Perceived social support moderates the association between household dysfunction adverse childhood experiences (ACEs) and self-reported drug use among men who have sex with men in Los Angeles, California.

BACKGROUND: Adverse childhood experiences (ACEs) have been shown to be associated with drug use in adulthood. The single ACE of household substance use history (part of the household dysfunction category) has frequently been associated with drug use. Resilience factors such as perceived social support appear to buffer the association between ACEs and drug use and may be particularly relevant for urban men who have sex with men (MSM). The current study of low-income mostly Black and Latino MSM aims to investigate whether the cumulative ACE score predicts self-reported drug use in a dose-respons

Polysubstance Use Among Patients Treated With Buprenorphine From a National Urine Drug Test Database.

IMPORTANCE: Polysubstance use is a concern for patients treated for opioid use disorder (OUD). While buprenorphine can curtail harmful opioid use, co-occurring use of nonprescribed substances, such as cocaine, methamphetamine, and other opioids, may negatively affect treatment outcomes. OBJECTIVE: To characterize factors associated with urine drug positivity for nonprescribed substances among patients prescribed buprenorphine. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study included patients who had been prescribed buprenorphine and who provided urine specimens for urine drug t

4-Fluoromethamphetamine (4-FMA) induces in vitro hepatotoxicity mediated by CYP2E1, CYP2D6, and CYP3A4 metabolism.

4-Fluoromethamphetamine (4-FMA) is an amphetamine-like psychoactive substance with recognized entactogenic and stimulant effects, but hitherto unclear toxicological mechanisms. Taking into consideration that the vast majority of 4-FMA users consume this substance through oral route, the liver is expected to be highly exposed. The aim of this work was to determine the hepatotoxic potential of 4-FMA using in vitro hepatocellular models: primary rat hepatocytes (PRH), human hepatoma cell lines HepaRG and HepG2, and resorting to concentrations ranging from 37 μM to 30 mM, during a 24-h exposure. Concentrations of drugs determined in blood samples collected from suspected drugged drivers in England and Wales.

This communication reports the blood concentrations of alcohol and drugs from 376 cases of alleged driving under the influence of drugs analysed at the Forensic Science Service Chorley and London laboratories between February 2010 and March 2011. The samples were analysed for alcohol, amphetamine, benzodiazepines, cocaine, MDMA, opiates, γ-hydroxybutyrate (GHB), ketamine, methadone and methylmethcathinone (the 4-isomer of which is known as mephedrone). The results were interpreted with respect to the number and type of drugs of abuse detected and the concentrations measured. Alcohol was quanti

Naloxone Use, 911 Calls, and Emergency Visits After Nonfatal Overdose.

IMPORTANCE: People may not call 911 or visit the emergency department (ED) after a nonfatal overdose (NFOD), particularly when a layperson has already administered naloxone. Understanding service-engagement patterns can improve postoverdose care. OBJECTIVE: To examine service use following an NFOD. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study of a telephone survey included adults who used cocaine, opioids, or methamphetamines in the prior 12 months. Respondents were recruited from harm-reduction, treatment, and social services organizations in New Jersey, Wisconsin, Michigan

Mental Telehealth Utilization Patterns Among High School Students from Racial and Ethnic Minority Backgrounds Affected by Violence and Substance Use.

BACKGROUND: Recent data show high school students from racial and ethnic minority (REM) backgrounds in the United States confront a twofold challenge, marked by the highest rates of firearm-related homicides since 1994 and increased youth substance use. The pandemic increased online and telehealth usage opportunities for at-risk REM youth. Therefore, this study investigated (1) the frequency and prevalence of co-occurring youth violence and substance use among REM adolescents, (2) racial/ethnic, age, and natal sex (as gender data was not collected) differences in patterns and trends in co-occu

Pharmacist-led HIV and hepatitis C point-of-care testing and risk mitigation counseling in individuals experiencing homelessness.

INTRODUCTION: Over half a million people experience homelessness on any given night in the United States. As a result of increased exposure to disease, violence, stigma, substance misuse and limited accessibility to health services, individuals experiencing homelessness are disproportionately affected by communicable diseases such as HIV and HCV with prevalence estimates as high as 21% and 36%, respectively. Pharmacists, being some of the most trusted and accessible healthcare providers, are in a unique position to have a significant impact on the nationwide initiatives in Ending the HIV Epide

Optimization of cloned enzyme donor immunoassay cut-offs for drugs of abuse in whole blood of drivers involved in road accidents.

INTRODUCTION: Immunoassay (IA) tests should be able to detect low concentrations of illegal drugs when used for the screening of drugs in drivers. False negatives should be avoided, and false positives should be reduced as far as possible. In this study, semi-quantitative results for blood samples containing illicit drugs (cannabinoids, cocaine, amphetamines/methamphetamines, opiates and methadone) obtained with cloned enzyme donor immunoassay (CEDIA), were compared with results of confirmatory analysis performed through gas chromatography-mass spectrometry (GC-MS). Fatal cyclist crashes in Australia.

OBJECTIVES: The number of casualty road crashes in Australia has steadily reduced over the past few decades; however, a concurrent reduction has not been achieved for crashes involving cyclists. This has resulted in a disproportionate overrepresentation of cyclists in fatal injury statistics. This article explores the contributing factors and injury mechanisms among coronial reported fatal cyclist crashes in Australia. METHODS: The National Coronial Information System (NCIS) is a remote data entry and retrieval system containing detailed coronial information managed by the Victorian Departmen

Decreased cerebral cortical serotonin transporter binding in ecstasy users: a positron emission tomography/[(11)C]DASB and structural brain imaging study.

Animal data indicate that the recreational drug ecstasy (3,4-methylenedioxymethamphetamine) can damage brain serotonin neurons. However, human neuroimaging measurements of serotonin transporter binding, a serotonin neuron marker, remain contradictory, especially regarding brain areas affected; and the possibility that structural brain differences might account for serotonin transporter binding changes has not been explored. We measured brain serotonin transporter binding using [(11)C] N,N-dimethyl-2-(2-amino-4-cyanophenylthio) benzylamine in 50 control subjects and in 49 chronic (mean 4 years)

Ian: a 7-year old with prenatal drug exposure and early exposure to family violence.

A 7(1/2)-year-old boy is brought to a new primary care pediatrician because his grandparents, who have legal custody, want a "fresh look" at his behavior. Ian's grandmother begins the history with the comment, "He started out kind of rough." He was exposed to methamphetamine and marijuana throughout gestation and his mother had bipolar disease and hypertension. A Cesarean section for failure to progress was followed by normal Apgar scores and an unremarkable neonatal course. Ian's parents physically fought during the first 6 months of his life; at that time, the parents separated and the grand

Hair analysis for drugs of abuse. XIX. Determination of ephedrine and its homologs in rat hair and human hair.

A sensitive GC-MS method was developed for the quantitative analysis of ephedrine (EP), phenylpropanolamine (PPA) and methylephedrine (ME) in animal and human hair. After washing with 0.1% sodium dodecyl sulfate, hair samples (10 mg) were added with deuterated internal standards, extracted by 1-h sonication and over night soaking in 2 ml of 5 M HCl-methanol (1:20) at room temperature. Following evaporation of the liquid phase, the residue was dissolved in phosphate buffer solution (pH 6.0) and purified using a solid-phase extraction procedure with Bond Elut Certify columns. Reinforcing effects of synthetic cathinones in rhesus monkeys: Dose-response and behavioral economic analyses.

The abuse of synthetic cathinones ("bath salts") with psychomotor stimulant and/or entactogenic properties emerged as a public health concern when they were introduced as "legal" alternatives to drugs of abuse such as cocaine or MDMA. In this study, experiments were conducted in nonhuman primates to examine how differences in transporter selectivity might impact the reinforcing effects of synthetic cathinones. Rhesus monkeys (N = 5) were trained to respond for intravenous injections under a fixed-ratio (FR) 30, timeout 60-s schedule of reinforcement. The reinforcing effects of selected cathino

Walking the line: stimulant use during sex and HIV risk behavior among Black urban MSM.

BACKGROUND: Although the association of stimulant use to sexual risk taking and HIV transmission has been well documented among white gay men, stimulant use during sex continues to be under-explored among Black men who have sex with men (MSM). METHODS: Black MSM (n=197) recruited via modified respondent-driven sampling between January and July 2008 completed an interviewer-administered quantitative assessment and optional HIV counseling and testing. Bivariate logistic regression procedures were employed to examine the association of demographics, sexual risk, and other psychosocial factors wi

The Alarming Surge of Driving Under the Influence-Related Motor Vehicle Crashes.

INTRODUCTION: The use of alcohol and illicit substances is increasing in the United States. However, it is not clear what proportion of motor vehicle crashes (MVCs) are related to driving under the influence (DUI) of alcohol or drugs. The aim of this study was to assess the prevalence and trends of positive drug and alcohol tests among drivers of MVCs admitted to trauma centers across the United States over the years. METHODS: This is a retrospective analysis of the American College of Surgeons-Trauma Quality Improvement Program database over 4 years (2017-2020). Using International Classific

Treating Benzodiazepine Withdrawal in a Bridge Clinic.

BACKGROUND: Benzodiazepine-involved overdose deaths are rising, driven by increasing use of nonprescribed benzodiazepine pills. For patients who wish to stop nonprescribed benzodiazepine use, rapid inpatient tapers are typically the only option to treat benzodiazepine withdrawal. Substance use disorder bridge clinics can provide the high-touch care needed to manage outpatient benzodiazepine tapers in patients at high risk due to other substance use disorders. OBJECTIVE: Describe the implementation and short-term outcomes of an outpatient benzodiazepine taper protocol to treat benzodiazepine w
