Complications of gold weight eyelid implants for treatment of fifth and seventh nerve paralysis.

Complications occurred in six patients after gold weights were implanted into the upper eyelid tissues for fifth and seventh nerve palsies. These complications included implant infection without extrusion (in one patient); entropion with trichiasis and presumed inflammatory reaction to the gold weight material (in one patient); upper eyelid distortion and poor eyelid contour with corneal ulceration and scarring (in one patient); significant residual lagophthalmos with exposure keratitis (in one patient); and blepharoptosis obscuring the pupillary access (in two patients). Extended Superolateral Periosteal Flap for Single-Stage Total Lower Eyelid Reconstruction.

Total lower eyelid reconstruction requires restoration of both lamellae and prevention of postoperative malposition. Traditional two-stage approaches often require a prolonged period of monocular vision. This report describes an extended superolateral periosteal flap harvested along the superior orbital rim to within 2-3 mm of the supraorbital notch, achieving a 35-mm length for single-stage total lower eyelid reconstruction. A 71-year-old woman with basosquamous carcinoma involving the entire lower eyelid underwent excision with 5-mm margins, creating a 30 × 35 mm defect. The periosteal flap

Postoperative complications in preseptal versus retroseptal transconjunctival approaches: A systematic review and meta-analysis.

Surgical correction of orbital fractures demands precise anatomical dissection with minimal complications. Transconjunctival incisions (TI) offer superior cosmetic outcomes, with two main techniques-preseptal (PS) and retroseptal (RS)-differing in anatomical pathways and associated risks. This systematic review and meta-analysis aimed to compare postoperative complications between PS and RS approaches. A comprehensive search of PubMed, Scopus, Ovid SP, Embase, Cochrane Library, and grey literature up to December 31, 2024, identified relevant studies. Inclusion criteria followed PICOT: adult pa

Evaluation of moderate and severe blepharoptosis correction using the interdigitated part of the frontalis muscle and orbicularis oculi muscle suspension technique: A cohort study of 235 cases.

BACKGROUND: Blepharoptosis is defined as an abnormally low-lying upper eyelid margin in the primary gaze, causing vertical narrowing of the palpebral fissure. It is difficult to achieve consistently satisfactory results in moderate or severe cases despite many surgical methods being available to correct them. METHODS: Between January 2001 and December 2014, a retrospective cohort study was conducted using medical records and perioperative photographs of 235 patients. All the patients, having presented with moderate or severe bilateral or unilateral blepharoptosis, underwent blepharoptosis cor

The zygomaticomaxillary complex fracture - an anthropometric appraisal of surgical outcomes.

Before undergoing repair of zygomaticomaxillary complex fractures, most patients are worried about their postoperative appearance. Furthermore, there is an ongoing discussion about the selection of the surgical approach to the inferior orbita and resulting eyelid deformities. We present a photo-assisted postoperative evaluation of zygomaticomaxillary complex fracture repair based on reference anthropometric data. Two hundred and twenty-one patients underwent zygomaticomaxillary complex fracture repair. An analysis of standardized postoperative photographs included measurements of eye fissure w

Inadequate inferior cul-de-sac in the anophthalmic socket.

A retrospective review of preoperative findings and postoperative results of conjunctival cul-de-sac fixation surgery performed on 12 patients with acquired anophthalmos and an inadequate inferior conjunctival cul-de-sac was performed. Patients with inadequate conjunctiva or other subconjunctival cicatricial shortening of the inferior cul-de-sac were not suitable candidates for conjunctival cul-de-sac fixation surgery and therefore were excluded from this retrospective study. Only those patients with adequate conjunctiva and loss of inferior cul-de-sac fixation to the lower eyelid retractor co

Lid margin split in the surgical correction of epiblepharon.

PURPOSE: To describe the technique of splitting the lid margin combined with the excision of redundant skin and muscle during the surgical correction of epiblepharon and to report its clinical outcome. METHODS: A combined procedure that included splitting the lid margin to repair lower eyelid epiblepharon was performed on 31 eyes of 19 consecutive patients. Lid margin splitting was performed along the grey line on the medial third or half of the lower eyelid by making a 1 mm-deep incision. Having made a transverse subciliary skin incision and a dissection between the tarsus and the orbiculari

Comprehensive management of orbital fractures.

Orbital fractures are some of the more challenging injuries faced by the plastic surgeon. As such a prominent facial feature, even the most minor asymmetries following trauma can be distressing to the patient. In treating these patients, there are certain crucial aspects of both diagnosis and management that are critical to obtaining an optimal result. These include a careful preparative eye examination focusing on extraocular motility and any evidence of optic nerve compression. Candidates for surgery must be carefully selected based on firm indications such as a large orbital floor defect (>

A girl with Hermansky-Pudlak syndrome.

A young girl with ocular albinism and the Hermansky-Pudlak syndrome is described. Ocular albinism generally occurs in males. In this condition, the pigmentation of the skin and hair is nearly normal, and the melanin pigment abnormality is limited to the eyeballs. The chief complaints are visual disturbance, nystagmus, and photophobia. A 3-year-old girl was recently brought to our hospital with nystagmus, which she had exhibited since the age of 1 year. Funduscopy resulted in a diagnosis of ocular albinism. Further investigations, specifically, microscopy of her platelets, led us to conclude th
