Flexible laryngeal mask as an alternative to reinforced tracheal tube for upper chest, head and neck oncoplastic surgery.

BACKGROUND AND OBJECTIVE: The flexible laryngeal mask airway has been mostly used in spontaneous ventilated children during short procedures to avoid the risk of kinking; little information has been reported about its airway morbidity. The aim of the study was to compare this airway device with the reinforced tracheal tube in mechanically ventilated adult patients. METHODS: 120 adult patients undergoing general anaesthesia for breast, head and neck oncoplastic surgery, expected to last up to 3 h, were stratified into two airway groups: flexible laryngeal mask airway (n = 60) or reinforced tra

Ventilatory post-stimulus potentiation in patients with brain damage.

In normal humans when a brief hypoxic ventilatory stimulus is terminated abruptly by breathing 100% O2, ventilation during hyperoxia gradually declines to baseline prehypoxic levels without an undershoot. This has been interpreted as evidence of decay of short-term potentiation (STP), a mechanism located in the brainstem and not dependent upon higher center inputs. STP decay may be important in preventing periodic breathing by damping ventilatory responses to cyclic stimuli. Patients with brain damage commonly have periodic breathing that may be caused partly by impairment of STP activation. T

Respiratory depression in children at different end tidal halothane concentrations.

Respiratory motor function and timing were investigated at end tidal halothane concentrations of 1.5%, 1.0% and 0.5% before and during 4% carbon dioxide stimulation in 10 spontaneously breathing children who weighed between 10.2 and 25.2 kg, during hypospadias repair under halothane anaesthesia. Their tracheas were intubated and all received a caudal block to eliminate surgical stimulation. Pneumotachography and capnography were used and in three cases movements of ribcage and abdomen were also studied by magnetometers. Respiratory drive was evaluated by occlusion tests. Ventilation was depres

Effectiveness of Manual Ventilation in Intubated Helicopter Emergency Services-Transported Trauma Patients.

BACKGROUND: Helicopter Emergency Medical Services agencies frequently transport intubated patients to definitive care. No evidence exists to determine the type of ventilation in this population. Practice varies amongst programs from bag-valve-mask to mechanical ventilation. STUDY OBJECTIVE: Evaluate the effectiveness of bag-valve ventilation in intubated trauma patients. We hypothesized manual ventilation provides adequate support to maintain physiologic ETCO2. METHODS: From June to December 2015, twenty patients were enrolled in this prospective, observational study. Included were endotrach

Reliability of Home Sleep Apnea Test as a Diagnostic Modality in Children With Obstructive Sleep Apnea- a Systematic Review.

BACKGROUND: The incidence of pediatric sleep disordered breathing has increased in the last decade because of many factors, such as childhood obesity, metabolic syndrome, and access to better diagnostic methods. OSA peaks between 2 and 8 years in children. Early diagnosis is recommended to prevent long-term complications. While PSG is the gold standard for diagnosis, it may not be feasible in certain circumstances; hence, HSAT is now being increasingly used to screen and diagnose OSA. METHODS: This review was conducted using the methodology in the "Cochrane handbook on systematic reviews". PR

Anaesthetic management and complications of a Humboldt penguin (Spheniscus humboldti) undergoing diagnostic imaging.

BACKGROUND: The presence of a tracheal septum dividing the trachea into two makes intubation one of the main challenges of penguin anaesthesia. Differences in the length and location of the aforementioned tracheal septum have been described in some penguin species. However, to the best of the authors' knowledge, it has not been reported in Humboldt penguins (Spheniscus humboldti). Therefore, one of the aims of this publication is to report the septal position in this Humboldt penguin. Furthermore, this publication describes the anaesthetic protocol and complications encountered and discusses s

Effects of brain training on brain blood flow (The Cognition and Flow Study-CogFlowS): protocol for a feasibility randomised controlled trial of cognitive training in dementia.

INTRODUCTION: Cognitive training is an emerging non-pharmacological treatment to improve cognitive and physical function in mild cognitive impairment (MCI) and early Alzheimer's disease (AD). Abnormal brain blood flow is a key process in the development of cognitive decline. However, no studies have explored the effects of cognitive training on brain blood flow in dementia. The primary aim of this study is to assess the feasibility for a large-scale, randomised controlled trial of cognitive training in healthy older adults (HC), MCI and early AD. METHODS AND ANALYSIS: This study will recruit

[Severe hemodynamic instability during the use of isoflurane in a patient with idiopathic scoliosis: case report.].

BACKGROUND AND OBJECTIVES: Isoflurane is considered a safe inhalational anesthetic. It has a low level of biotransformation, and low hepatic and renal toxicity. In clinical concentrations, it has minimal negative inotropic effect, causes a small reduction in systemic vascular resistance, and, rarely, can cause cardiac arrhythmias. The objective of this report was to present a case of severe hemodynamic instability in a patient with idiopathic scoliosis. CASE REPORT: Male patient, 13 years old, ASA physical status I, with no prior history of allergy to medications, scheduled for surgical repai

Detection and correction of accidental oesophageal intubation during flexible lightwand-guided intubation via the intubating laryngeal mask.

In the following two-part study, we determined the efficacy of observation of the light glow to detect correct placement of the tracheal tube after lightwand-guided tracheal intubation via the intubating laryngeal mask (ILM). We also determined the efficacy of a protocol to correct oesophageal intubation in this situation. In study 1, 80 ASA 1-3, anaesthetized, paralysed patients were randomly assigned to have a tracheal tube, preloaded with a flexible lightwand, placed into either the trachea (n=40) or oesophagus (n=40) under laryngoscope guidance. A blinded observer experienced with the ligh

The effect of end-tidal carbon dioxide levels on dynamic cerebral autoregulation and clinical outcome in acute ischemic stroke: An INFOMATAS study.

INTRODUCTION: Carbon dioxide is a key determinant of cerebral blood flow and its regulatory mechanisms. Previous work has shown patients with acute ischemic stroke (AIS), including lacunar stroke syndromes (LSS), are more likely to be spontaneously hypocapnic. We aimed to estimate the prevalence of hypocapnia in AIS and determine the association between end-tidal CO2 (EtCO2) and patient characteristics, dynamic cerebral autoregulation (dCA) parameters, and modified Rankin Score (mRS). METHODS: Participating centers were identified through systematic literature search and direct invitation. Da

Early prediction of intraoperative hypotension: development and validation of the HypoBridCast hybrid deep learning model.

BACKGROUND: Intraoperative hypotension (IOH) is a frequent and clinically important complication associated with adverse postoperative outcomes. Early prediction may facilitate timely intervention, although existing models have limitations in integrating multimodal physiological data and patient-specific characteristics. METHODS: We developed Hypotension Hybrid Forecast (HypoBridCast), a hybrid architecture integrating 1D Convolutional Layer (Conv1D) and Transformer (TF) modules. The model integrates intraoperative waveform data (arterial pressure, electrocardiogram, photoplethysmogram, and c

Preventing Hypoxia in Pediatric MRI Sedation: A Randomized Controlled Trial of Prophylactic Nasopharyngeal Airway Placement.

BACKGROUND: Deep procedural sedation for magnetic resonance imaging (MRI) is associated with airway collapse, causing hypoxia. The nasopharyngeal airway (NPA) is a simple rescue device, but there is limited literature on its prophylactic use to prevent hypoxia. This study evaluates the incidence of hypoxic episodes with and without prophylactic NPA placement in children undergoing MRI under sedation. METHODS: Two hundred and sixty-two American Society of Anesthesiologists (ASA) I and II children aged 1-10 years scheduled for MRI under sedation were randomized into the NPA and control groups.

An endotracheal tube inserted to the level of the hypopharynx is an easy, safe, and effective technique for ventilation of pediatric patients: A prospective observational study.

BACKGROUND: Difficult intubation and facemask ventilation in infants and small children can lead to rapid oxygen desaturation. An endotracheal tube (ETT) with its tip positioned in the hypopharynx is readily available and bypasses the soft palate, tongue, and epiglottis of the child, thus saving time by providing safe and efficient hypopharyngeal ventilation to ensure proper oxygenation. METHODS: This prospective, observational study included 85 infants and children aged >1 month and ≤ 6 years old, undergoing elective surgery under general anesthesia with orotracheal intubation. The effective

Risk stratification for postoperative hypoxemia among bariatric surgery patients.

BACKGROUND: Patients with obesity, especially those suffering from obstructive sleep apnea (OSA), are prone to postoperative respiratory hypoxemia. The PRODIGY (prediction of opioid-induced respiratory depression in patients monitored by capnography) Score is used to predict respiratory complications that factor in sleep-disordered breathing. Data on the impact of OSA on the frequency and timing of postoperative desaturation trends after bariatric surgery are lacking. METHODS: This cohort study included 195 patients who underwent robotic-assisted bariatric surgery between June 2022 and Decemb

Effects of inhaled bronchodilators on dynamic lung mechanics during exercise in COPD: protocol for a randomised, placebo-controlled crossover trial.

BACKGROUND: Activity-related dyspnoea in moderate-severe COPD is centrally related to heightened wasted ventilation in the physiological dead space colliding with constraints to tidal expansion due to acute-on-chronic gas trapping. Both phenomena are ultimately related to small airway dysfunction. However, it remains unknown whether the positive effects of pharmacological deflation on exertional dyspnoea in COPD can be ascribed to improved small airway conductance, enhancing ventilation-perfusion matching and neuromechanical coupling. MAIN HYPOTHESES: We postulate that inhaled bronchodilators

Prediction of nocturnal ventilation by pulmonary function testing in patients with amyotrophic lateral sclerosis.

BACKGROUND: In amyotrophic lateral sclerosis (ALS) prognosis is poor due to progressive weakening of the respiratory muscles. Survival and quality of life can be improved by noninvasive ventilation (NIV), which is initially applied while sleeping. The indication for NIV is based on pulmonary function testing (PFT) and polysomnography (PSG) with capnography (tCO2). While it is desirable to predict nocturnal ventilation by waking PFT in ALS, the parameters suited for reliable predictions remain elusive. METHODS: We retrospectively analyzed parameters derived from PFT (spirometry, body plethysmo
