Natural strabismus in monkeys: accommodative errors assessed by photorefraction and their relationship to convergence errors.

PURPOSE: To examine accommodation and accommodative convergence relationships in naturally strabismic monkeys. METHODS: Photorefraction was used to measure accommodative responses of each eye under monocular and binocular viewing conditions. These accommodative results were then compared to assessments of vergence state made under monocular viewing conditions using methods previously described. RESULTS: Accommodation was always accurate under monocular viewing conditions, with the exception of the inability of one myopic eye to focus distant targets. The accommodative response in the two eye

Electrophysiologic findings and prognosis in Bell's palsy.

Electrophysiologic investigations were carried out on 45 patients with Bell's palsy at periodic intervals after the onset of paralysis. It was found that there was a good correlation between prognosis in Bell's palsy and the amplitude of evoked motor response obtained after six or more days of clinical paresis. When the average amplitude of evoked motor response was within normal limits (i.e., 504 mu V or greater), complete recovery with no residual deficits took place two to six weeks after the onset of facial palsy. When the evoked motor response was absent in all three major branches of the

Automatic grading of patients with a unilateral facial paralysis based on the Sunnybrook Facial Grading System - A deep learning study based on a convolutional neural network.

PURPOSE: In order to assess the severity and the progression of a unilateral peripheral facial palsy the Sunnybrook Facial Grading System (SFGS) is a well-established grading system due to its clinical relevance, sensitivity, and robust measuring method. However, training is required in order to achieve a high inter-rater reliability. This study investigated the automated grading of facial palsy patients based on the SFGS using a convolutional neural network. METHODS: A total of 116 patients with a unilateral peripheral facial palsy and 9 healthy subjects were recorded performing the Sunnybro

Efficacy of early physical therapy in severe Bell's palsy: a randomized controlled trial.

BACKGROUND: Bell's palsy (BP) is the most frequent form of peripheral palsy of the facial nerve. Prognosis for recovery is good for most patients; in the remaining cases, different grades of residual impairment persist. Physical therapy, in association with drug administration, aims to improve outcomes. OBJECTIVE: To assess the efficacy of early physical therapy in association with standard drug administration versus pharmacological therapy only, in terms of time to maximum gains and grade of recovery of function, and to examine who will most benefit from rehabilitation. METHODS: From June 2

Surgical anatomy of the recurrent laryngeal nerve: implications for laryngeal reinnervation.

Functional laryngeal reinnervation depends upon the precise reinnervation of the laryngeal abductor and adductor muscle groups. While simple end-to-end anastomosis of the recurrent laryngeal nerve (RLN) main trunk results in synkinesis, functional reinnervation can be achieved by selective anastomosis of the abductor and adductor RLN divisions. Few previous studies have examined the intralaryngeal anatomy of the RLN to ascertain the characteristics that may lend themselves to laryngeal reinnervation. Ten human larynges without known laryngeal disorders were obtained from human cadavers for RLN

Effect of botulinum toxin injection on speech outcomes among patients with synkinetic facial nerve palsy: An assessment of speech intelligibility and quality of life.

BACKGROUND: Facial nerve palsy (FNP) considerably impairs functional outcomes and quality of life. For patients with synkinetic FNP, physiotherapy and botulinum toxin A (BTX) are commonly used to reduce hyperkinesis and improve facial expression. This study assessed the effect of BTX administration on oral competence in patients with FNP. METHODS: Patients referred for BTX for management of their synkinetic FNP were recruited from the Sydney Facial Nerve Service. BTX was administered by a single surgeon. Their oral competence was measured using clinician and patient-rated outcome measures bef

Corticosteroids for Bell's palsy (idiopathic facial paralysis).

BACKGROUND: Inflammation and oedema of the facial nerve are implicated in causing Bell's palsy. Corticosteroids have a potent anti-inflammatory action that should minimise nerve damage. This is an update of a review first published in 2002 and last updated in 2010. OBJECTIVES: To determine the effectiveness and safety of corticosteroid therapy in people with Bell's palsy. SEARCH METHODS: On 4 March 2016, we searched the Cochrane Neuromuscular Specialised Register, the Cochrane Central Register of Controlled Trials (CENTRAL), MEDLINE, EMBASE and LILACS. We reviewed the bibliographies of the r

Ultrasound Guidance for Botulinum Toxin Injection of Muscles Innervated by the Facial Nerve: A Systematic Review of Anatomical Precision, Safety, and Outcomes.

Botulinum toxin type A (BoNT-A) chemodenervation is commonly used for facial synkinesis and aesthetic indications, but landmark-based techniques are limited by anatomical variability and risk of off-target delivery. High-resolution ultrasound (US) can be used to enhance precision and safety. This systematic review explores the role of US-guided BoNT-A facial chemodenervation in evaluating anatomical accuracy, clinical efficacy, complications, and patient satisfaction. A comprehensive search of 6 databases through April 2025 identified studies assessing US-guided BoNT-A chemodenervation for fac

Transcutaneous Facial Nerve Frontal Branch Stimulation to Restore Dynamic Elevation of the Paralyzed Eyebrow in Synkinetic Patients.

BACKGROUND: While patients commonly recover motion following facial nerve injury, aberrant neural regeneration can lead to synkinesis and chronic nonflaccid facial palsy (NFFP); many patients fail to recover volitional eyebrow elevation. OBJECTIVE: To assess if transcutaneous electrical stimulation of injured frontal branches can achieve eyebrow elevation. METHODS: We studied healthy volunteers (N = 10) and patients with unilateral NFFP (N = 36). We performed monopolar transcutaneous stimulation of healthy and paralyzed frontal branches with 4-10 mA pulse trains. We measured eyebrow elevatio

Does supercharging with cross-face nerve graft enhance smile in non-flaccid facial paralysis patients undergoing selective neurectomy?

BACKGROUND: Cross face nerve grafting (CFNG) is a well-established nerve transfer technique in facial reanimation; however, no study has assessed outcome of supercharging the smile with CFNG in patients with synkinesis. The goal of this study was to examine the smile outcome in non-flaccid facial paralysis (NFFP) patients after supercharging with CFNG during selective neurectomy. METHODS: NFFP patients who underwent CFNG with end-to-side coaptation to a smile branch on the paralyzed side during selective neurectomy were retrospectively identified and their charts were reviewed. Facial reanimation with interposition nerve graft or masseter nerve transfer: a comparative retrospective study.

Both interposition nerve grafts and masseter nerve transfers have been successfully used for facial reanimation after irreversible injuries to the cranial portion of the facial nerve. However, no comparative study of these two procedures has yet been reported. In this two-site, two-arm, retrospective case review study, 32 patients were included. Of these, 17 patients (eight men and nine women, mean age 42.1 years) underwent interposition nerve graft after tumor extirpation or trauma between 2003 and 2006 in the Ear Institute, School of Medicine, Shanghai Jiao Tong University, China, and 15 pat

Vocal fold motion outcome based on excellent prognosis with laryngeal electromyography.

OBJECTIVES/HYPOTHESIS: As laryngeal electromyography (LEMG) becomes more refined, accurate predictions of vocal fold motion recovery are possible. Focus has been on outcomes for patients with poor prognosis for vocal fold motion recovery. Limited information is available regarding the expected rate of purposeful vocal fold motion recovery when there is good to normal motor recruitment, no signs of denervation, and no signs of synkinetic activity with LEMG, termed excellent prognosis. The objective of this study is to determine the rate of vocal fold motion recovery with excellent prognosis fin

Efficacy and Short-term Durability of Combined Botulinum Toxin Type A and Rehabilitation for Chronic Facial Asymmetry After Facial Nerve Palsy.

BACKGROUND: Although the benefits of botulinum toxin last for 3-4 months, the durability has not been quantitatively evaluated. Recent studies have suggested that combining botulinum toxin injections with neuromuscular retraining can enhance both functional and aesthetic results. This study aims to assess the efficacy and short-term durability up to 4 months of this combined approach in patients with chronic facial asymmetry after facial nerve palsy. METHODS: We retrospectively evaluated patients with facial asymmetry due to facial palsy treated with botulinum toxin type A injections followed

Age-related changes in platysma insertion height and the clinical role of high-resolution ultrasound in the elderly.

BACKGROUND: There is no consensus on the exact insertion height of the platysma muscle, and the influence of aging on its cranial position remains unexplored. Additionally, clinicians currently rely solely on clinical examination to assess the muscle-a limitation that may contribute to complications when treating platysma synkinesis with botulinum toxin injections. This study aimed to determine the cranial insertion height of the platysma in an elderly cohort, explore how aging affects its cranial position, and validate high-resolution ultrasound as a non-invasive tool for visualizing and asse
