Urgent Care Stewardship for Glucocorticoid Overuse in Acute Respiratory Infections.

IMPORTANCE: Systemic glucocorticoids have little or no demonstrated efficacy for acute respiratory infections (ARIs) and have known adverse effects. Clinicians in urgent care centers vary widely in their use of glucocorticoids for patients seen for ARIs. OBJECTIVE: To develop and test a multifaceted urgent care stewardship program to reduce the use of glucocorticoids for ARIs. DESIGN, SETTING, AND PARTICIPANTS: This quality improvement study included clinicians seeing patients from January 2023 through April 2025 presenting with ARIs at 2 urgent care centers within an academic health system.

Communication and Cholera Morbus: measures and fears in 19th Century Italy in the face of an unknown evil. Medical ignorance and ideological paradigms unchanged over time.

ABSTRACT: The arrival of Cholera Morbus in Europe and Italy elicited a prompt institutional reaction. Starting in 1831, preventive measures characteristic of the plagues of the "Ancien Régime" were reactivated. The new disease triggered several instinctive and irrational reactions in the population, which resulted in the most diverse fears: fear of death, of doctors, of food, isolation, and abuse of food and alcohol. Ignorance of the origin of the disease, its etiology and modes of transmission meant that containment measures and treatment were often inefficacious. When patients recovered from

Cardioprotective and mentally protective effects of the Mediterranean-dietary approaches to stop hypertension intervention for neurodegenerative delay diet combined with forest bathing among adults with elevated blood cholesterol: a parallel group randomised controlled study protocol.

INTRODUCTION: The prevalence of cardiovascular diseases (CVDs) is rapidly increasing across Asia, with the burden particularly high among individuals aged ≥50 years. Elevated low-density lipoprotein cholesterol (LDL-C) level is a well-established causal risk factor for CVDs. Mediterranean-Dietary Approaches to Stop Hypertension Intervention for Neurodegenerative Delay (MIND) diet is a cardioprotective diet, which is rich in plant-based foods. Combining it with stress-reducing practices, including forest bathing (FB), which involves immersive exposure to forest environments, has been shown to r

Adherence to medication regimens: updating a complex medical issue.

Clinicians face nonadherence as the norm in everyday medical practice. The literature suggests a number of techniques that are likely to increase adherence when incorporated into regular clinical practices and routines. Central to these guidelines appears to be the doctor-patient relationship. For instance, the physician who uses understandable language, encourages open doctor-patient exchange, fosters participation by patients in their own medical care, and creates a friendly and efficient environment should increase the likelihood of adherence. Clinicians can also check adherence to medicati

Comparison of the effect of educational software and booklet on anxiety and pain during labour: a randomised controlled clinical trial.

This randomised clinical trial was conducted on 153 pregnant women. Participants were assigned into two intervention groups including educational software and an educational booklet and a control group through block randomisation. A training session was implemented for the intervention groups at 30-36 weeks and they were taught how to use the educational methods. Participants were followed-up until the childbirth time and the severity of pain was measured at four stage of cervical dilatation (4, 6, 8 and 10 cm) by Visual Analogue Scale (VAS). Spielberger State-Trait Anxiety Inventory was compl

Balancing obligations: should written information about life-sustaining treatment be neutral?

Parents who are facing decisions about life-sustaining treatment for their seriously ill or dying child are supported by their child's doctors and nurses. They also frequently seek other information sources to help them deal with the medical and ethical questions that arise. This might include written or web-based information. As part of a project involving the development of such a resource to support parents facing difficult decisions, some ethical questions emerged. Should this information be presented in a strictly neutral fashion? Is it problematic if narratives, arguments or perspectives

What is the impact on recruitment of a shortened compared with a standard-length participant information leaflet? PROMETHEUS in IBD-BOOST: study within a trial, a decentralised UK randomised controlled trial.

BACKGROUND: Participant Information Leaflets (PILs) are lengthy and increasingly complex, and could deter research participation. A shortened PIL may be more appealing as it is likely to provide a more a manageable volume of information. Previous research has found that shortened PILs are no less effective for recruitment outcomes, and we deemed it useful to replicate this in an online setting. We also decided to compare retention rates, given the potential for more information to increase participants' motivation. AIM: To evaluate the efficacy of a shortened vs standard-length PIL on trial r

A realist evaluation of patients' decisions to deprescribe in the EMPOWER trial.

BACKGROUND AND OBJECTIVES: Successful mechanisms for engaging patients in the deprescribing process remain unknown but may include: (1) triggering motivation to deprescribe by increasing patients' knowledge and concern about medications; (2) building capacity to taper by augmenting self-efficacy and (3) creating opportunities to discuss and receive support for deprescribing from a healthcare provider. We tested these mechanisms during theEliminating Medications through Patient Ownership of End Results (EMPOWER) () trial and investigated the contexts that led to positive and negative deprescrib

Taking ACTION to reduce pain: ACTION study rationale, design and protocol of a randomized trial of a proactive telephone-based coaching intervention for chronic musculoskeletal pain among African Americans.

BACKGROUND: Rates of chronic pain are rising sharply in the United States and worldwide. Presently, there is evidence of racial disparities in pain treatment and treatment outcomes in the United States but few interventions designed to address these disparities. There is growing consensus that chronic musculoskeletal pain is best addressed by a biopsychosocial approach that acknowledges the role of psychological and environmental factors, some of which differ by race. METHODS/DESIGN: The primary aim of this randomized controlled trial is to test the effectiveness of a non-pharmacological, sel

Association Between Utilization of Quitline Services and Probability of Tobacco Abstinence in Low-Income Smokers.

INTRODUCTION: Quitlines (QL) are an effective means for smoking cessation, but a paucity of data exist examining the dose-response relationship between use of QL services and quit rates, especially among low-income smokers. The purpose of this study was to study the relationship between tobacco abstinence and use of QL services among low-income smokers. METHODS: Secondary analysis of a randomized trial of every- or some-day smokers aged 18 years or older visiting an urban emergency department. Inclusion criteria included self-pay or Medicaid insurance, as a proxy for low-income and low socioe

A replicable process for redesigning ethnically relevant educational materials.

BACKGROUND: To serve the populations targeted by Well-Integrated Screening and Evaluation for Women Across the Nation (WISEWOMAN) effectively, healthcare providers need educational materials that are evidence based and ethnically relevant and can be easily incorporated into busy clinic settings. We describe a replicable process used to redesign and tailor physical activity and diet education materials for African American women in the southeastern United States. METHODS: The process consists of seven phases. Quantitative and qualitative analyses were used on data gathered in 2000 from two exp

The timing, format and content of school based sex education: an experience with a lasting effect?

School based sex education provides an opportunity to prepare young people for their sexual careers. However, research has criticised the education provided in terms of it being too late, using didactic teaching methods and focusing on biological information. The present study aimed to explore young peoples' experiences of, and beliefs about, their school sex education in terms of its timing, format and content, and to examine the relationship between these factors and their intentions to use condoms. Sixteen to 19 year olds (n = 967) from educational institutions within the South Thames regio

Conducting a diabetes mellitus prevention trial in women with GDM in Pakistan: a feasibility study.

BACKGROUND: Women with gestational diabetes mellitus (GDM) are at a greater risk of developing type 2 diabetes mellitus (T2DM) than women without GDM. Despite this elevated risk, few trials on the prevention of T2DM among South Asian women with GDM have been reported. Therefore, this study aimed to assess the feasibility of conducting a diabetes prevention program on women with a history of GDM to inform the development of a contextually relevant definitive trial. METHODS: Using a randomized controlled trial, women with GDM (n = 180) who delivered at the study hospitals (one public and one pr

Syphilis in art: an entertainment in four parts. Part 1.

It is widely recognised that the history of art reveals the contemporary attitudes of societies and artists to changing patterns of social and sexual behaviour. This collection of artistic creations shows that representations of syphilis in art, over more than five centuries, are consistent with this view. The first quarter century of the morbus gallicus in Europe, starting in 1493, coincided with the spread of Renaissance influence, including printing. A host of pamphlets with woodcut illustrations reflected public alarm at the epidemic proportions and severity of the new disease, with its di

Comparison of the effects of therapeutic exercise with either an educational booklet or vitamin-D3 supplement in the management of chronic low back pain: study protocol for an assessor blinded multicenter randomized clinical trial.

Background: Chronic low back pain (CLBP) is a common musculoskeletal disorder that significantly impairs quality of life and is frequently associated with vitamin D deficiency. Therapeutic exercise is a cornerstone of management, yet the optimal adjunct intervention to restore serum vitamin D levels remains uncertain. No prior randomized trial has compared oral vitamin D supplementation with lifestyle education on sun exposure, nutrition, and physical activity, both combined with therapeutic exercise. We hypothesize that therapeutic exercise plus an education booklet (TEB) may be superior to t

Improving the donation experience and reducing venipuncture pain by addressing fears among whole-blood and plasma donors.

BACKGROUND: Fear of blood donation is implicated in vasovagal reactions, donor recruitment, and retention. This study examined the extent to which fear among donors is associated with various donor outcomes in an Australian sample, and whether fear can be addressed on-site to reduce adverse reactions and improve the donation experience. STUDY DESIGN AND METHODS: Six hundred and sixty-four donors (age M = 33.4, SD = 12.7; 55% female) participated in a two-center, pragmatic, parallel group, individually randomized controlled trial. Following donor registration and consent, whole-blood (n = 539)
