Stakeholder Perspectives on Advancing Understanding of Prenatal Opioid Exposure and Brain Development From the iOPEN Consortium of the Healthy Brain and Child Development Study.

Introduction: There is a dire need for research regarding the implications of opioid use during pregnancy on fetal and childhood development to better inform both medical practice and policy. The Healthy Brain and Child Development Study will examine brain and behavioral development from birth through the first decade of life. Due to large scope and anticipated complexity of this initiative, an 18-month planning phase was implemented across 28 sites across the nation. A core element of the Phase I initiative involved the development of Stakeholder Advisory Committees to inform the next phase o

Farm Fresh Foods for Healthy Kids (F3HK): An innovative community supported agriculture intervention to prevent childhood obesity in low-income families and strengthen local agricultural economies.

BACKGROUND: Childhood obesity persists in the United States and is associated with serious health problems. Higher rates of obesity among children from disadvantaged households may be, in part, attributable to disparities in access to healthy foods such as fruits and vegetables. Community supported agriculture can improve access to and consumption of fresh produce, but the upfront payment structure, logistical barriers, and unfamiliarity with produce items may inhibit participation by low-income families. The aim of this project is to assess the impact of subsidized, or "cost-offset," communit

Gay marriage, same-sex parenting, and America's children.

Same-sex marriage, barely on the political radar a decade ago, is a reality in America. How will it affect the well-being of children? Some observers worry that legalizing same-sex marriage would send the message that same-sex parenting and opposite-sex parenting are interchangeable, when in fact they may lead to different outcomes for children. To evaluate that concern, William Meezan and Jonathan Rauch review the growing body of research on how same-sex parenting affects children. After considering the methodological problems inherent in studying small, hard-to-locate populations--problems t

The social validity of Social Stories™ for supporting the behavioural and communicative functioning of children with autism spectrum disorder.

This study examines the social validity of a family-centred collaborative approach to developing Social Stories™ to support the behavioural and communicative functioning of children with autism spectrum disorder (ASD). Twenty children with Pervasive Developmental Disorder (PDD)/Autism, PDD-Not Otherwise Specified, or Asperger's Disorder (aged 4-12 years) participated in a multiple baseline design across behaviours with a 6-week follow-up. The effects of behaviour stories (to reduce problem behaviours) and communication stories (to facilitate communication) as assessed by parental subjective pe

Illicit drug use among pregnant women enrolled in treatment for cigarette smoking cessation.

INTRODUCTION: Smoking during pregnancy is the leading preventable cause of poor pregnancy outcomes in the United States. In population studies and nationwide surveys, pregnant smokers report more illicit drug use than pregnant nonsmokers. The purpose of this study was to examine the prevalence of illicit drug use among pregnant women enrolled in clinical trials for smoking cessation. METHODS: Urine specimens from 115 pregnant women were tested for illicit drug use during a study intake visit (~10th week of pregnancy) and during the final antepartum (FAP) smoking-status assessment (~28th week

Molecular epidemiology, evolution, and transmission dynamics of raccoon rabies virus in Connecticut.

In North America, raccoon rabies virus (RRV) is a public health concern due to its potential for rapid spread, maintenance in wildlife, and impact on human and domesticated animal health. RRV is an endemic zoonotic pathogen throughout the eastern USA. In 1991, an outbreak of RRV in Fairfield County, Connecticut, spread through the state and eventually throughout the Northeast and into Canada. Factors that contribute to, or curb, RRV transmission should be explored and quantified to guide targeted rabies control efforts, including the size and location of buffer zones of vaccinated animals. How

Genetic population structure and reproductive system of two invasive Asian earthworms, Amynthas tokioensis and Amynthas agrestis.

The invasive Asian earthworms, Amynthas tokioensis and A. agrestis, have been successful in entering North American forests in recent decades, with significant damage to both soils and above-ground environments. This success could be driven in part by a polyploid genetic system and parthenogenetic reproduction, often suggested as benefits for invasive species. Therefore, we assessed the genetic population structure, genetic diversity, and reproductive system of both species using morphological traits and panels of microsatellite markers. A total of 216 A. tokioensis and 196 A. agrestis from si

Neonatal and fetal growth charts to identify preterm infants <30 weeks gestation at risk of adverse outcomes.

BACKGROUND: It is unclear whether a neonatal or a fetal growth standard is a better predictor of adverse in-hospital newborn infant outcomes. OBJECTIVE: We aimed to evaluate and compare the power of birthweight for gestational age to predict adverse neonatal outcomes using neonatal and fetal growth charts. Gestational age-specific birthweight was examined either as a percentile score or as a binary indicator for birthweight <10th percentile (small for gestational age) with the use of 3 fetal growth charts (National Institute of Child Health and Human Development, World Health Organization, an

Survey and evidence based review of probiotics used in very low birth weight preterm infants within the United States.

OBJECTIVE: Data from multiple clinical trials, mostly conducted outside the US, indicate that probiotic prophylaxis is an effective intervention for prevention of necrotizing enterocolitis (NEC) in preterm infants. Probiotics are routinely used in many countries. However, in the US, probiotic use in preterm infants is limited (6.7% of very low birth weight (VLBW) infants in the US were exposed to probiotics in 2014, Vermont Oxford Network (VON)). Probiotic products are often considered in 'generic' terms, but considerable variation exists between commercially available probiotics in terms of t

NICU bedside caregivers sustain process improvement and decrease incidence of bronchopulmonary dysplasia in infants < 30 weeks gestation.

BACKGROUND: The objective of this study was to investigate whether a respiratory care bundle, implemented through participation in the Vermont Oxford Network-sponsored Neonatal Intensive Care Quality Improvement Collaborative (NIC/Q 2005) and primarily dependent on bedside caregivers, resulted in sustained decrease in the incidence of bronchopulmonary dysplasia (BPD) in infants < 30 wk gestation. METHODS: A retrospective cohort study was conducted. Infants inborn between 23 wk and 29 wk + 6 d of gestation were included. Patients with congenital heart disease, significant congenital or lung an

Use of chemical warming packs during delivery room resuscitation is associated with decreased rates of hypothermia in very low-birth-weight neonates.

BACKGROUND: : Hypothermia is an independent contributor to neonatal mortality. All very low-birth-weight (VLBW) newborns have the potential to undergo cold stress or frank hypothermia during delivery room stabilization. Thus, clinicians aiming to maintain normothermia in VLBW neonates are compelled to use multiple adjuncts of unknown efficacy or safety. OBJECTIVE: : To evaluate the effectiveness of thermoregulation procedures in maintaining normothermia during delivery room resuscitation and to assess the impact of an unanticipated change in equipment at our institution on the admission tempe

Randomized controlled trial of probiotics for the prevention of spontaneous preterm delivery associated with intrauterine infection: study protocol.

BACKGROUND: Spontaneous preterm deliveries that occur before the 34th week of gestation, and particularly before the 32nd week of gestation, have been strongly associated to intrauterine infection, ascending from vagina, and represent the largest portion of neonatal deaths and neurological problems. Bacterial vaginosis, characterized by a diminished or absent flora of lactobacilli and increased colonization of several anaerobic or facultative microorganisms, increases two times the risk of preterm delivery before the 34th week. Trials of antibiotics failed to show efficacy and effectiveness ag

Emergency Department Visits for Falls and First-Time Seizure.

BACKGROUND AND OBJECTIVES: Unrecognized seizures occur in up to 77% of individuals with epilepsy, leading to delays in diagnosis with significant consequences including injury, mortality, impaired psychosocial functioning, and negative socioeconomic impacts. Falls are a common reason to present to the emergency department (ED) and may be a sentinel sign of an unrecognized seizure. Whether falls represent missed opportunities for earlier seizure recognition remains unknown. We therefore evaluated the temporal association between ED visits for falls and first-time seizure. METHODS: We conducted

Mortality Trends in Patients Aged 25 and Older With Comorbid COPD and Diabetes Mellitus in the United States: A CDC WONDER Database Study From 2000 to 2023.

INTRODUCTION: Diabetes mellitus (DM) and chronic obstructive pulmonary disease (COPD) are among the leading chronic diseases that significantly impact global morbidity and mortality. Growing evidence indicates a bidirectional interplay between metabolic and pulmonary dysfunction. This study aims to identify the demographic and regional trends of DM and COPD-related mortality in the United States from 2000 to 2023. METHODOLOGY: DM- and COPD-related deaths in the United States from 2000 to 2023 were obtained from the CDC WONDER database using ICD-10 code E10-14 for DM and J40-44 as the underlyi

Epidemiological trends and disparities in mortality from gastrointestinal tract obstruction in the United States: A CDC WONDER analysis (1999-2023).

Gastrointestinal obstruction is an important cause of morbidity and mortality in the United States, but full descriptions of time trends and disparities are poorly defined. This study aims to examine trends in mortality from gastrointestinal obstruction over a span of 25 years in various demographics and geographic locations within the United States from 1999 to 2023. We conducted a retrospective analysis using CDC WONDER multiple cause-of-death data. Deaths were identified when gastrointestinal obstruction was listed as either the underlying or a contributing cause of death. Age-adjusted mort

At-Home Transvaginal Pelvic Ultrasonography and Image Quality in Premenopausal Women: A Nonrandomized Clinical Trial.

IMPORTANCE: Transvaginal pelvic imaging is an integral and sensitive aspect of gynecologic care that individuals find both invasive and burdensome. At-home transvaginal imaging provides an opportunity to navigate these barriers to care. OBJECTIVE: To evaluate the use of at-home transvaginal ultrasonography for gynecologic assessment, as measured by diagnostic imaging quality and participant satisfaction, in a broad sampling of women aged 22 to 50 years. Regional, demographic, and temporal trends in psychoactive substance use-related mental disorder and cancer mortality in U.S. adults: a nationwide CDC WONDER analysis (1999-2020).

BACKGROUND: Cancer remains a major global health burden, with 10 million deaths in 2020 and among the 35 million adults worldwide with psychoactive substance use disorders (SUDs), it is a major contributor to premature mortality. In the U.S., tobacco causes over 30% of cancer deaths and alcohol nearly 5%, highlighting the role of modifiable behaviors. Despite this clinical and economic burden, national long-term data on co-occurring SUDs and cancer mortality remain limited. We therefore analyzed 22 years (1999-2020) of U.S. mortality data from CDC WONDER (Centers for Disease Control and Preven

The role of prenatal food insecurity on breastfeeding behaviors: findings from the United States pregnancy risk assessment monitoring system.

BACKGROUND: In addition to its health and nutritional benefits, breastfeeding can save low-income, food insecure mothers the cost of infant formula so that money can be spent on food and other necessities. Yet breastfeeding may exacerbate food insecurity by negatively affecting maternal employment. The relationship between food insecurity and breastfeeding has been explored previously, with varying results. The purpose of this study was to determine the relationship between prenatal food insecurity and breastfeeding initiation and early cessation (< 10 weeks) among U.S. mothers. METHODS: Data

Prospective evaluation of postnatal steroid administration: a 1-year experience from the California Perinatal Quality Care Collaborative.

OBJECTIVE: Postnatal steroids (PNSs) are used frequently to prevent or treat chronic lung disease (CLD) in the very low birth weight (VLBW) infant, and their use continues despite concerns regarding an increased incidence of longer-term neurodevelopmental abnormalities in such infants. More recently, there has been a suggestion that corticosteroids may be a useful alternative therapy for hypotension in VLBW infants, but there have been no prospective reports of such use for a current cohort of VLBW infants. METHODS: The California Perinatal Quality Care Collaborative (CPQCC) requested members

Agoraphobia, panic disorder and generalized anxiety disorder: some implications of recent advances.

The nature of the relationship between 'panic disorder', agoraphobia and general anxiety disorder remains open. The aetiological theories which have tried to link them with the aid of biological and psychological concepts fail to take account of conflicting observations. 'Panic' attacks are not confined to agoraphobic and related disorders, being indistinguishable from the attacks of acute anxiety and phobic aversion manifest in a wide range of anxiety and affective disorders. There is continuity and discontinuity in the evolution of agoraphobia; those affected differ in respect of a range of
