Degeneration in ACL Injured Knees with and without Reconstruction in Relation to Muscle Size and Fat Content-Data from the Osteoarthritis Initiative.

BACKGROUND: Anterior cruciate ligaments (ACL) injuries represent a major risk factor for early osteoarthritis (OA). PURPOSE: To evaluate the prevalence and 4-year progression of knee OA measured with 3T MR-imaging in individuals with ruptured, reconstructed or normal ACL and to assess the impact of thigh muscle characteristics. METHODS: A total of 54 knees (23/54 male, 31/54 female) were recruited from the Osteoarthritis Initiative (OAI). At baseline, 15/54 subjects had prevalent ACL ruptures and 15/54 subjects had prevalent ACL reconstruction (24/54 normal ACL). Western Ontario and McMaster

Achilles tendon allograft for augmentation of the Hanssen patellar bone grafting.

PURPOSE: Severe patellar bone loss precludes the insertion of another patellar implant in revision total knee arthroplasty (TKA) and weakens the attachment of the patella to the quadriceps tendon. Hanssen described securing a tissue flap to the patellar rim allows the delivery of cancellous bone graft into the patellar bone defect. The purpose of this study is to describe the surgical technique and early clinical results of a modified Hanssen patelloplasty with an Achilles tendon allograft. METHODS: The modified technique described in three patients undergoing revision TKA aims to augment the

Electrical stimulation accelerates motor functional recovery in autograft-repaired 10 mm femoral nerve gap in rats.

Electrical stimulation has been shown to enhance peripheral nerve regeneration after nerve injury. However, the impact of electrical stimulation on motor functional recovery after nerve injuries, especially over long nerve gap lesions, has not been investigated in a comprehensive manner. In the present study, we aimed to determine whether electrical stimulation (1 h, 20 Hz) is beneficial for motor functional recovery after a 10 mm femoral nerve gap lesion in rats. The proximal nerve stump was electrically stimulated for 1 h at 20 Hz frequency prior to nerve repair with an autologous graft. The

Muscle herniation: a complication at the anterolateral thigh perforator flap donor site.

The anterolateral perforator flap is becoming the flap of choice for a wide variety of complex defects. It has been known to provide excellent donor site morbidity even in cases where vastus lateralis is included within the flap. We report a case of herniation of the vastus lateralis and rectus femoris muscles through the overlying fascia, following ALT perforator flap harvest. In this case muscle herniation at the donor site required surgical repair.

Cadence effects on cardiorespiratory and muscular responses during eccentric cycling.

PURPOSE: Eccentric cycling (ECC) enables high mechanical power output at a low metabolic cost, yet the influence of cadence on cardiorespiratory and neuromuscular responses during ECC remains poorly defined. This study examined the interactive effects of cadence and power output on metabolic, cardiovascular, and muscle activation responses during ECC. METHODS: Fifteen healthy men performed randomized incremental ECC trials at 50 and 80 rpm, with successive 4-min stages at 80, 200, and 320 W. Oxygen uptake [Formula: see text], gross mechanical efficiency (GME), cardiac output [Formula: see tex

Effect of Dynamic Neuromuscular Stabilization Along With Aquatic Exercises on Pain, Functional Mobility, and Muscle Performance in Bilateral Medial Compartment Knee Osteoarthritis.

BACKGROUND: Bilateral medial compartment knee osteoarthritis (KOA) is characterized by the gradual breakdown of joint structures, which frequently leads to chronic pain, reduced flexibility, and compromised physical function. Although exercise serves as the foundation of non-pharmacological care, there is still a lack of consensus regarding the ideal integration of different therapeutic interventions. Dynamic Neuromuscular Stabilization (DNS) and aquatic therapy have independently demonstrated benefits in improving neuromuscular control and reducing joint loading. OBJECTIVE: The objective of

Successful corticosteroid-free management of anti-HMG-CoA reductase immune-mediated necrotizing myopathy with subcutaneous immunoglobulins monotherapy: a case report.

BACKGROUND: Given the high rate of cardiovascular comorbidities in anti-HMG-CoA reductase (HMGCR)-immune-mediated necrotizing myopathy (IMNM), a glucocorticosteroids (GC)-free treatment approach remains an appealing strategy. While intravenous immunoglobulins (IVIg) represents an effective therapy in most subgroups of idiopathic inflammatory myositis (IIM), there remains limited evidence supporting the use subcutaneous immunoglobulins (SCIg), a potentially safer, more convenient and cost-effective alternative. CASE PRESENTATION: We present a case of a 68-year-old male of European descent with

Paravertebral block vs. epidural block for percutaneous nephrolithotomy: A prospective, randomized study.

BACKGROUND: Percutaneous nephrolithotripsy (PCNL) is the main method for urinary calculi. An anesthesia method with little effect on the blood circulation and which does not affect the postoperative activity of a patient is lacking. OBJECTIVE: To compare the effects of paravertebral nerve block (PNB) and epidural block (EPB) on quadriceps femoris muscle (QFM) strength in patients after PCNL. METHODS: 163 patients were separated into two groups: EPB (81) and PNB (82). Primary outcome parameters were QFM strength and range of motion (RoM) of the knee 1 h, 2 h, 3 h, and 24 h after anesthesia in

Joint effusion, anteroposterior stability, muscle strength and degree of patellofemoral osteoarthritis significantly impact outcome following revision ACL reconstruction.

PURPOSE: Effusion, impaired muscle function and knee instability are considered as some of the most important factors effecting outcome following anterior cruciate ligament reconstruction (ACL-R) but the impact on revision ACL-R remains unclear. It was hypothesized that these factors will significantly worsen clinical outcome following revision ACL-R. METHODS: Seventy knees (13 female and 57 male) were followed retrospectively after revision ACL-R at a mean follow-up of 47.8 ± 20.7 months. Clinical examination was based on the International Knee Documentation Evaluation Form-2000 (IKDC), Tegn

Development of a Standard Set of Outcome Domains and Proposed Measures for Chronic Obstructive Pulmonary Disease in Primary Care Physical Therapy Practice in the Netherlands: a Modified RAND/UCLA Appropriateness Method.

BACKGROUND: Standardization of measures in a common set opens the opportunity to learn from differences in treatment outcomes which can be used for improving the quality of care. Furthermore, a standard set can provide the basis for development of quality indicators and is therefore useful for quality improvement and public reporting purposes. The aim of this study was to develop a standard set of outcome domains and proposed measures for patients with COPD in Dutch primary care physical therapy practice, including a proposal to stratify patients in subgroups. MATERIAL AND METHODS: A consensu

Dynamic stretching alone can impair slower velocity isokinetic performance of young male handball players for at least 24 hours.

There are many adult studies reporting static stretch (SS)-induced deficits and dynamic stretch (DS) performance improvements shortly after the intervention. However, there is only a single study examining stretch-induced performance changes with youth at 24 hours' post-stretch. The objective of this study was to examine physiological responses of young trained athletes at 24-hours after experiencing SS or DS protocols. Eight young male, elite handball players (age: 16.1±5.1 years) were tested prior to-, 3-minutes and 24-hours following the three conditions (DS, SS, Control) in a randomized an

Posterior Cruciate Ligament Injuries Associated With Military Survival Swim Training.

OBJECTIVES: Posterior cruciate ligament (PCL) injuries are relatively common injuries associated with athletic activities and high-energy trauma. Posterolateral corner (PLC) injuries frequently accompany injury to the PCL. Diagnosis can be challenging and requires a comprehensive history and physical examination. Patients frequently report vague, nonspecific symptoms and the mechanism of injury is often useful in localizing injured structures. Two of the more common mechanisms for PCL injury include a direct blow to the proximal anterior tibia with the knee flexed, as well as a significant kne
