Estimating malaria antigen dynamics and the time to negativity of next-generation malaria rapid diagnostic tests.

BACKGROUND: Rapid diagnostic tests (RDTs) used to diagnose Plasmodium falciparum predominantly target the antigen Histidine Rich Protein 2 (HRP2) exclusively. With the emergence of hrp2/hrp3 gene deletions, RDTs targeting other antigens such as the essential enzyme Lactate Dehydrogenase (LDH) are needed. The dynamics of LDH relative to HRP2 are currently not well described but are needed to inform the use of next-generation (NG-) LDH and HRP2 RDTs that are designed to address hrp2/hrp3 gene deletions. METHODS: A longitudinal cohort study conducted in a low transmission setting in Namibia was

A descriptive summary of the WHO availability assessments of medical abortion medicines in eight African countries.

BACKGROUND: The use of medical abortion using either a combination of mifepristone and misoprostol, or misoprostol alone has contributed to increased safety and decreased mortality and morbidity. The availability of quality medical abortion medicines is an essential component in the provision of quality abortion care. Understanding the factors that influence the availability of medical abortion medicines is important to help in-country policymakers, program planners, and providers improve availability and use of medical abortion. METHODS: Using a national assessment protocol and an availabili

Rutting vocal display in male impala (Aepyceros melampus) and overlap with alarm context.

BACKGROUND: The rutting vocal display of male impala Aepyceros melampus is unique for its complexity among ruminants. This study investigates bouts of rutting calls produced towards potential mates and rival males by free-ranging male impala in Namibia. In particular, a comparison of male rutting and alarm snorts is conducted, inspired by earlier findings of mate guarding by using alarm snorts in male topi Damaliscus lunatus. RESULTS: Rutting male impala produced 4-38 (13.5 ± 6.5) rutting calls per bout. We analyzed 201 bouts, containing in total 2709 rutting calls of five types: continuous r

Drivers of HIV-1 drug resistance to non-nucleoside reverse-transcriptase inhibitors (NNRTIs) in nine southern African countries: a modelling study.

INTRODUCTION: The rise of HIV-1 drug resistance to non-nucleoside reverse-transcriptase inhibitors (NNRTI) threatens antiretroviral therapy's long-term success (ART). NNRTIs will remain an essential drug for the management of HIV-1 due to safety concerns associated with integrase inhibitors. We fitted a dynamic transmission model to historical data from 2000 to 2018 in nine countries of southern Africa to understand the mechanisms that have shaped the HIV-1 epidemic and the rise of pretreatment NNRTI resistance. METHODS: We included data on HIV-1 prevalence, ART coverage, HIV-related mortalit

Prevalence and correlates of sexual intercourse among sexually active in-school adolescents: an analysis of five sub-Sahara African countries for the adolescent's sexual health policy implications.

BACKGROUND: Early and unprotected sex with multiple partners among adolescents carries a high risk of acquiring HIV infections, other sexually transmitted infections as well as high rates of teenage pregnancy. Sub-Saharan Africa (SSA) has a higher burden of HIV/AIDS: the leading cause of deaths among adolescents. We estimated the prevalence and examined the correlates of sexual intercourse among in-school adolescents of SSA. The purpose is to inform the public health programs dedicated to tackling the burden of HIV/AIDS. METHODS: We did a secondary analysis of the Global School-based Student

Guideline Concordance of Treatment and Outcomes Among Adult Non-Hodgkin Lymphoma Patients in Sub-Saharan Africa: A Multinational, Population-Based Cohort.

BACKGROUND: Although non-Hodgkin lymphoma (NHL) is the 6th most common malignancy in Sub-Saharan Africa (SSA), little is known about its management and outcome. Herein, we examined treatment patterns and survival among NHL patients. METHODS: We obtained a random sample of adult patients diagnosed between 2011 and 2015 from 11 population-based cancer registries in 10 SSA countries. Descriptive statistics for lymphoma-directed therapy (LDT) and degree of concordance with National Comprehensive Cancer Network (NCCN) guidelines were calculated, and survival rates were estimated. FINDINGS: Of 516

Contraception needs and pregnancy termination in sub-Saharan Africa: a multilevel analysis of demographic and health survey data.

BACKGROUND: Women in sub-Saharan Africa (SSA) have a higher risk of unintended pregnancies that are more likely to be terminated, most of which are unsafe with associated complications. Unmet need for contraception is highest in SSA and exceeds the global average. This study investigates the association between unmet/met need for contraception and pregnancy termination SSA. METHODS: We used pooled data from Demographic and Health Surveys conducted from January 2010 to December 2018 in 32 countries in SSA. Our study involved 265,505 women with diverse contraception needs and with complete data

SAGES white paper: strategic principles, operational considerations, and practical recommendations for building sustainable laparoscopic surgical training in low-resource settings: a 10-step guide.

BACKGROUND: Minimally invasive surgery provides clear benefits to patient outcomes and is now standard practice in high-income countries. However, it remains underutilized in low- and middle-income countries (LMICs) due to limited training opportunities, infrastructure, mentorship, and long-term program sustainability. Recognizing these challenges, the Global Affairs Committee of the Society of American Gastrointestinal and Endoscopic Surgeons (SAGES) launched the Global Laparoscopic Advancement Program (GLAP) to build sustainable laparoscopic training capacity. Over the past decade, GLAP has

Modelling impact and cost-effectiveness of oral pre-exposure prophylaxis in 13 low-resource countries.

INTRODUCTION: Oral pre-exposure prophylaxis (PrEP) provision is a priority intervention for high HIV prevalence settings and populations at substantial risk of HIV acquisition. This mathematical modelling analysis estimated the impact, cost and cost-effectiveness of scaling up oral PrEP in 13 countries. METHODS: We projected the impact and cost-effectiveness of oral PrEP between 2018 and 2030 using a combination of the Incidence Patterns Model and the Goals model. We created four PrEP rollout scenarios involving three priority populations-female sex workers (FSWs), serodiscordant couples (SDC

Cannabis and amphetamine use among school-going adolescents in sub-Saharan Africa: a multi-country analysis of prevalence and associated factors.

BACKGROUND: Global evidence indicates that early onset of illicit substance use among adolescents and emerging adults is associated with negative mental-health related-outcomes that can persist into adulthood. However, the lack of quality regional data on adolescent illicit substance use and its determinants remains a common barrier to evidence-based policy-making and the development of school-based interventions in Africa. The purpose of our study was to estimate the prevalence and describe the correlates of cannabis and amphetamine use among school-going adolescents in eight sub-Saharan Afri

Partner alcohol consumption and intimate partner violence against women in sexual unions in sub-Saharan Africa.

INTRODUCTION: Intimate partner violence is increasingly gaining attention as the leading form of violence against women globally, particularly sub-Saharan Africa. Given that substance abuse, especially alcohol consumption has long been associated with aggressive behaviour, emotional abuse, and sexual misconduct, it is surprising that studies on the potential association between partner's alcohol consumption and intimate partner violence are scarce. The current study seeks to fill this gap in the literature by examining the association between partner's alcohol consumption and intimate partner

[Malaria is still a leading cause of fever and death among children and pregnant women in Africa in 2015].

The prevalence and morbidity of P. vivax, P. ovale (curisi et wallikeri) and P. malariae remain underestimated. However important progress has been made. According to the WHO World Malaria report between 2000 and 2015 the malaria incidence has decreased by 42% while the incidence of malaria deaths has decreased by 66%. This is the result of the important progress made in scaling up the main interventions such the rapid diagnosis test, Artemisinin-based combination therapies, long lasting insecticide treated nets, indoor residual house spraying, intermittent preventive treatment during pregnanc

Synergistic effects of seasonal rainfall, parasites and demography on fluctuations in springbok body condition.

1. Seasonality of rainfall can exert a strong influence on animal condition and on host-parasite interactions. The body condition of ruminants fluctuates seasonally in response to changes in energy requirements, foraging patterns and resource availability, and seasonal variation in parasite infections may further alter ruminant body condition. 2. This study disentangles the effects of rainfall and gastrointestinal parasite infections on springbok (Antidorcas marsupialis) body condition and determines how these factors vary among demographic groups. 3. Using data from four years and three study

An imprecise probability approach-based determination of over-represented southern African plant genera and families used in ethnopharmacology.

ETHNOPHARMACOLOGICAL RELEVANCE: The analyses of patterns of over-representation of southern African traditional medicinal plants at the genus and family level provide information about the differences in plant use among southern African countries and disease categories. 'Over-representation' refers to the phenomenon whereby the proportion of plants belonging to a taxonomic group is higher in ethnobotanical pharmacopoeia than in the total flora. AIM OF THE STUDY: This study aimed to use the Imprecise Dirichlet Model (IDM) to analyse data from ten southern African countries to establish how ove

The RHD Action Small Grants Programme: Small Investment, Big Return!

Rheumatic Heart Disease (RHD) remains endemic in low- and middle-income countries (LMICs) despite its virtual elimination in high-income countries. RHD Action was launched to amplify global efforts to control RHD in 2015 by World Heart Federation and Reach, with demonstration projects in Uganda and Tanzania, and support from Medtronic Foundation. The Small Grants Programme focuses on three domains: People and Communities, Medicines and Technologies, and Systems and Services. It is designed to support patient and community groups in promoting awareness, advocacy, and to build health workers' ca

[Breastfeeding in Africa: will positive trends be challenged by the AIDS epidemic?].

In Africa, more than 95% of infants are currently breastfed, but feeding practices are often inadequate: feeding water, and other liquids, to breastfed infants is a widespread practice. Consequently, the rate of exclusive breast-feeding is low, particularly in West Africa. The rate of bottle-feeding is high in some countries (exceeding 30% in Tunisia, Nigeria, Namibia and Sudan). Nevertheless prolonged breastfeeding is common, and the median duration of breastfeeding ranges between 16 and 28 months. Urbanization and mothers' education are the major factors that tend to shorten breastfeeding. N

Determinants of institutional delivery in Sub-Saharan Africa: findings from Demographic and Health Survey (2013-2017) from nine countries.

INTRODUCTION: Institutional delivery is a major concern for a country's long-term growth. Rapid population development, analphabetism, big families, and a wider range of urban-rural health facilities have had a negative impact on institutional services in Sub-Saharan Africa (SSA) countries. The aim of this study was to look into the factors that influence women's decision to use an institutional delivery service in SSA. METHODS: The most recent Demographic and Health Survey (DHS), which was conducted in nine countries (Senegal, Ethiopia, Malawi, Rwanda, Tanzania, Zambia, Namibia, Ghana, the D

Performance of national HIV testing algorithms in 14 Population-based HIV Impact Assessment surveys: accuracy of HIV diagnosis using a two-test strategy, with or without a tie-breaker test, in different prevalence settings, 2015-2022.

UNLABELLED: Population-based HIV Impact Assessment surveys were conducted in 14 countries to measure HIV prevalence, incidence, and viral load suppression in children aged 0-14 years and adults aged ≥15 years. We examined the performance of rapid HIV testing algorithms in multiple countries. Survey participants received an HIV diagnosis based on two serial tests in Cameroon, Côte d'Ivoire, Eswatini, Haiti, Kenya, Lesotho, Malawi, Mozambique, Tanzania, and Zambia. Whereas in Ethiopia, Kenya, Namibia, Uganda, and Zimbabwe, diagnosis was based on a tie-breaker algorithm. Testers received intensiv
