Inferior Vena Cava Septic Thrombophlebitis: A Severe Complication of Paraspinal Steroid Injection Leading to Methicillin-Resistant Staphylococcus aureus Dissemination.

Paraspinal steroid injections, while widely employed for the treatment of back pain, carry risks of serious complications including infection. We describe a 54-year-old immunocompetent woman who developed a disseminated methicillin-resistant Staphylococcus aureus (MRSA) infection following a lumbar paraspinal steroid injection for back pain. She presented with fever, dyspnea, and acute low back pain, progressing to hypoxic respiratory failure requiring intubation. Diagnostic workup revealed MRSA bacteremia complicated by a lumbar spinal epidural abscess, bilateral psoas and quadratus lumborum

Acute Hematogenous Osteomyelitis of the Pelvis in Children.

BACKGROUND: Pelvic involvement has been reported in 3%-14% of acute hematogenous osteomyelitis (AHO) cases in children. One guideline suggests need for a longer antibiotic course in pelvic AHO, however, recent data are lacking. We describe the clinical course of children with pelvic AHO and compare it to nonpelvic AHO. METHODS: A retrospective review of patients with a diagnosis of AHO admitted to Texas Children's Hospital from January 2012 to December 2020 was conducted. Patients 6 months-<19 years old and with ≤14 days of symptoms at admission were eligible. Patients with sickle cell diseas

Biomarkers for Pediatric Bacterial Musculoskeletal Infections in Lyme Disease-Endemic Regions.

OBJECTIVES: Bacterial musculoskeletal infections (MSKIs) are challenging to diagnose because of the clinical overlap with other conditions, including Lyme arthritis. We evaluated the performance of blood biomarkers for the diagnosis of MSKIs in Lyme disease-endemic regions. METHODS: We conducted a secondary analysis of a prospective cohort study of children 1 to 21 years old with monoarthritis presenting to 1 of 8 Pedi Lyme Net emergency departments for evaluation of potential Lyme disease. Our primary outcome was an MSKI, which was defined as septic arthritis, osteomyelitis or pyomyositis. W

Severe infections of Panton-Valentine leukocidin positive Staphylococcus aureus in children.

Infections caused by Panton-Valentine leukocidin-positive Staphylococcus aureus (PVL-SA) mostly present as recurrent skin abscesses and furunculosis. However, life-threatening infections (eg, necrotizing pneumonia, necrotizing fasciitis, and osteomyelitis) caused by PVL-SA have also been reported. We assessed the clinical phenotype, frequency, clinical implications (surgery, length of treatment in hospitals/intensive care units, and antibiotic treatments), and potential preventability of severe PVL-SA infections in children.Total, 75 children treated for PVL-SA infections in our in- and outpati

Clinical Care Guideline for Improving Pediatric Acute Musculoskeletal Infection Outcomes.

BACKGROUND: Acute pediatric musculoskeletal infections are common, leading to significant use of resources and antimicrobial exposure. In order to decrease variability and improve the quality of care, Children's Hospital Colorado implemented a clinical care guideline (CCG) for these infections. The purpose of this study is to evaluate clinical and resource outcomes PRE and POST this CCG. METHODS: Retrospective chart review evaluated patients admitted to a large pediatric quaternary referral center (CHCO) diagnosed with acute osteomyelitis, septic arthritis, pyomyositis, and/or musculoskeletal

MRSA Femoral Osteomyelitis from Superinfected Scabies Lesions: A Pediatric Case Report.

Scabies is a skin infestation from the Sarcoptes scabiei. It is considered a public health issue causing concern in developing countries and is considered a "neglected tropical disease" by the World Health Organization (WHO). Scabies skin lesions may cause severe itching and can be the portal of entry for opportunistic and pathogenic bacteria, which can cause serious systemic infections. We report the case of a 3-year-old boy with recurrent scabies infections who presented to the emergency department because of a fever and refusal to walk. Blood tests showed neutrophilic leukocytosis and signi

Calcitonin Gene-Related Peptide for Identifying Pediatric Bacterial Musculoskeletal Infections: A Prospective, Multicenter Study.

OBJECTIVES: Bacterial musculoskeletal infections (MSKIs) can be challenging to diagnose. We compared the accuracy of calcitonin gene-related peptide (CGRP), a neuropeptide which is transcribed from the same gene as procalcitonin, to procalcitonin for the diagnosis of a MSKI in children. METHODS: We conducted a prospective cohort study of patients 21 years old or younger who underwent evaluation for MSKIs at one of 3 emergency departments. Our primary outcome was a MSKI, defined as septic arthritis, osteomyelitis, or pyomyositis. We used a Spearman correlation coefficient to measure the associ

Delay to diagnosis in childhood bone and joint infection.

AIM: To determine the proportion of delayed diagnosis among cases of acute childhood bone and joint infection (BJI) and examine the impact of delayed diagnosis on illness trajectory. METHODS: A retrospective review was undertaken of patients <16 years with acute haematogenous osteomyelitis (AHO) or septic arthritis (SA) treated in the Auckland region from 2018-2023. Electronic case information was used to identify any alternative diagnosis given prior to identification of BJI (delayed disease recognition). Cases were grouped into the following subtypes: multifocal sepsis or shock, "isolated"

Troublesome Tuberculosis: A Case Report on Multi-focal Tuberculous Osteomyelitis in An Immunocompetent Patient.

UNLABELLED: Bone and joint tuberculosis is a chronic debilitating condition that leads to progressive damage and even deformity of joints. It may affect one or multiple sites. It could present in a myriad of ways which may result in an incorrect diagnosis being made. Common misdiagnoses include seronegative inflammatory arthritis, septic arthritis, malignancy, osteoporotic fractures and mechanical type back pain. It was initially only diagnosed in patients with previous active tuberculosis or latent tuberculosis. However, in recent years, it has also been reported in patients without a history

Neurological disorders in Nigerian Africans: a community-based study.

In a Nigerian town with a stable population of 20,000, a door-to-door survey was conducted, using a questionnaire involving a complete census and a simple neurological evaluation which had previously showed a 95% sensitivity and an 80% specificity for detecting neurological disease. Positive responders were evaluated and categorised, using agreed criteria for diagnoses. Nearly 100% cooperation was obtained. Life prevalence ratio for at least one episode of headache was 51/1000. Crude point prevalence ratio for migrainous headache was 5.3/100, and peak age-specific ratio was in the first decade

Direct 16S/18S rRNA Gene PCR Followed by Sanger Sequencing as a Clinical Diagnostic Tool for Detection of Bacterial and Fungal Infections: a Systematic Review and Meta-Analysis.

rRNA gene Sanger sequencing is being used for the identification of cultured pathogens. A new diagnostic approach is sequencing of uncultured samples by using the commercial DNA extraction and sequencing platform SepsiTest (ST). The goal was to analyze the clinical performance of ST with a focus on nongrowing pathogens and the impact on antibiotic therapy. A literature search used PubMed/Medline, Cochrane, Science Direct, and Google Scholar. Eligibility followed PRISMA-P criteria. Quality and risk of bias were assessed drawing on QUADAS-2 (quality assessment of diagnostic accuracy studies, rev
