that the ipsilesional vestibular evoked potentials were abolished in every patient after unilateral vestibular neurectomy. In vestibular neuritis patients we found that the ipsilesional vestibular evoked potentials were absent in some but not in all cases. These findings confirm that the p13-n23 potential is of vestibular origin and also show that it is not of lateral canal origin.

Preoperative computed tomography in patients requiring a bone-anchored hearing aid.

A preoperative computed tomography technique, "Dentascan" or "Dentatool", was used to find the optimal site for implant installation in patients requiring a bone-anchored hearing aid. The technique is demonstrated in a patient with bilateral bone conduction hearing loss who was treated with bilateral bone-anchored hearing aids.

Extreme lateral transcondylar approach: technical improvements and lessons learned.

An extreme lateral transcondylar or extreme lateral transfacetal surgical approach was used to treat 22 patients with complex lesions over a 22-month period. The lesions included basilar invagination with vertebral artery pathology, giant aneurysm or arteriovenous fistula of the vertebral artery, meningioma, chordoma, chondrosarcoma, and paraganglioma. The approach was used alone or in combination with a presigmoid petrosal or subtemporal-infratemporal approach. Refinements of the operative technique, treatment strategies for complex lesions, and the avoidance of complications are discussed. C

Tympanoplasty surgery at the Red Cross War Memorial Children's Hospital 1986-1988.

Results of tympanoplasty surgery over a 3 year period in 96 children aged 2-12 years were analysed to better define indications for surgery and prognostic factors. Nineteen percent had a hearing loss associated with a dry perforation--grafts were successful in 84% and 71% improved hearing to less than 20 dB. Forty-two percent had a hearing loss but also troublesome recurrent infection--grafts were successful in 88% but only 65% improved hearing to less than 20 dB. Thirteen percent had troublesome recurrent infection--grafts were successful in 79%. Twenty-two percent had uncontrolled infection,

Temporal bone histopathological findings in trisomy 13 syndrome.

This study reports the histopathological findings of 14 temporal bones from infants with trisomy 13 syndrome. The most primitive anomalies in the structures of the inner and middle ears in the present series are those of the semicircular canals, particularly of the horizontal canals: flattened horizontal canal cristae, absence or opening of the utricular endolymphatic valve, small facial nerve, and obtuse angle of the geniculate area of the facial nerve. Each ear demonstrated more than one of those anomalies. The anomalies present features similar to those found in the structures of the normal

Post mortem cadaveric and imaging mapping analysis of the influence of cochlear implants on cMRI assessment regarding implant positioning and artifact formation.

OBJECTIVES: In times of an aging society and considering the escalating health economic costs, the indications for imaging, particularly magnetic resonance imaging (MRI), must be carefully considered and strictly adhered to. This cadaver study aims to examine the influence of cochlear implant (CI) on the assessment of intracranial structures, artifact formation, and size in cranial MRI (cMRI). Furthermore, it seeks to evaluate the potential limitations in the interpretability and diagnostic value of cMRI in CI patients. Additionally, the study investigates the imaging of the brain stem and the
