Patients with oral tumors. Part 2: Quality of life after treatment with resection prostheses. Resection prosthetics: evaluation of quality of life.

In the present study, the oral health-related quality of life of 18 patients (13 men and 5 women) was evaluated using validated questionnaires as proposed by the European Organization of Research and Treatment of Cancer (EORTC). The patients belonged to a cohort of 48 patients, whose prosthetic treatment was performed during the years 2004-2007. In the course of tumor resection, 12 patients underwent graft surgery and 14 patients radiotherapy. One patient required a nasal epithesis since resection of the nose became necessary. Five patients underwent a full block resection of the mandible, and

Prosthetic rehabilitation of an extensive midfacial and palatal postsurgical defect with an implant-supported cross arch framework: a clinical report.

When aggressive cancer is surgically treated, the extent of the lesion dictates the amount of tissue excised. The removal of extensive amounts of tissue, including crucial landmarks, complicates prosthesis design. This clinical report details the rehabilitation of a patient who had a total maxillectomy coupled with extensive surgical removal of the midface. With the aid of computer-generated implant placement, a large computer-aided design and computer-aided manufacturing milled titanium framework, a Hader bar, and magnetic keepers, a successful implant retained definitive obturator and facial

An implant-retained maxillary overdenture to obturate a patent oronasal communication: a case report.

This is a case report of dental prosthetic treatment of a 72-year-old patient with cleft palate with a persisting oral nasal communication in the anterior hard palate. The remaining teeth were extracted, and dental implants were surgically placed in the maxilla and mandible to retain removable overdentures. The maxillary complete overdenture had full palatal coverage as opposed to partial coverage. It was retained by 5 implants with low-profile light retentive retainers. The mandibular overdenture was retained with 4 implants of different diameters and brands. Apparent parameters for successfu

Relationship between BMI and Postoperative Complications with Free Flap in Anterolateral Craniofacial Reconstruction.

BACKGROUND: Although we have seen tremendous advancement in microsurgery over the last 2 decades and free tissue transfer has become standard for head and neck reconstruction, surgeons still struggle to prevent postoperative complications. We examined the relationship between body mass index (BMI) and postoperative complications in patients undergoing rectus abdominis free flap transfer after anterolateral craniofacial resection. METHODS: This was a retrospective review of reconstructive surgery using rectus abdominis musculocutaneous free flap in patients with locally advanced maxillary sinu

Subepithelial connective tissue graft associated with apicoectomy and root-end fillings in the treatment of deep localized gingival recession with apex root exposure: case report.

Periodontal reconstructive surgery procedures seek to correct mucogingival defects, including gingival recession. This case report describes the use of a subepithelial connective tissue graft (SCTG) associated with root-end fillings using mineral trioxide aggregate (MTA) for the treatment of Miller Class II recession with root apex exposure. A partial-thickness double pedicle flap was made, followed by root preparation with curette and bur finishing. The exposed root apex was removed and the canal was filled with MTA. An SCTG taken from the palate was placed over the root surface and covered w

Immediate obturator stabilization using mini dental implants.

Edentulous patients with maxillary defects face a more challenging oral rehabilitation process than dentate patients. With the use of mini dental implants (MDIs), it is now possible to immediately increase obturator retention and stability. Implant patients can have a retentive obturator that enhances the overall efficacy of the prosthesis both in comfort and function.

Odontogenic myxoma--a case series and review of the surgical management.

BACKGROUND: Odontogenic myxomas (OMs) are considered to be a benign odontogenic tumour with locally aggressive behaviour. Since these neoplasms are rare in the oral cavity, the possible surgical management can be quite variable. Literature recommendation can vary from simple curettage and peripheral ostectomy up to segmental resection. CASE REPORTS: In the present article, a series of three new cases are presented and discussed. The first case is a 43-year-old male presented with a relatively large OM in the left maxillary area. The second case is a 37-year-old female presented with a relativ

Zygoma implant-supported prosthetic rehabilitation after partial maxillectomy using surgical navigation: a clinical report.

The rehabilitation of patients with acquired defects of the maxilla is a challenge in terms of reestablishing oronasal separation. In most patients these goals are met by means of prosthetic rehabilitation with an obturator prosthesis. If the remaining dentition does not offer sufficient retention and support, the placement of zygoma implants can enhance the stability of the prosthesis. Due to the anatomic intricacies of the zygomatic bone and the implant length, computer-supported navigated implant placement can be advantageous. In the following clinical report, a diabetic patient with a stat

Fabrication of an immediate surgical obturator for a patient with resection of the maxillary antrum and bony orbital floor.

An immediate surgical obturator is necessary for maxillectomy procedures to minimize functional disabilities in speech, swallowing, and egress of food and liquid into the surgical defect. Sometimes the extent of the tumor may be such that it may even require the removal of the maxillary sinus along with resection of the orbital floor. The resected orbital floor can be surgically reconstructed with autogenous soft and/or hard tissues or with alloplastic materials. This clinical report highlights one such situation where the surgical resection of the maxilla and the orbital floor were rehabilita

Potential risk of serious oral infections in the diabetic patient: a clinical report.

Diabetes has many manifestations, including increased vulnerability to bacterial and fungal infections, that may be of concern to the dental profession. Oral candidosis and other opportunistic fungal infections may be early, nonspecific signs of uncontrolled diabetes. Rhinocerebral involvement often extends to the paranasal sinuses, pharynx, palate, orbit, and brain, causing severe tissue destruction and necrosis. This clinical report describes a young woman with diabetes who developed mucormycosis of the maxilla. Tissue destruction resulted in loss of the entire premaxilla, the anterior half

Reduced need for alveolar bone grafting by presurgical orthopedics and primary gingivoperiosteoplasty.

OBJECTIVE: The purpose of this study was to evaluate if narrowing and approximation of the alveolar cleft through presurgical alveolar molding followed by gingivoperiosteoplasty (GPP) at the time of lip repair reduces the need for a bone-grafting procedure. DESIGN: This was a retrospective blind study of patients with unilateral or bilateral alveolar clefts who underwent presurgical infant alveolar molding and GPP by a single surgeon. Alveolar bone formation was assessed prior to the eruption of the maxillary lateral incisor or canine by clinical examination, panoramic and periapical radiogra

Patterns of Recurrence and Oncologic Outcomes After Maxillectomy: Does Reconstructive Modality Matter?

BACKGROUND: Reconstruction following maxillectomy for oral cavity malignancies is essential for restoring form and function. Obturator reconstruction facilitates direct visualization of the surgical bed, whereas flap-based reconstructions may obscure sites of potential recurrence. This has raised concern that reconstructive modality could influence the timeliness of recurrence detection, and subsequently, oncologic outcomes. PURPOSE: The purpose of this study was to measure the association between reconstructive modality and time to detection of local recurrence and survival outcomes. STUDY

Process and outcome study of multidisciplinary prosthetic treatment for velopharyngeal dysfunction.

BACKGROUND: A prosthetic approach to velopharyngeal dysfunction (VPD) is not new. However, a collaborative interdisciplinary team approach by a speech-and-language therapist, dental specialist and maxillofacial technician, including accurate fitting using nasendoscopy, has provided an opportunity to define the clinical care pathway, and audit the outcomes of this intervention. Systematic outcome studies of the effectiveness of prosthetic appliances are few and largely anecdotal. AIMS: The aim is twofold: first, to provide a description of the process including diagnosis, clinical and technica
