Thrombotic thrombocytopenic purpura (TTP) is a classic, but not a common disorder of hematology. Plasma exchange (PE) was shown to nearly reverse its 90% mortality rate. However, there are still some fatal outcomes in this dramatic disease. We present our experience of plasma exchange plus corticosteroids for the treatment of TTP in our hospital. Patients with TTP diagnosed between January 1996 and January 2005 were identified by a retrospective review of records of the Uludag University Hospital, Bursa (the largest referral center for adults with this disorder in this region with an estimated

BACKGROUND: Autosomal dominant pathogenic variants in PLCG2 are associated with autoinflammation and PLCG2-associated antibody deficiency and immune dysregulation (APLAID), characterized by immunological abnormalities and multi-system inflammation. Descriptions of associated neurological manifestations and vascular anomalies are limited. We describe three patients with APLAID, vasculopathies, neuroinflammation, and cerebral vascular tortuosity. These cases highlight underrecognized and clinically important manifestations of APLAID.
CASE PRESENTATIONS: All three patients had de novo variants i

BACKGROUND AND OBJECTIVES: Unrecognized seizures occur in up to 77% of individuals with epilepsy, leading to delays in diagnosis with significant consequences including injury, mortality, impaired psychosocial functioning, and negative socioeconomic impacts. Falls are a common reason to present to the emergency department (ED) and may be a sentinel sign of an unrecognized seizure. Whether falls represent missed opportunities for earlier seizure recognition remains unknown. We therefore evaluated the temporal association between ED visits for falls and first-time seizure.
METHODS: We conducted

RATIONALE: Hormones such as estrogen and progesterone influence seizure thresholds resulting in catamenial epilepsy in a subset of women with epilepsy. While prior studies assessing the correlation between menarche and seizure onset have found mixed results, little is known about the effects of puberty on seizure burden in patients with developmental and epileptic encephalopathies, such as Lennox-Gastaut syndrome (LGS). Determining whether seizure burden in LGS is affected by pubertal changes such as menarche will inform prognosis and anticipatory guidance for caregivers of patients.

Epilepsy is a severe neurological disorder with complex pathogenesis. Mitochondrial dysfunction (MitD) is increasingly recognized as a key driver of epileptogenesis and seizure generation, contributing to neuronal hyperexcitability and network instability. However, the potential mechanisms linking MitD to epilepsy remain incompletely understood. This study aimed to identify MitD-related biomarkers in epilepsy and investigate their associations with alterations in the immune cell infiltration landscape during epileptogenesis. On the basis of the GSE47752 dataset, differential expression analysi

OBJECTIVE: The EEG (electroencephalogram) plays a crucial role in prognosticating outcomes for comatose patients after cardiac arrest (CA), but the precise probability of poor outcome (PPO) associated with specific EEG patterns and how it changes over time is not well understood. We aimed to quantify the PPO for individual EEG patterns, assess its precision, and evaluate time-dependent changes after CA.
METHODS: We retrospectively analyzed continuous EEGs from comatose adults treated at three Boston hospitals for CA (2010-2023). We evaluated the prognostic significance of 22 ACNS-standard pat

UNLABELLED: Polyhydramnios, Megalencephaly, and Symptomatic Epilepsy syndrome (PMSE/STRADA-related disorder) is a rare neurodevelopmental disorder characterized by megalencephaly (ME), early-onset drug-resistant epilepsy, neurocognitive impairment, and high early mortality, often due to status epilepticus. PMSE is caused by a multi-exon deletion in STRADA , encoding STRADA, which regulates the mechanistic target of rapamycin (mTOR) pathway. GABAergic inhibitory interneurons (INs) critically modulate the excitatory:inhibitory balance in cortical and hippocampal networks, and IN deficits contrib

RATIONALE: Mitochondrial encephalomyopathy with lactic acidemia and stroke-like episodes (MELAS) syndrome is a maternally inherited mitochondrial disorder caused by mutations in mitochondrial DNA, most commonly the m.3243A>G variant. This mutation impairs oxidative phosphorylation, leading to inadequate cellular energy production, particularly in high-demand tissues such as the brain and muscles. The resultant energy deficit manifests as neurological and muscular dysfunction, including stroke-like episodes, seizures, and lactic acidosis.
PATIENT CONCERNS: Twin brothers presented with heteroge

Acute symptomatic seizures (ASS) represent one of the most frequent and clinically significant neurological complications following pediatric liver and kidney transplantation. Despite their clinical relevance, available evidence remains fragmented; pediatric-specific data are limited, and no standardized diagnostic or management framework has been established. To summarize current evidence regarding the epidemiology, etiological mechanisms, diagnostic evaluation, risk factors, and management strategies of acute symptomatic seizures after pediatric liver and kidney transplantation, and to propo

BACKGROUND AND OBJECTIVES: Developmental and epileptic encephalopathies (DEEs) with early burst-suppression EEG (EIDEE-BS) are among the most severe neonatal epileptic syndromes, typically presenting in the first months of life with refractory seizures and profound neurodevelopmental impairment. Although variants in the KCNQ2, STXBP1, and SCN2A genes are recognized as major causes, the full genetic spectrum remains uncertain. We aimed to delineate the electroclinical characteristics, genetic etiologies, and long-term outcomes in a large MRI-negative EIDEE-BS cohort.
METHODS: We retrospectivel

Ventriculitis commonly arises as a complication of various central nervous system conditions. The causes of ventriculitis include both iatrogenic conditions, such as catheter-related infections, and non-iatrogenic conditions, such as community-acquired bacterial meningitis. The incidence of pyogenic ventriculitis associated with community-acquired bacterial meningitis remains unclear. Additionally, the optimal treatment strategy for pyogenic ventriculitis secondary to community-acquired bacterial meningitis remains uncertain. RGS14 is a complex multifunctional scaffolding protein that is highly enriched within pyramidal cells (PCs) of hippocampal area CA2. There, RGS14 suppresses glutamate-induced calcium influx and related G protein and ERK signaling in dendritic spines to restrain postsynaptic signaling and plasticity. Previous findings show that, unlike PCs of hippocampal areas CA1 and CA3, CA2 PCs are resistant to a number of neurological insults, including degeneration caused by temporal lobe epilepsy (TLE). While RGS14 is protective against peripheral injury, similar roles for RGS14 during pathological injury
