Clinical and Endoscopic Characteristics of Pyogenic Granuloma in the Small Intestine: A Case Series with Literature Review.

Pyogenic granuloma (PG) generally appears in the skin or oral cavity, but rarely occurs in the small intestine, where it can cause bleeding. To date, only 35 cases of small intestinal PG have been reported in the English literature. We retrospectively collected information from the clinical records of seven cases of small intestinal PG that were managed in our hospital and summarized the characteristics. Further information on the clinical characteristics was obtained from the literature. Capsule endoscopy, useful for identifying the source of hemorrhage in obscure gastrointestinal bleeding, c

[Exploration of the small intestine through single-balloon enteroscopy, experience in the Mohammed V Training Military Hospital in Rabat: about 51 cases].

Enteroscopy has become an indispensable technique for the exploration and, in particular, the treatment of small intestine lesions. It is usually performed following video capsule endoscopy of the small intestine. Three equivalent techniques exist: double balloon enteroscopy, single balloon enteroscopy and spiral enteroscopy. The purpose of this study is to describe the technical feasibility of single balloon enteroscopy as well as its tolerance, indications and the results obtained in our context. We conducted a retrospective and descriptive analysis of the records of patients undergoing sing

Endoscopic transsphenoidal drainage of a petrous apex cholesterol granuloma using a frontal sinus nasal stent: a case report with long term follow up.

BACKGROUND: Cholesterol granulomas of the petrous apex are benign cysts affecting 0.6 patients per 1 million. The pathophysiology involves cholesterol crystals and lipids that are surrounded by giant cells. The cholesterol deposits induce an inflammatory response likely from acute hemorrhage leading to the formation of a fibrous capsule. The capsule expands over time compressing nearby cranial nerves (VI-VIII), which can cause worsening symptoms. Surgical resection has been shown to provide excellent improvement in symptoms. Historically, open approaches such as the infracochlear, infralabyrin

Visible small-intestinal mucosal injury in chronic NSAID users.

BACKGROUND & AIMS: Patients who regularly take nonsteroidal anti-inflammatory drugs (NSAIDs) have an increased risk for small-intestinal mucosal ulceration and bleeding, which may present as anemia of undetermined gastrointestinal origin or protein loss. The prevalence and severity of small-intestinal lesions remains unclear. Our aim was to assess the frequency of NSAID-induced small-bowel injury among chronic NSAID users. METHODS: Ambulatory patients with various types of arthritides who took NSAIDs daily (>3 mo duration) or took either acetaminophen alone or nothing were enrolled in the stu

Personalized Acute Upper GI Bleeding Diagnostics for Patients at Highest Risk for Endoscopy: Real-World Experience of a Novel, Binary, Bedside Gastric Blood Detection Device.

Background: Upper gastrointestinal bleeding (UGIB) is a common medical emergency associated with significant morbidity, mortality, and healthcare costs. Many patients undergo early endoscopy despite the absence of active bleeding. PillSense is a novel Food and Drug Administration (FDA)-cleared ingestible capsule that rapidly detects the presence of blood in the upper GI tract and may optimize triage decisions. Methods: We conducted a retrospective study evaluating the impact of PillSense on the management of suspected UGIB in an academic center. The primary outcome was the association between

Lengthening Bowel Endoscopically: Hydrogel-Enabled Endoscopic Spring-Mediated Distraction Enterogenesis.

We evaluated the feasibility to lengthen small bowel endoscopically utilizing a spring device constrained by an engineered, slow biodegrading, hyaluronan and elastin-like protein (HELP) hydrogel as a potential treatment for short bowel syndrome. While we previously demonstrated that the HELP hydrogel could provide device confinement and enable non-invasive device removal, here we demonstrate that the HELP hydrogel can also be designed to be catheter-injectable while still being stiff enough to achieve device confinement. Six pigs underwent endoscopic spring-mediated distraction enterogenesis (

Gastrointestinal Angiodysplasia in Patients with Severe Aortic Stenosis: The Endoscopic Features of Heyde's Syndrome.

INTRODUCTION: Aortic stenosis (AS) is sometimes associated with gastrointestinal bleeding, and this phenomenon is known as Heyde's syndrome. Such bleeding is most often considered to originate from gastrointestinal angiodysplasias, but the frequency and endoscopic features of such bleeding remain unclear. This study aimed to determine the frequency and endoscopic features of gastrointestinal angiodysplasia in patients with severe AS. PATIENTS AND METHODS: In this multicenter, retrospective study, we evaluated consecutive patients who underwent transcatheter aortic valve implantation (TAVI) wi

An ambulant porcine model of acid reflux used to evaluate endoscopic gastroplasty.

BACKGROUND: There is a lack of suitable models for testing of therapeutic procedures for gastro-oesophageal reflux disease. Endoscopic sewing methods might allow the development of a new less invasive surgical approach to treatment of gastrointestinal disorders. AIMS: To develop an animal model of gastro-oesophageal reflux for testing the efficacy of a new antireflux procedure, endoscopic gastroplasty, performed at flexible endoscopy without laparotomy or laparoscopy. METHODS: At endoscopy a pH sensitive radiotelemetry capsule was sewn to the oesophageal wall, 5 cm above the lower oesophagea

Does clinical response correlate with mucosal healing in patients with Crohn's disease of the small bowel? A prospective, case-series study using wireless capsule endoscopy.

BACKGROUND: There are no studies assessing mucosal healing of the small bowel in patients with Crohn's disease (CD). Our aim was to assess the correlation between clinical response and mucosal healing of the small bowel using wireless capsule endoscopy (WCE). METHODS: This was a prospective, multicenter, case-series study. Forty patients with known or suspected CD were included, recruited in 4 tertiary referral centers of Athens. They all had an acute flare-up of their disease (CD Activity Index [CDAI] >150), involvement of the small bowel, and the nonstricturing, nonpenetrating type of the d

Using a mobile unit to deliver capsule sponge testing for Barrett's oesophagus in the community in England through GP invitation and self-referral: an implementation study.

OBJECTIVE: Offering individuals with gastro-oesophageal reflux a non-endoscopic capsule sponge test increased detection of Barrett's oesophagus (BO), a precursor to oesophageal adenocarcinoma. However, only 24% of individuals identified through general practitioner (GP) records participated. We therefore performed an implementation study to assess the feasibility of screening on a mobile unit and screening uptake among GP-invited and self-referred individuals. METHODS: In this implementation study, 3293 individuals aged ≥50 with a repeat proton pump inhibitor prescription were invited to part

Efficacy of pediatric colonoscopy used as push enteroscopy in the management of capsule endoscopy findings.

BACKGROUND: recent advances in endoscopy have enabled us to explore the small intestine more efficiently, both with capsule endoscopy and double-balloon enteroscopy. However, these methods are not available in all hospitals. Therefore, when capsule endoscopy reveals proximal jejunal lesions, a possible alternative for treatment could involve push enteroscopy combined with colonoscopy. Lesions can thus be marked for subsequent monitoring. OBJECTIVES: to describe the efficacy of pediatric colonoscopy for diagnosis, the marking of the area explored, and therapeutic options. [Flexible 'chip-on-the-tip' endoscopy for larynx diagnostics].

BACKGROUND: Laryngoscopic procedures can be performed either with rigid or with flexible endoscopes (FE). While the benefit of FEs is a better reflection of the physiological situation, rigid endoscopes (RE) usually offer better image quality. Modern flexible 'chip-on-the-tip' endoscopes promise to combine the advantages of both procedures. MATERIAL AND METHODS: A total of 38 patients with dysphonia were examined with RE, conventional FE, and"chip-on-the-tip" endoscopes (TCE). Image quality was assessed and compared and procedures were evaluated with regard to patient contentment. RESULTS: I
