BACKGROUND: Throughout the literature, patellofemoral instability (PI) is defined as an increased risk of re-/luxation of the patella within the patellofemoral joint (PFJ). In most patients it is caused by traumatic patella luxation or the existence of a range of predisposing anatomic risk factors leading to an unphysiological movement sequence within the PFJ also known as patellofemoral maltracking. In order to provide an individualized therapy approach, clinical and radiological evaluation of those risk factors of variable magnitude becomes essential. Diagnostic imaging such as magnetic reso

Osteoarthritis of the knee is associated with deformities of the lower limb. Tibia valga is a contributing factor to lower limb alignment in valgus knees. We evaluated 97 valgus knees and 100 varus knees. Long-leg films were taken in weight bearing with both knees in full extension. For valgus knees, 52 knees (53%) had a tibia valga deformity. Average tibia valgus deformation was 5.0°. For varus knees, there was only 1 case of tibia valga (1%), with a deformation of 2.5°. The aim of this study was to assess the prevalence of primary tibia valga in valgus and varus knees and understand how it a

INTRODUCTION: Magnetic intramedullary nails are used for deformity correction and limb lengthening because of their minimally invasive nature and reduced complication rates compared to external fixators. However, complications such as non-union and nail breakage remain rare but significant clinical challenges, particularly in cases with patient-specific risk factors. This case demonstrates a successful union after nail fracture without the removal of the intramedullary nail, using lateral plate fixation, offering a salvage option in this uncommon scenario.

INTRODUCTION: Recurrent patellar dislocation presents a significant rehabilitation challenge, particularly in athletes engaged in high-demand, multi-directional sports such as badminton. This case report describes a complex presentation in a state-level athlete with multiple patellar dislocations, followed by medial patellofemoral ligament (MPFL) reconstruction and a subsequent comminuted patellar fracture. The case was further complicated by proprioceptive deficits, extension lag, sensory changes, and kinesiophobia, all of which limited the athlete's functional performance and return to sport

Varus-producing distal femoral osteotomy (DFO) is an established procedure to correct genu valgum in patients with osteoarthritis and gait abnormalities. DFO has also been proposed for the treatment of patellar instability in patients with valgus alignment. However, it is not known how DFO affects parameters associated with patellar instability. This study evaluated radiographic changes after DFO with variable degrees of correction on six cadaveric knees, with the hypothesis that improving mechanical alignment would improve radiographic parameters related to patellar instability. Controlled la

OBJECTIVE: The aims of this study were (1) to investigate the tibial slope (TS) changes following guided growth by the eight-plate and to determine the relationship between the TS change in the sagittal plane and the plate position.
METHODS: In this retrospective study, children who underwent guided growth by the eight-plate for genu varum or valgum were included. Thirty-eight extremities of 23 children (14 girls and 9 boys; mean age = 9.2 years, age range = 3-17 years) were then divided into two groups according to the plate location in the sagittal plane (anterior or midline). Preoperative

CONTEXT: Distal femoral osteotomy (DFO) is a joint preservation procedure that corrects genu valgum deformities and patellofemoral maltracking, thereby restoring kinematics and unloading contact pressures in the lateral tibiofemoral and patellofemoral compartments.
OBJECTIVE: To evaluate the rates of return to work (RTW) and return to sport (RTS) after DFO for valgus malalignment and lateral compartment osteoarthritis through a systematic review of the literature.
DATA SOURCES: A systematic review using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guideline

BACKGROUND: Bilateral femur fractures are rare in the pediatric population with few cases reported in the literature. The purpose of this study was to review our institutional experience with a case series of simultaneous bilateral femur fractures to highlight the presentation, treatment, and outcomes of these rare injuries as well as perform a preliminary comparison to similar unilateral femur fractures in order to identify any clinically relevant differences that may guide future management.
METHODS: We undertook a retrospective chart review of patients who had presented with simultaneous b

OBJECTIVE: The aim of this study was to evaluate the results of fixator assisted correction of the distal femoral valgus deformities and the precision of the correction.
METHODS: Seventeen extremities of 13 patients (7 women and 6 men; mean age: 16 ± 5.4 years) who had fixator assisted plating of the distal femur for genu valgum deformity were evaluated. Mechanical axis deviation (MAD) and mechanical lateral distal femoral angles (mLDFA) were measured pre-operatively and post-operatively. mLDFA was graded as perfect if it is between 85° and 90°(85° ≤ x ≤ 90°); overcorrection if it is between

Mucopolysaccharidoses (MPS) are a group of diseases characterized by abnormal accumulation of glycosaminoglycans (GAGs). Although there are differences among the various disease types, the osteoarticular system is always involved. The aim of the present study was to establish a framework for MPS-related orthopaedic manifestations and for their treatment. The authors, affiliated to three different Italian Orthopaedic Centres, report data taken from the literature reviewed in light of their accumulated professional experience. Bone alterations make up what is known as dysostosis multiplex, invol

PURPOSE: Mucopolysaccharidoses (MPS) are a group of rare lysosomal storage disorders associated with involvement of multiple organs along with a generalised skeletal dysplasia. Both haematopoetic stem cell transplant and enzyme replacement therapy have improved the outlook for patients while surgery remains high-risk and there is little information on clinical or functional outcome to justify many of the surgical procedures performed. This paper aims to summarise the orthopaedic surgical procedures in MPS patients for which quality of life (QoL) and functional data are available and to describ

BACKGROUND: Anterior cruciate ligament (ACL) tears are becoming more common in the skeletally immature population as participation in high-risk sports continues to grow. This presents a challenge for the treating surgeon as ACL reconstruction in this patient set has the added aim of preservation of the growth plate anatomy. The purpose of this investigation is to report on 4 patients who developed growth arrest following ACL reconstruction and offer a review of the available literature.
METHODS: Four skeletally immature patients (2 male and 2 female) were identified who underwent ACL reconstr

BACKGROUND: A relationship has been reported between total body irradiation (TBI) and later development of osteochondromas in children who receive radiation therapy as conditioning before hematopoietic stem cell transplantation (HSCT). The goal of this study was to better characterize osteochondromas occurring in these children.
METHODS: We identified all children (0 to 18 y) who received an allogeneic HSCT and TBI from 2000 to 2012 from a blood and marrow transplant (BMT) database. Thereafter, we identified those who developed osteochondromas through a chart review. In addition, we searched

PURPOSE: Morquio-Brailsford syndrome (MS) is an autosomal recessive lysosomal storage disorder, a mucopolysaccharidosis, characterized by abnormal metabolism of glycosaminoglycans. Major treatable concerns in patients with MS involve C1 to C2 instability, genu valgum, and hip subluxation. Untreated hip subluxation has been shown to predispose to early onset of arthritis of the hip. Early appropriate pelvic osteotomies may restore (improve) load transmission and retard the onset of arthritis. Computed tomographic (CT) measurements can help determine the site and severity of acetabular deficienc

BACKGROUND: Skeletal deformities (SD) in children and adolescents can lead to arthritic conditions, impairment of quality of life, and high treatment costs in the long term. However, comprehensive data on the prevalence of SDs in children and adolescents are limited and it remains therefore unclear whether there is a healthcare gap. "OrthoKids" is a project that addresses this evidence gap by implementing an orthopaedic screening for children and adolescents that supplements existing detection examinations within statutory standard care in Germany.
OBJECTIVE: To detect SDs so that they can be

There is a high rate of lower limb deformity and limb length discrepancy in patients with hereditary multiple exostoses (HME). The aim of this study was to evaluate the type and frequency of lower limbs axial deviation and limb length discrepancy and the type of exostoses being risk factors for theses deformities. We retrospectively reviewed standing full-length radiograph of 32 HME patients (64 limbs) followed in our institution between October 2009 and December 2020. Patient demographics were recorded. Radiographic analysis of the coronal limb alignment was performed, limb length discrepancy
