Hearing results after ossiculoplasty using Polycel prosthesis.

CONCLUSIONS: Polycel is an effective material to use in ossiculoplasty. Good prognostic factors for hearing improvement after ossiculoplasty were healthy middle ear mucosa and the presence of stapes superstructure. OBJECTIVE: During the last decade, the surgical use of alloplasts has become increasingly widespread among otologists. This study aimed to evaluate the hearing results after ossiculoplasty using Polycel prosthesis. MATERIALS AND METHODS: We retrospectively reviewed 188 patients who underwent ossicular chain reconstruction using Polycel prosthesis and were followed up postoperative

Histopathology of the vestibular end organs after intratympanic gentamicin failure for Meniere's disease.

CONCLUSION: To our knowledge, this is the first report of the histopathology of the vestibular end organs following intratympanic gentamicin for intractable Meniere's disease. There was relative sparing of the utricular macula, compared with the cristae ampullares. However, the utricular macula exhibited severe hair cell loss. Clinically, the patient has been free from vertigo spells for 3 years following labyrinthectomy. OBJECTIVE: To describe the histopathology and morphometry of the vestibular end organs from a 59-year-old Meniere's patient who underwent transmastoid labyrinthectomy for re

Chronic suppurative otitis media and related complications at the University Clinic of Kinshasa.

During the five-year period from 1998 to 2002, the ENT department of the University Clinic of Kinshasa treated 343 patients suffering from chronic suppurative otitis media (CSOM) and complications. Two hundred and seven patients had simple, 28 had cholesteatomatous form of CSOM, and 108 patients presented with intratemporal or intracranial complications. Roughly half of the patient group consisted of children younger than 10 years. The simple form of CSOM affects men and women equally, but the cholesteatomatous form was more prevalent among men. Otorrhoea and hearing loss were the major presen

Audiometric results of bilateral bone-anchored hearing aid application in patients with bilateral congenital aural atresia.

The effect of bilateral application of bone-anchored hearing aids (BAHAs) was examined in terms of directional hearing and speech recognition in quiet and in noise in four patients with bilateral congenital atresia who, out of pure necessity, had been using a unilateral bone-conduction hearing aid since early life. This study comprised a prospective clinical evaluation in a single subject design; four patients with bilateral congenital atresia originating from the Nijmegen BAHA series participated. Three patients had Treacher Collins syndrome. All four patients had conductive, most probably, s

Drill-induced hearing loss in the nonoperated ear.

The reversible hearing loss in the nonoperated ear noted by patients after ear surgery remains unexplained. This study proposes that this hearing loss is caused by drill noise conducted to the nonoperated ear by vibrations of the intact skull. This noise exposure results in dysfunction of the outer hair cells, which may produce a temporary hearing loss. Estimations of outer hair cell function in the nonoperated ear were made by recording the change in amplitude of the distortion-product otoacoustic emissions before and during ear surgery. Reversible drill-related outer hair cell dysfunction wa

Autologous peripheral blood stem cell transplantation in a patient with previous invasive middle ear mucormycosis.

A 52-year-old male diagnosed with acute myeloid leukemia (AML) developed an invasive middle-ear mucormycosis during the neutropenic period after consolidation chemotherapy which resolved successfully with surgery and antifungal therapy. The patient underwent autologous peripheral blood stem cell transplantation (APBSCT) in first complete remission with antifungal prophylaxis with liposomal amphotericin B (AmB). There was no clinical, radiological or microbiological data of mycotic reactivation. This is the first reported stem cell transplantation (SCT) in a patient with prior invasive mucormyc

Protons versus photons for the treatment of chordoma.

BACKGROUND: Chordoma is a rare primary bone tumour with a high propensity for local recurrence. Surgical resection is the mainstay of treatment, but complete resection is often morbid due to tumour location. Similarly, the dose of radiotherapy (RT) that surrounding healthy organs can tolerate is frequently below that required to provide effective tumour control. Therefore, clinicians have investigated different radiation delivery techniques, often in combination with surgery, aimed to improve the therapeutic ratio. OBJECTIVES: To assess the effects and toxicity of proton and photon adjuvant r

A 27-year-old man with recurrent sinopulmonary and cutaneous infections.

The increasing availability of genetic testing for modern immunologists in the evaluation of immune diseases could provide a definite diagnosis in elusive cases. A 27-year-old white male patient presented to the clinic with recurrent sinopulmonary and cutaneous infections since childhood. The patient's mother had seronegative polyarthritis, and one of two sisters of the patient had chronic sinopulmonary infections. Serum immunoglobulins, immunoglobulin G (IgG) subclasses, lymphocyte subset markers, mannose-binding lectin, mitogen and antigen stimulation, bacteriophage study, and Streptococcus

The validity of ear prediction guidelines used in facial approximation.

This study examined eight previously published ear prediction methods by Welcker, Gerasimov, Fedosyutkin and Nainys, and Broadbent and Mathews. Computed tomography scans of 78 living adults (n(1)) did not support any of these previously published recommendations. Free earlobes were found to accompany protruding supramastoid crests (Pearson's χ² < 0.05); and ear length [l] and width [w] differed by sex (p < 0.05), correlated with age (r = 0.38[l]; 0.32[w]), and correlated with facial height (r = 0.37[l]; 0.30[w]). New regression equations (for ear length and width) were generated using these va

Delayed otogenic hydrocephalus after acute otitis media in pediatric patients: the changing presentation of a serious otologic complication.

CONCLUSIONS: The clinical presentation of otogenic dural sinus thrombosis (DST) as a complication of acute otitis media (AOM) can be masked by antibiotic treatment. Morning episodes of vomiting and/or headache, visual impairment and a history of AOM seem to be indicative of otogenic hydrocephalus. We therefore advocate that the MRI scans of patients with similar symptoms should be carefully studied to facilitate the early diagnosis of a potentially life-threatening complication. OBJECTIVE: To describe the frequency, pathognomonic signs, clinical course and outcome of otogenic hydrocephalus an

Usefulness of heavily T2-weighted magnetic resonance imaging in patients with cerebellopontine angle tumors.

OBJECTIVE: Heavily T2-weighted magnetic resonance imaging (MRI) has been reported to be useful for the diagnosis of lesions of the inner ear or its central connections. We evaluated the usefulness of heavily T2-weighted MRI in 18 patients with cerebellopontine angle tumors. METHODS: The lesions were acoustic neuromas in 14 patients and meningiomas in 4 patients. The findings of heavily T2-weighted MRI were compared with those of three-dimensional T1-weighted gradient field echo MRI. RESULTS: An accurate description of the situation of the tumor in the internal auditory canal (IAC) was possib

Topical antiseptics for chronic suppurative otitis media.

BACKGROUND: Chronic suppurative otitis media (CSOM), sometimes referred to as chronic otitis media (COM), is a chronic inflammation and often polymicrobial infection of the middle ear and mastoid cavity, characterised by ear discharge (otorrhoea) through a perforated tympanic membrane. The predominant symptoms of CSOM are ear discharge and hearing loss. Topical antiseptics, a possible treatment for CSOM, inhibit the micro-organisms that may be responsible for the infection. Antiseptics can be used alone or in addition to other treatments for CSOM, such as antibiotics or ear cleaning (aural toi
