Semaglutide alleviates osteoarthritis independent of weight loss via GLP-1R-mediated activation of autophagy through AKT/mTOR inhibition.

OBJECTIVE: The development of osteoarthritis (OA) is closely associated with systemic metabolic disorders, yet there remains a lack of disease-modifying therapeutic strategies that simultaneously target metabolic abnormalities and inflammatory responses. This study aims to systematically evaluate the therapeutic potential of semaglutide, a long-acting glucagon-like peptide-1 receptor (GLP-1R) agonist used for diabetes management, in OA and to elucidate its underlying molecular mechanisms. METHODS: We utilized a zebrafish cartilage injury repair model to screen and assess the impact of several

Preclinical Evaluation of Human Donor-Derived Micronized Bone Marrow Stroma/Parenchyma Versus Bone Marrow Aspirate Concentrate in a Rat Model of Post-Traumatic Knee Osteoarthritis.

Bone marrow aspirate concentrate (BMAC) is widely used as a source of mesenchymal stromal/stem cells (MSCs) for musculoskeletal regeneration; however, BMAC lacks essential bone marrow extracellular matrix (ECM) components, a critical component of the stem cell niche that regulates MSC survival, paracrine signaling, and regenerative capacity. We previously demonstrated that an ECM-retaining micronized bone marrow product (BMAX™) preserves pro-regenerative MSC phenotypes in vitro. Human bone marrow from a single donor was processed into conventional BMAC or BMAX™. Post-traumatic osteoarthritis w

Bilateral anterior cruciate ligament reconstruction is more vulnerable to graft failure than unilateral anterior cruciate ligament reconstruction.

PURPOSE: To investigate the graft survival rates of bilateral anterior cruciate ligament reconstruction (ACLR) and to evaluate whether the graft failure rate of bilateral ACLR is different from that of unilateral ACLR. HYPOTHESIS: The graft survival rate of bilateral ACLR is lower than that of unilateral ACLR. METHODS: All ACLRs followed up in our institution from 1997 to 2022 were retrospectively identified. ACLRs were divided into the bilateral and unilateral ACLR groups. Each ACLR was classified as bilateral group if contralateral knee had undergone ACLR; otherwise, it was classified as u

A novel meniscal root refixation pull-in technique with an all-suture anchor shows biomechanical properties comparable to standard suture anchor and transtibial pull-out techniques.

PURPOSE: Repair techniques for posterior meniscal root (PMMR) tears include repair with transtibial pull-out and anchors. An alternative approach uses all-suture anchors pulled in, avoiding a posterior medial portal. While clinical feasibility has been assessed, biomechanical properties of this technique remain unknown. We hypothesised that the biomechanical properties using the pull-in technique would be comparable to those achieved with conventional repair techniques. METHODS: Fifty fresh-frozen porcine tibiae were fixed in a steel pot. Whereas in group (1) the native meniscal root was kept

Sodium tanshinone IIA sulfonate alleviates osteoarthritis through targeting SIRT1.

BACKGROUND: Osteoarthritis (OA), a chronic degenerative disease, is characterized by the loss of articular cartilage, impacting more than 500 million individuals worldwide. Sodium tanshinone IIA sulfonate (STS) is a water-soluble derivative of tanshinone IIA derived from Salvia miltiorrhiza and has anti-inflammatory and anti-oxidative functions. Although STS shows significant pharmacological effects and mechanisms in treating various diseases in vivo and in vitro, its specific treatments and mechanisms for OA remain largely unknown. MATERIALS AND METHODS: Primary chondrocytes were stimulated

Increased Medial Meniscal Slope Is Associated With Greater Risk of Ramp Lesion in Noncontact Anterior Cruciate Ligament Injury.

BACKGROUND: A special type of meniscal lesion involving the peripheral attachment of the posterior horn of the medial meniscus (PHMM), termed ramp lesion, is commonly associated with anterior cruciate ligament (ACL) injury. However, no study has investigated its anatomic risk factors. Recently, increased meniscal slope has been identified as an independent anatomic risk factor for noncontact ACL injury. HYPOTHESIS: Increased medial meniscal slope (MMS) as measured on magnetic resonance imaging (MRI) will correlate with greater risk of concomitant ramp lesion in noncontact ACL injury. STUDY D

[Treatment of chondral lesions from knee injuries in the professional soccer players. Review of 34 cases].

OBJECTIVE: Evaluate the results of Pridie chondroplasty as an efficient treatment to recover the complete activity in soccer players and compare this with others chondral repair techniques. MATERIAL AND METHODS: The patients were professional soccer players, all with a knee cartilage injury treated with the Pridie technique in an arthroscopic surgery between March 1999--December 2004. The age of the patients and the presence of a simple meniscal tear wasn't a fact to exclude a patient. Complex meniscal tear and a ACL rupture were an exclusion criteria. We deferred the support by 6 to 8 weeks.

Functional gait adaptations in patients with anterior cruciate ligament deficiency over time.

Functional adaptations in knee kinematics and kinetics during level walking in 30 patients with unilateral anterior cruciate ligament deficiency and 30 healthy control subjects were studied. To examine the impact of time after injury on the functional adaptations, patients were placed into three time interval groups: early, intermediate, and chronic. The patients with anterior cruciate ligament deficiency had significantly decreased external knee flexion moments (balanced by net quadriceps contraction) and significantly increased external knee extension moments (balanced by net knee flexors co

Warm Needle Acupuncture Attenuates Pain-Related Responses and Inflammation in Knee Osteoarthritis: Evidence for Involvement of TAK1/MAPK Signaling from Human Cartilage and Experimental Models.

BACKGROUND: Warm needle acupuncture (WNA) can improve symptoms in knee osteoarthritis (KOA). However, the molecular changes linked to its effects remain unclear. METHODS: This study combined human cartilage proteomics, a rat model of destabilization of the medial meniscus (DMM), and ex vivo primary chondrocyte analysis. The final clinical analysis included 61 patients with advanced KOA (control, n = 31; WNA, n = 30). Cartilage and synovial fluid were collected after a 21-day preoperative intervention. Cartilage samples underwent four-dimensional data-independent acquisition (4D-DIA) proteomic

Cistanche deserticola Y.C.Ma protects rats against osteoporotic osteoarthritis by inhibiting the phosphatidylinositol 3-kinase/serine-threonine protein kinase B/hypoxia-inducible factor 1 alpha signaling pathway.

ETHNOPHARMACOLOGICAL RELEVANCE: Osteoporotic osteoarthritis (OPOA) is characterized by kidney deficiency, muscle debility, and lumbar weakness, which is align closely with "deficiency in kidney yang" in Traditional Chinese Medicine (TCM) theory. As a tonic TCM herb, Cistanche deserticola Y.C.Ma. (CD) has been used for "reinforcing kidney yang" for thousand years. Although CD has been extensively studied for OP management, active components and mechanism of CD on OPOA treatment remain elusive. AIM OF THE STUDY: This study aims to investigate the impact of osteoporosis (OP) conditions on OPOA p

Primary Fixation of Single Stitch All-Inside and Inside-Out Meniscal Devices for Repairing Vertical Longitudinal Meniscal Tears: A Human Microscopic Imaging and Biomechanical Study.

BACKGROUND: Optimized surgical fixation and meniscal stabilization during rehabilitation increase healing success. However, the latest generation of all-inside devices has not yet been biomechanically compared with inside-out suture tape (IO-ST) repair. HYPOTHESIS: (1) The contact area of a suture anchor (SA) would compensate for a meniscal defect better than polyether ether ketone anchors (PA); (2) adjustable tensioning for all-inside meniscal repair fixation would result in higher initial load than IO-ST repair; and (3) stiffer constructs would decrease secondary displacement.
