An ultrasound imaging and computational fluid dynamics protocol to assess hemodynamics in iliac vein compression syndrome.

OBJECTIVE: Elevated shear rates are known to play a role in arterial thrombosis; however, shear rates have not been thoroughly investigated in patients with iliac vein compression syndrome (IVCS) owing to imaging limitations and assumptions on the low shear nature of venous flows. This study was undertaken to develop a standardized protocol that quantifies IVCS shear rates and can aid in the diagnosis and treatment of patients with moderate yet symptomatic compression. METHODS: Study patients with and without IVCS had their iliac vein hemodynamics measured via duplex ultrasound (US) at two of

Outcome of Staged Repair of Tetralogy of Fallot with Pulmonary Atresia and a Ductus-dependent Pulmonary Circulation: Should Primary Repair Be Considered?

BACKGROUND: The tetralogy of Fallot (TOF) with pulmonary atresia (PA) and a ductus-dependent pulmonary circulation (no major aorto-pulmonary collateral arteries (MAPCAs)) has been treated with staged repair or primary repair depending on the preference of surgeons or institutions. We evaluated the 19-year outcome of staged repair for this anomaly to find out whether our surgical strategy should be changed. MATERIALS AND METHODS: Forty-four patients with TOF/PA with patent ductus arteriosus (PDA) who underwent staged repair from June 1991 to October 2010 were included in this retrospective stu

[New orientations in the management of advanced, metastatic gastrointestinal stromal tumors (GIST): combination of surgery and systemic therapy with imatinib in a case of primary gastric location].

Gastrointestinal stromal tumours (GIST) are rare neoplasms originating from connective tissue in the digestive tract with an incidence of less than 1% and account for most non-epithelial primitive digestive tumours. Metastasis diagnosed at the time of disease discovery confirms GIST malignancy. Kit protein, a trans-membrane tyrosine kinase receptor of staminal cells, is characteristically expressed by GIST. Most GIST have a mutation in the kit proto-oncogene. Resistance to conventional chemotherapy is commonly shown by malignant GIST. Most patients with advanced malignant GIST achieve clinical

[Deep venous thrombosis: epidemiologic, diagnostic and therapeutic aspects].

Deep vein thrombosis (DVT) has a high social and economic cost disease being its prevalence in the general population elevated and producing possibly fatal (pulmonary embolism) or disabling (post-thrombotic syndrome) complications. Thus, it appears of great importance to know the epidemiological and clinical characteristics of DVT in order to perform the best diagnosis, therapy and prophylaxis. The study population is composed by 146 patients (84 males and 62 females, mean age 60.9 +/- 15.3 years, range 19.92 years), arrived in our Vascular Echography Laboratory with the clinical suspect of DV

Variations in surgical procedures for hind limb ischaemia mouse models result in differences in collateral formation.

OBJECTIVE: To identify the optimal mouse model for hind limb ischaemia, which offers a therapeutic window that is large enough to detect improvements of blood flow recovery, for example, using cell therapies. MATERIALS AND METHODS: Different surgical approaches were performed: single coagulation of femoral and iliac artery, total excision of femoral artery and double coagulation of femoral and iliac artery. Blood flow restoration was analysed with laser Doppler perfusion imaging (LDPI). Immuno-histochemical stainings, angiography and micro-computed tomography (CT) scans were performed for vis

Neurological status and tissue perfusion changes after radial artery harvesting for myocardial revascularization: importance of the Allen test.

OBJECTIVE: Although radial artery (RA) removal for coronary revascularization is considered safe, there is still considerable suspicion concerning adequate hand perfusion. We investigated the blood flow alterations in the donor forearm by quantitative perfusion scintigraphy tests in patients with RA conduits, and compared the results between the patients using different Allen test refill lengths . METHODS: 50 patients undergoing coronary artery bypass graft surgery with RA grafts were investigated. Perfusion studies and a detailed physical examination of the donor hand were performed preopera

Vasoconstrictor use in liver transplantation: is there evidence for rational use?

Liver transplantation is a challenging surgical operation performed in recipients with major hemodynamic perturbations related to portal hypertension. The pathophysiologic alterations in portal hypertension include a hyperdynamic circulation and decline in systemic vascular resistance and mean arterial pressure. Cardiac function can also be depressed due to cirrhosis related cardiomyopathy. These cirrhosis related changes often lead to a tenuous state in which organ perfusion is threatened and declines rapidly in the setting of many other insults including blood loss, infection, and use of med

Creating arteriovenous fistulas in patients with chronic central venous obstruction.

PURPOSE: Central venous obstruction Occlusion (CVO) has been considered a contraindication for creating a vascular access due to fear of developing a swollen extremity. However, many of these individuals developed large collateral veins and are asymptomatic. We report our experience constructing arteriovenous fistulas (AVFs) in these challenging patients. METHODS: Patients with a new AVF constructed in the presence of known CVO were identified. Venous imaging confirmed proximal obstruction and extensive collateral venous return. The AVF was constructed in the extremity with the most favorable

Emergency EC-IC bypass for symptomatic atherosclerotic ischemic stroke.

Previous studies have shown that extracranial-intracranial (EC-IC) bypass surgery has no preventive effect on subsequent ipsilateral ischemic stroke in patients with symptomatic atherosclerotic internal carotid occlusion and hemodynamic cerebral ischemia. A few studies have assessed whether an urgent EC-IC bypass surgery is an effective treatment for main trunk stenosis or occlusion in acute stage. The authors retrospectively reviewed 58 consecutive patients who underwent urgent EC-IC bypass for symptomatic internal carotid artery or the middle cerebral artery stenosis or occlusion between Jan

Aortic valve disease in pulmonary atresia and major aortopulmonary collaterals.

BACKGROUND: Aortic valve disease in association with pulmonary atresia, ventricular septal defect, and major aortopulmonary collaterals (PA/VSD/MAPCAs) is an extremely rare combination of congenital heart defects. The presence of aortic stenosis or insufficiency or both imposes an additional physiologic burden that complicates the management of PA/VSD/MAPCAs. This report summarizes our experience with 7 patients who underwent surgical repair of this rare combination of defects. METHODS: This was a retrospective review of patients who had both aortic valve disease and PA/VSD/MAPCAs. Four patie

The outcome of failed endografts inserted for superficial femoral artery occlusive disease.

OBJECTIVE: Endografts represent a relatively new treatment modality for occlusive disease of the superficial femoral artery, with promising results. However, endografts may occlude collateral arteries, which may affect outcome in case of failure. The purpose of this study was to analyze the clinical outcome of failed endografts in patients with superficial femoral artery occlusive disease. METHODS: All patients treated with one or more polytetrafluorethylene-covered stents between November 2001 and December 2011 were prospectively included in a database. Patients with a failure of the endogra

Induction of smooth muscle cell migration during arteriogenesis is mediated by Rap2.

OBJECTIVE: Collateral artery growth or arteriogenesis is the primary means of the circulatory system to maintain blood flow in the face of major arterial occlusions. Arteriogenesis depends on activation of fibroblast growth factor (FGF) receptors, but relatively little is known about downstream mediators of FGF signaling. METHODS AND RESULTS: We screened for signaling components that are activated in response to administration of FGF-2 to cultured vascular smooth muscle cells (VSMCs) and detected a significant increase of Rap2 but not of other Ras family members, which corresponded to a stron

Iliofemoral stenting for venous occlusive disease.

BACKGROUND: Venous hypertension is a significant cause of patient morbidity and decreased quality of life. Common etiologies of venous hypertension include deep venous thrombosis (DVT) or congenital abnormalities resulting in chronic outflow obstruction. We have implemented an aggressive endovascular approach for the treatment of iliac venous occlusion with angioplasty and stenting. The purpose of this study was to determine the patency rates with this approach at a large tertiary care center. MATERIALS/METHODS: All patients undergoing iliofemoral venous angioplasty and stenting over a 4-year

Factors influencing neurologic outcome after neonatal cardiopulmonary bypass: what we can and cannot control.

Advances in cardiopulmonary bypass and surgical techniques have led to progress in the early repair of congenital heart defects in children. However, as increasing numbers survive their initial cardiac operation, an awareness is emerging that significant early and late neurologic morbidities continue to complicate otherwise successful operative repairs. Adverse neurologic outcomes after neonatal cardiac surgery are multifactorial and relate to both fixed and modifiable mechanisms. The purpose of this review is to (1) review mechanisms of brain injury after neonatal cardiopulmonary bypass, (2)
