Mortality And Prognostic Factors In Patients Operated For Acute Epidural Hematoma.

BACKGROUND: Epidural hematoma is one of the most common surgical emergencies encountered in neurosurgery. This study was conducted to determine the mortality and prognostic factors in patients operated for traumatic intracranial epidural hematoma in a resource-constrained setting from a developing country. METHODS: This retrospective study was conducted in the Department of Neurosurgery at Ayub Teaching Hospital Abbottabad from 1st January 2019 to 31st Dec 2021. Inclusion and exclusion criteria were created. The medical record of 116 patients admitted and operated on for traumatic extradural

Giant Cell Arteritis: Updates and Controversies.

ABSTRACT: Giant cell arteritis (GCA) is a systemic granulomatous vasculitis affecting the medium and large-size arteries, and may present with a range of ophthalmic findings. This review will cover GCA epidemiology, pathophysiology, clinical presentation, diagnostic workup, and treatment. EPIDEMIOLOGY AND PATHOPHYSIOLOGY: GCA is commonly found in elderly patients and individuals of Scandinavian descent. Recent publications suggest it may be more common in African Americans and Hispanics than previously thought. It is very rare in Asian and Middle-Eastern populations, and there is little data

Atypical Manifestation of Direct Low-Flow Carotid-Cavernous Fistula: Case Report and Review of the Literature.

BACKGROUND: Carotid cavernous fistula (CCF) is a rare type of arteriovenous shunt that develops within the cavernous sinus (CS). Direct CCFs entail a direct communication between the cavernous internal carotid artery and the CS and are typically high-flow lesions. Most CCFs drain into the ophthalmic veins (typical venous drainage pattern), leading to the pathognomic ocular clinical triad associated with a CCF. When an obstruction of the typical venous outflow is present, the arterial pressure generated by the fistula is transmitted into the cerebral venous system via the sphenoparietal sinus,

Extended endoscopic transsphenoidal approach for tuberculum sellae meningiomas.

OBJECTIVE: Tuberculum sellae meningiomas are classically removed through several different surgical transcranial approaches, including the pterional transsylvian route. Recently, the indications for the transsphenoidal technique, traditionally proposed only for the treatment of intrasellar lesions, have been extended to include lesions located in the supra- and parasellar areas and, among them, tuberculum sellae meningiomas. We describe the surgical technique for the purely endoscopic endonasal variant of the extended transsphenoidal "low route" to tuberculum sellae meningiomas. PATIENTS AND

Owner reported prevalence of autonomic signs in epileptic dogs.

INTRODUCTION: Epilepsy is a common neurological disorder in dogs. Autonomic manifestations are commonly recognised but poorly reported in veterinary literature. This study aimed to determine owner reported prevalence and course of autonomic signs in epileptic dogs. METHODS: A survey for owners of dogs with epilepsy created with Qualtrics was circulated on Facebook between March 2025 and August 2025. RESULTS: There were 1,154 responses, with 704 included in statistical analysis. The overall frequency of autonomic signs was 697/704 dogs (99.0%). When analysed by body system, gastrointestinal s

Spontaneous Intracranial Hypotension Presenting as Cryptogenic Coma: A Case Report and Literature Review Emphasizing the Role of Advanced Myelography.

Spontaneous intracranial hypotension (SIH) is a difficult-to-diagnose condition that most commonly presents with a postural headache. In this case report we detail an instance of SIH whereby a 57-year-old right-hand-dominant male presented with chief complaint of positional headaches and was found to have a spontaneous right-sided subdural hematoma (SDH). Initially, he was neurologically intact; however, after several days he decompensated and became comatose with anisocoria. A computed tomography scan (CT) of the head showed SDH enlargement and midline shift, prompting emergent craniotomy. He

Quantitative pupillometry as a potential biomarker in early concussion assessment.

BACKGROUND: There are limited objective methods when it comes to identifying and diagnosing concussion. Pupil assessment is performed routinely as a standard-of-care following traumatic brain injury (TBI). Unlike the highly subjective and limited reliability of pupil assessment using penlights and flashlights, Quantitative pupillometry (QP) is an established, valid, and reliable method of pupillary assessment. This study aims to investigate the use of QP values in concussion evaluation. METHODS: This observational study analyzed data collected by the North Texas Concussion Registry (ConTex).

Pediatric intracranial neurenteric cyst of the oculomotor nerve: a case-based review.

BACKGROUND: Neurenteric cysts (NECs) of the central nervous system (CNS) are uncommon congenital entities arising from embryonal elements. Intracranial NECs in the pediatric population are rare. METHODS: The authors describe the presentation, radiographic imaging, and pathologic findings of an 11-year-old boy with a right oculomotor nerve NEC. A literature review was performed to identify additional cases of pediatric intracranial NECs published in the English language, over the past 30 years (1990-2020). The authors discuss the presentation, investigations, management, and prognosis of this

Tentorial meningiomas with special aspect to the tentorial fold: management, surgical technique, and outcome.

BACKGROUND: From a surgical perspective, tentorial fold (TF) meningiomas (TFM) are a unique entity of tumors. They involve the supra- and infratentorial space and often are in close contact to the cavernous sinus, cranial nerves, and the mesencephalon. Complete resection is challenging and can be hazardous. We present our experience with this rare tumor entity and demonstrate the surgical outcome related to a topographical classification. METHODS: A retrospective analysis on 21 consecutive patients (female/male ratio 17/4) with meningiomas originating from the TF, who underwent surgery betwee

[Rupture of intracranial aneurysm due to cerebral angiography (author's transl)].

UNLABELLED: The authors report a case of rupture of intracranial aneurysm by angiography which was done four hours after the subarachnoid hemorrhage. Case; A thirty-one year old male patient was brought to our outpatient's clinic by ambulance because of conscious loss and convulsive seizure on Feb. 5th, 1974. Lumbar puncture showed grossly hemorrhage in the CSF. Immediately he was hospitalized and administered anticonvulsants, hypotensive drugs, antibrinolytic agents and corticosteroid. His signs and symptoms on admission were mild headache, nausea, nuchal rigidity, anisocoria (right greater t

[A case of delayed postanoxic encephalopathy with bilateral lesions of the corpus striatum].

Delayed neurological deterioration following anoxia is known to result from carbon monoxide exposure. However, it may also occur with anoxia of other types as well. The present report describes a case of delayed postanoxic encephalopathy with bilateral striatal lesions demonstrated by magnetic resonance imaging. A 27-year-old man exhibited anoxic anoxia caused by upper airway obstruction following general anesthesia for shoulder fracture surgery. Initially he was delirious and markedly excited for one day and became apparently normal for the following three days. Then he relapsed into delayed

Total Ophthalmoplegia as an Early Manifestation of Herpes Zoster Ophthalmicus in an Elderly Patient With Asthma: A Case Report.

Herpes zoster ophthalmicus (HZO) may cause ocular surface disease, but ocular motor cranial neuropathy and total ophthalmoplegia are rare. These neurological deficits generally develop after the cutaneous eruption, making their presence at initial presentation unusual. Asthma has recently gained attention as a risk factor for herpes zoster, potentially through Th2-skewed inflammation, relative Th1-mediated antiviral immune insufficiency, and impaired cellular control of latent varicella-zoster virus (VZV). We report a 71-year-old man with poorly controlled asthma who presented with a one-day h

Myopia Control in Caucasian Children with 0.01% Atropine Eye Drops: 1-Year Follow-Up Study.

Background and Objectives: Myopia is the most widespread ocular disorder globally and its prevalence has been increasing over the past decades. Atropine eye drops stand out as the only pharmacological intervention used in clinical practice to control myopia progression. The aim of this study was to explore the effect of 0.01% atropine eye drops on myopia progression. Patients and Methods: Healthy children aged 6-12 years with cycloplegic spherical equivalent (SE) from -0.5 D to -5.0 D and astigmatism ≤1.5 D were included. Myopia progression was assessed by changes in SE and axial length (AL) o

Case report: Unilateral optic atrophy and intracranial hypertension in anti-NF155 nodopathy: a case managed with sequential efgartigimod and rituximab.

BACKGROUND: Autoimmune nodopathy (AN) associated with anti-neurofascin-155 (NF155) antibodies is a distinct disorder characterized by treatment-resistant peripheral neuropathy. Although central nervous system (CNS) involvement is theorized due to the presence of NF155 in oligodendrocytes, definitive clinical reports remain limited. CASE PRESENTATION: A 31-year-old male presented with a seven-year history of relapsing-remitting progressive sensorimotor neuropathy, tremor, and sensory ataxia. He was initially diagnosed with chronic inflammatory demyelinating polyneuropathy (CIDP) and demonstrat

Isolated Abducens Nerve Palsy Following Spinal Anesthesia.

A healthy 28-year-old lady, para 1, presented to the emergency department with persistent frontal headache, nausea, and vomiting following an emergency cesarean section four days ago. She experienced difficulties with six failed attempts of spinal anesthesia intrapartum before conversion to general anesthesia. A 25-gauge Whitacre needle was utilized for administering spinal anesthesia under a sitting position. The anesthetist noticed a loss of resistance upon needle insertion, but only a negligible amount of cerebrospinal fluid was obtained upon removing the stylet. The patient underwent an em

Munich cCT Rule for Patients with Recreational Drug and Ethanol Poisoning.

Background: Patients with recreational drug and ethanol poisoning often present with reduced consciousness, coma, or disorientation. It is often unclear if there was recent head trauma. Algorithms to perform cranial computed tomography (cCT) like the Canadian CT Head Rule (CCHR), the National Emergency X-Radiography Utilization Study Head CT Decision Instrument (NEXUS DI), or the New Orleans Criteria (NOC) exist for patients with head trauma. It is unclear whether these algorithms can be applied to this patient collective. Methods: This is a retrospective data analysis of patients admitted to
