The antibiotic susceptibility pattern of gas gangrene-forming Clostridium spp. clinical isolates from South-Eastern Hungary.

Introduction:Clostridium perfringens and other gas gangrene-forming clostridia are commensals of the human gut and vaginal microbiota, but can cause serious or even fatal infections. As there are relatively few published studies on antibiotic susceptibility of these bacteria, we decided to perform a 10-year retrospective study in a South-Eastern Hungarian clinical centre. Methods: A total of 372 gas gangrene-forming Clostridium spp. were isolated from clinically relevant samples and identified with rapid ID 32A (bioMérieux, France) and MALDI-TOF MS (Bruker Daltinics, Germany) methods. Antibioti

Prevalence of extended-spectrum β-lactamase and carbapenemase-producing Klebsiella pneumoniae in humans, animals and the environment in Northwest Amhara, Ethiopia.

Extended-spectrum β-lactamase-producing K. pneumoniae (ESBL-PKP) and carbapenemase-producing K. pneumoniae (CPKP) are major causes of life-threatening infections in humans and animals. In 2019, the bacterium was responsible for 100,000 to 200,000 deaths globally attributable to antimicrobial resistance (AMR). Ethiopia, home to the largest livestock population in Africa, is characterized by close human-animal interaction and common practices of raw milk and meat consumption that favor the transmission of resistant bacteria. Despite these risks, the epidemiology of ESBL-PKP and CPKP has not been

The efficacy and safety of omadacycline plus β-lactam versus moxifloxacin plus β-lactam for the treatment of severe community-acquired pneumonia: study protocol of a multicenter, prospective, randomized controlled non-inferiority trial.

BACKGROUND: Severe community-acquired pneumonia (sCAP) is associated with high morbidity and mortality. Current empirical antimicrobial regimens are increasingly challenged by rising antimicrobial resistance and safety concerns (e.g., fluoroquinolone-related adverse events). Omadacycline, a novel aminomethylcycline antibiotic, has demonstrated broad-spectrum activity against common respiratory pathogens (including drug-resistant strains) and a favorable safety profile. However, direct comparative evidence to support its use in combination with β-lactams in this setting is lacking. Therefore, t

Oral treatment of Whipple's disease with doxycycline and hydroxychloroquine versus intravenous therapy with ceftriaxone followed by oral trimethoprim-sulfamethoxazole in Germany: a phase 2/3, prospective, open-label, randomised, controlled, non-inferiority trial.

BACKGROUND: Previous studies have shown that intravenous ceftriaxone or meropenem for 14 days, followed by oral trimethoprim-sulfamethoxazole for 1 year, cures 98% of people with Whipple's disease. However, intravenous therapy requires hospitalisation and carries risks for treatment-associated complications. The aim of this study was to investigate whether oral-only treatment for Whipple's disease is non-inferior to intravenous therapy. METHODS: This phase 2/3, prospective, open-label, randomised, controlled, non-inferiority trial enrolled individuals aged 18 years or older with confirmed Whi

Investigation of Plasmid-Mediated Colistin Resistance Genes (mcr-1-8) in Enterobacterales Isolates.

Background The escalating global rise in multidrug-resistant gram-negative bacteria presents an increasingly substantial threat to patient safety. Over the past decade, carbapenem-resistant Enterobacterales (CRE) have emerged as one of the most critical pathogens in hospital-acquired infections, notably within intensive care units. Colistin has become one of the last-resort antimicrobial agents utilized to combat infections caused by CRE. However, the use of colistin has been accompanied by a notable increase in the prevalence of colistin-resistant bacteria. This study aimed to investigate pla

Prevalence of Bacteria and Antimicrobial Resistance Genes in Hospital Water and Surfaces.

Purpose Antimicrobial resistance (AMR) has become a worldwide environmental and public health problem, causing more than 250,000 deaths per year. Unregulated usage, unsafe hospital practices, and misuse in veterinary contribute to the development of multidrug resistance in various bacteria. Hospital water was hypothesized to be a hotspot for AMR transmission because of (1) increased exposure to antibiotic load, (2) poor drainage and sanitation system, (3) interaction between environmental and clinical microbes. The purpose of the research was to assess the biodiversity and AMR in hospital tap

Strain-specific outer membrane vesicle-mediated transfer of a blaOXA-24-like plasmid in Acinetobacter baumannii restricts cross-species dissemination.

The rapid rise of multidrug-resistant Acinetobacter spp. poses a major global health threat. In Acinetobacter spp., carbapenem resistance is frequently mediated by OXA-type carbapenemases encoded by blaOXA genes, including blaOXA-24-like genes such as blaOXA-72. Outer membrane vesicles (OMVs) are nanoscale proteoliposomes secreted by Gram-negative bacteria that can facilitate plasmid transfer, yet the mechanisms limiting their dissemination remain unclear. Nationwide screening in China of 2,880 Acinetobacter clinical isolates identified 10 strains carrying blaOXA-24 and blaOXA-72 plasmids acro
