Misdiagnosis of Vocal Fold Nodules.

INTRODUCTION: Benign vocal fold masses (BVMs) include nodules, polyps, cysts, and other pathology. Nevertheless, some otolaryngologists and other physicians utilize "vocal fold nodules" as an umbrella diagnosis for vocal fold masses. Patients subsequently evaluated by a laryngologist are found to have a different vocal fold mass, which often has a different prognosis and treatment plan compared to nodules. OBJECTIVE: The purpose of this study was to investigate the rate at which vocal fold nodules are misdiagnosed. METHODS: Adult voice patients who presented to our voice center having been e

Quantitative electromyographic characteristics of idiopathic unilateral vocal fold paralysis.

OBJECTIVES/HYPOTHESIS: Unilateral vocal fold paralysis with no preceding causes is diagnosed as idiopathic unilateral vocal fold paralysis. However, comprehensive guidelines for evaluating the defining characteristics of idiopathic unilateral vocal fold paralysis are still lacking. In the present study, we hypothesized that idiopathic unilateral vocal fold paralysis may have different clinical and neurologic characteristics from unilateral vocal fold paralysis caused by surgical trauma. STUDY DESIGN: Retrospective, case series study. METHODS: Patients with unilateral vocal fold paralysis wer

Diagnosis and management of unilateral thyroarytenoid muscle palsy.

The objective of this study is to assess and propose a method of diagnosis and management of patients with unilateral thyroarytenoid muscle palsy (TAMP). This is a retrospective review of clinical records. The records of seven patients diagnosed as having idiopathic TAMP were reviewed. Despite the adductive and abductive functions of the vocal folds being within normal range, apparent palsy was seen in the unilateral thyroarytenoid muscle of these patients. TAMP was confirmed by laryngeal electromyography, and the adductive and abductive movements of the vocal folds were evaluated as the mobil

Glottic closure patterns: type I thyroplasty versus type I thyroplasty with arytenoid adduction.

OBJECTIVES/HYPOTHESIS: The goal of laryngeal framework surgery in patients with unilateral vocal fold paralysis is to improve glottic closure by medializing the paralyzed vocal fold. Type I thyroplasty (Th) and arytenoid adduction (AA) are two of the most commonly performed procedures. Two of the main rationales for performing an AA are to improve closure of the posterior glottis and correct vertical height discrepancy. The purpose of this study was to evaluate if AA with Th yields better posterior glottic closure and vertical height equality than Th alone. STUDY DESIGN: Retrospective. METHO

Long-term results of autologous fascia in unilateral vocal fold paralysis.

The objective of this retrospective clinical review was to evaluate the long-term results of injection laryngoplasty with autologous fascia as a single, primary procedure in unilateral vocal fold paralysis. Forty-three patients who had undergone injection laryngoplasty between 1996 and 2003 entered the study. Clinical examination and videostroboscopy were performed and the voice handicap index was analyzed postoperatively. Pre- and post-operative evaluation included computerized acoustic analysis and perceptual evaluation. The results remained stable 3-10 years and were not affected by the len

Applicability of the Polish Vocal Tract Discomfort (VTD) scale in the diagnostics of occupational dysphonia.

Occupational dysphonia is mainly of hyperfunctional origin, expressed by complaints experienced in the vocal tract. The aim of the paper was to evaluate the applicability of the Polish Vocal Tract Discomfort (VTD) scale in the diagnostics of occupational dysphonia. Patients with occupational dysphonia (n = 218) and controls (n = 140) were subjected to the VTD scale, VHI evaluation, and maximum phonation time (MPT) measurement. The patients also underwent videostroboscopy. The comparison of the frequency and severity of VTD symptoms showed that the results of the study group were significantly

Outcome of laryngeal manual therapy in four Dutch adults with persistent moderate-to-severe vocal hyperfunction: a pilot study.

A relatively new management strategy for the treatment of voice disorders is the use of laryngeal manual therapy. The main purpose of the present pilot study is to document the outcome of vocal quality after a well-defined laryngeal manual therapy (LMT) program. Four Dutch professional voice users with a persistent moderate or severe muscle tension dysphonia were studied pretreatment (1 week before LMT) and posttreatment (1 week) after completion of manual therapy (25 sessions). These subjects had received several months of traditional voice therapy, without any success. To measure and compare

Is it possible to predict functional recovery of vocal folds palsy? Role of intraoperative neuromonitoring and laryngostroboscopy findings.

BACKGROUND: Vocal fold palsy is a common reason for claims after thyroidectomy. We evaluated the prognostic role of intraoperative neuromonitoring and postoperative flexible fiberoptic laryngostroboscopy findings in patients with type I or type II loss of signal, indicating the injury in an exact point of recurrent laryngeal nerve and whether the damage level cannot be determined, respectively. METHODS: Among 4,526 intraoperative neuromonitoring-assisted thyroidectomies (2021-2024), all cases of loss of signal were prospectively analyzed. Flexible fiberoptic laryngostroboscopy was performed i

Phonation Threshold Pressure/Flow for Reflecting Glottal Closure in Unilateral Vocal Fold Paralysis.

OBJECTIVE: Previous theoretical analysis predicted that phonation threshold flow (PTF) could be a more sensitive aerodynamic measure than phonation threshold pressure (PTP) for reflecting glottal incompetence. This study investigated the feasibility of whether PTP and PTF may differentiate subjects with unilateral adductor vocal fold paralysis and paresis (UAVFP) from those without, and whether PTP and PTF could reflect the extent of incomplete glottal closure associated with UAVFP. METHODS: PTP and PTF were quantified for 13 subjects with UAVFP and 21 control subjects with normal voice, and
