Estimating risks of common complex diseases across genetic and environmental factors: the example of Crohn disease.

BACKGROUND: Progress has been made in identifying mutations that confer susceptibility to complex diseases, with the prospect that these genetic risks might be used in determining individual disease risk. AIM: To use Crohn disease (CD) as a model of a common complex disorder, and to develop methods to estimate disease risks using both genetic and environmental risk factors. METHODS: The calculations used three independent risk factors: CARD15 genotype (conferring a gene dosage effect on risk), smoking (twofold increased risk for smokers), and residual familial risk (estimating the effect of

Crohn's disease risk alleles on the NOD2 locus have been maintained by natural selection on standing variation.

Risk alleles for complex diseases are widely spread throughout human populations. However, little is known about the geographic distribution and frequencies of risk alleles, which may contribute to differences in disease susceptibility and prevalence among populations. Here, we focus on Crohn's disease (CD) as a model for the evolutionary study of complex disease alleles. Recent genome-wide association studies and classical linkage analyses have identified more than 70 susceptible genomic regions for CD in Europeans, but only a few have been confirmed in non-European populations. Our analysis

Intracellular adhesion molecule-1 is regulated by porphyromonas gingivalis through nucleotide binding oligomerization domain-containing proteins 1 and 2 molecules in periodontal fibroblasts.

BACKGROUND: The mechanism by which Porphyromonas gingivalis regulates intracellular adhesion molecule 1 (ICAM-1) expression in human periodontal ligament cells (hPDLCs) and human gingival fibroblasts (hGFs) is unknown. The aim of this study is to investigate whether nucleotide binding oligomerization domain-containing protein (NOD) 1 and NOD2 are involved in this process and the clinical significance of ICAM-1 in periodontitis. METHODS: hPDLCs and hGFs were treated with P. gingivalis, l-Ala-γ-d-glutamyl-mesodiaminopimelic acid (an agonist for NOD1), and muramyl dipeptide (an agonist for NOD2)

Postbiotic Lactobacillus sakei CVL-001 promotes mucosal tolerance via NOD2-dependent programming of tolerogenic CD11c+ antigen-presenting cells.

Postbiotics offer improved safety and stability over live probiotics for modulating intestinal immunity, yet the receptor-mediated mechanisms underlying mucosal tolerance remain unclear. Nucleotide-binding oligomerization domain 2 (NOD2), a cytosolic sensor of bacterial cell wall components and a major genetic risk factor for inflammatory bowel disease (IBD), may link microbial signals to regulatory immune programs. Here, we show that heat-killed Lactobacillus sakei CVL-001 (HK CVL-001) promotes intestinal homeostasis through NOD2 signaling. HK CVL-001, but not its culture supernatant, signifi

Clinical and molecular characteristics of isolated colonic Crohn's disease.

BACKGROUND: Clinical, serological, and molecular data support the existence of discrete subsets of Crohn's disease (CD) defined by location of disease. Little is known about the epidemiology and natural history of isolated CD of the colon (Montreal Classification L2) because most studies have not accurately distinguished it from ileocolonic disease. Our objectives were to describe the clinical features and natural history of isolated colonic CD in a rigorously characterized patient cohort and to investigate the association of polymorphisms in a number of genes with colonic location of disease

Commensal bacteria direct selective cargo sorting to promote symbiosis.

Mucosal immunity protects a host from intestinal inflammation and infection and is profoundly influenced by symbiotic bacteria. Here we report that in mice symbiotic bacteria directed selective cargo sorting in Paneth cells to promote symbiosis through Nod2, a cytosolic bacterial sensor, and the multifunctional protein kinase LRRK2, both encoded by inflammatory bowel disease (IBD)-associated genes. Commensals recruited Nod2 onto lysozyme-containing dense core vesicles (DCVs), which was required for DCV localization of LRRK2 and a small GTPase, Rab2a. Deficiency of Nod2, LRRK2 or Rab2a or deple

Molecular genetics in adult-onset Still's disease: next-generation sequencing in 24 patients and literature review.

OBJECTIVE: Next-generation sequencing (NGS) panels are increasingly used for the diagnosis of monogenic systemic autoinflammatory diseases (SAIDs). However, their role in patients with adult-onset Still's disease (AOSD) remains unknown. This study aims to assess the usefulness of NGS panels in AOSD patients to improve diagnosis and management of the disease. METHODS: This observational, multicenter study included all patients with AOSD diagnosis who underwent NGS panel testing in northern Spain. Clinical manifestations, laboratory parameters, complications, and therapeutic responses were reco

The NOD2 defect in Blau syndrome does not result in excess interleukin-1 activity.

OBJECTIVE: Blau syndrome is a rare, autosomal-dominant, autoinflammatory disorder characterized by granulomatous arthritis, uveitis, and dermatitis. Genetics studies have shown that the disease is caused by single nonsynonymous substitutions in NOD-2, a member of the NOD-like receptor or NACHT-leucine-rich repeat (NLR) family of intracellular proteins. Several NLRs function in the innate immune system as sensors of pathogen components and participate in immune-mediated cellular responses via the caspase 1 inflammasome. Mutations in a gene related to NOD-2, NLRP3, are responsible for excess cas

Goblet Cell Loss Linked to NOD2 and Secondary Resection in Crohn's Disease Is Induced by Dysbiosis and Epithelial MyD88.

BACKGROUND & AIMS: The role of goblet cells in small intestinal inflammation in Crohn's disease (CD) is unknown. Polymorphisms of NOD2 confer risk for CD and associate with small intestinal disease location. We previously showed in mice that Nod2 deficiency leads to overexpansion of Phocaeicola vulgatus in the gut and downstream goblet cell defects, which preceded small intestinal inflammation. In this study, we ask whether goblet cell defects occur in patients with CD with NOD2 polymorphisms and investigate in mice how Pvulgatus signals through the intestinal epithelium. METHODS: We performe

TLR2/TLR4-dependent exaggerated cytokine production in hyperimmunoglobulinaemia D and periodic fever syndrome.

OBJECTIVE: The autoinflammatory hyperimmunoglobulinaemia D and periodic fever syndrome (HIDS) is characterized by recurrent episodes of fever and inflammation. As part of the mevalonate kinase deficiency spectrum, it is caused by MVK mutations, resulting in decreased mevalonate kinase activity in the isoprenoid pathway. Although IL-1β is considered a major cytokine in its pathogenesis, IL-1 blockade is not successful in a proportion of patients. We aimed to further characterize the pro-inflammatory cytokine profile of HIDS. METHODS: Peripheral blood mononuclear cells from HIDS patients and he

A distinct pattern of disease-associated single nucleotide polymorphisms in IBD risk genes in a family with Crohn's disease.

Recent studies have identified more than 160 inflammatory bowel disease susceptibility loci and provided evidence for genetic heritability in disease pathogenesis. Here we describe a case of a 47-year-old White woman suffering from Crohn's disease (CD), who had four children, two with CD and two with a factor V Leiden variation. We analysed the presence of single nucleotide polymorphisms in several CD susceptibility genes. SNP analysis was carried out using commercially available assays. The female CD patient had a positive inflammatory bowel disease family history. All of the patients had a m

Bacterial sensor Nod2 prevents inflammation of the small intestine by restricting the expansion of the commensal Bacteroides vulgatus.

Nod2 has been extensively characterized as a bacterial sensor that induces an antimicrobial and inflammatory gene expression program. Therefore, it is unclear why Nod2 mutations that disrupt bacterial recognition are paradoxically among the highest risk factors for Crohn's disease, which involves an exaggerated immune response directed at intestinal bacteria. Here, we identified several abnormalities in the small-intestinal epithelium of Nod2(-/-) mice including inflammatory gene expression and goblet cell dysfunction, which were associated with excess interferon-γ production by intraepithelia

High post surgical opioid requirements in Crohn's disease are not due to a general change in pain sensitivity.

Crohn's disease (CD) is a painful inflammatory bowel disease with complex multigenic inheritance. Suggested on the basis of a few isolated reports CD patients require significantly higher post operative opioid doses than patients undergoing comparable severe abdominal surgery. Crohn's disease therefore may be a suitable model for the identification of novel pain susceptibility genes. In order to confirm this observation and to elucidate the underlying molecular mechanisms, we investigated if higher opioid needs of CD patients are due to a general change in pain sensitivity. Quantitative sensor

No impact of NOD2/CARD15 on outcome after SCT.

Recent studies have pointed towards an association between certain single nucleotide polymorphisms (SNPs) in the NOD2/CARD15 gene, and negative outcome of Allo-SCT. In this study, 198 patients and their corresponding donors were analyzed retrospectively for the occurrence of NOD2/CARD15 mutations to evaluate the impact on clinical results after Allo-SCT. In all, 7.6% of the patients and 11% of the donors were heterozygous for one of three SNPs 8, 12 or 13. Contrary to earlier findings, we found no significant impact on incidence of acute GVHD or TRM following Allo-SCT. These differences in res

Immunostimulatory effects of Heyndrickxia coagulans SANK70258.

Here, we examined the immunomodulating effects of Heyndrickxia coagulans SANK70258 (SANK70258). Mouse splenocytes treated with γ-ray-irradiated SANK70258 produced higher levels of interferon (IFN)-γ than those with 7 types of lactic acid bacteria. IFN-γ was mainly produced by natural killer (NK) cells, involving IL-12/IL-23, dendritic cells (DCs), and NF-κB signaling. SANK70258 induced the release of IL-6, IL-10, and IL-12p40 from mouse DCs and the expression of cytokine genes in the human monocyte. Cytokine release from SANK70258-treated DCs was partially reduced by the knockdown of Tlr2 or N
