Therapeutic Options and Outcomes for the Treatment of Neonates and Preterms with Gram-Negative Multidrug-Resistant Bacteria: A Systematic Review.

(1) Background: Infections caused by multidrug-resistant (MDR) or extensively drug-resistant (XDR) bacteria represent a challenge in the neonatal population due to disease severity and limited therapeutic possibilities compared to adults. The spread of antimicrobial resistance and drug availability differ significantly worldwide. The incidence of MDR bacteria has constantly risen, causing an increase in morbidity, mortality, and healthcare costs in both high-income (HIC) and low- and middle-income countries (LMIC). Therefore, more evidence is needed to define the possible use of newer molecule

Replication Dynamics for Six Gram-Negative Bacterial Species during Bloodstream Infection.

Bloodstream infections (BSI) are a major public health burden due to high mortality rates and the cost of treatment. The impact of BSI is further compounded by a rise in antibiotic resistance among Gram-negative species associated with these infections. Escherichia coli, Serratia marcescens, Klebsiella pneumoniae, Enterobacter hormaechei, Citrobacter freundii, and Acinetobacter baumannii are all common causes of BSI, which can be recapitulated in a murine model. The objective of this study was to characterize infection kinetics and bacterial replication rates during bacteremia for these six pa

Chemogenomic model identifies synergistic drug combinations robust to the pathogen microenvironment.

Antibiotics need to be effective in diverse environments in vivo. However, the pathogen microenvironment can have a significant impact on antibiotic potency. Further, antibiotics are increasingly used in combinations to combat resistance, yet, the effect of microenvironments on drug-combination efficacy is unknown. To exhaustively explore the impact of diverse microenvironments on drug-combinations, here we develop a computational framework-Metabolism And GENomics-based Tailoring of Antibiotic regimens (MAGENTA). MAGENTA uses chemogenomic profiles of individual drugs and metabolic perturbation

Evaluation of intraventricular colistin utilization: A case series.

PURPOSE: Multi-drug resistant organisms (MDROs) are an increasing concern in health systems. Pathogens such as Pseudomonas aeruginosa, Acinetobacter baumanii, and carbapenamase-producing Enterobacteriaceae hold highest mortality rates especially when the central nervous system is involved. When MDROs are cultured treatment options are limited and reliance on medications such as colistin is becoming more prevalent. Penetration of these therapies into the central nervous system is concerning therefore local administration is a potential concomitant therapy. METHODS: This study was a retrospecti

ESKAPE Gram-negative bacteria escape culture-based detection upon desiccation on abiotic surfaces.

ESKAPE bacteria, namely Enterococcus spp., Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa, and Enterobacter spp., are leading causes of hospital-acquired infections and a major therapeutic challenge due to multidrug resistance. Hospital surfaces and medical devices are critical reservoirs for the transmission of these pathogens to patients. Standard methods for detecting microorganisms in the hospital environment are culture-based, so they cannot identify bacteria in the viable but non-culturable (VBNC) state. VBNC bacteria remain metabolically ac

Discovery and functional characterization of a novel PilV variant in a mcr-1-positive IncI2 plasmid.

IncI2-type plasmids are major vehicles for the dissemination of mcr-1. These plasmids encode a type IV secretion system (T4SS) that mediates conjugative transfer and a type IV pilus (T4P) that facilitates mating-pair formation. Shufflon-mediated diversity of tip adhesin PilV of T4P may contribute to the extensive dissemination of IncI2 plasmids. Herein, we identified a novel shufflon segment forming an active new PilV variant from an IncI2 plasmid, which was named PilV-E and was detected in 127 of 501 (25.35%) complete IncI2 plasmids in public databases. We further demonstrated that strains ha

Specimen-specific distribution of multidrug-resistant organisms and its association with glycemic control in adults with diabetes: a retrospective clinical microbiology study in southern Taiwan.

BACKGROUND: Patients with diabetes mellitus are susceptible to bacterial infections and may be exposed to repeated or prolonged antimicrobial therapy, which can increase the risk of multidrug-resistant organism (MDRO)-positive cultures. However, the distribution of MDROs across different specimen types and the relationship between glycemic control and MDRO isolation remain insufficiently characterized in regional clinical microbiology settings. This study investigated specimen-specific MDRO profiles and evaluated the association between glycated hemoglobin (HbA1c) levels and MDRO-positive cult

Enhanced microbiological evaluation of brain abscesses by proteomics-based techniques: a prospective cohort study.

OBJECTIVE: Microbiological analysis might not be able to identify the causative pathogen in one-third of brain abscess patients. Proteomics analysis of brain abscesses can aid in microbiological diagnosis. This could help tailor the antimicrobial therapy, give possible insights into the source of the abscess, and aid in developing targeted antimicrobial agents. METHODS: This was a single-center prospective study including patients with brain abscess recruited over a 1-year period. Proteome-based analysis was used to identify the causative pathogen, and the results were compared using conventi
