Characterization of a new rat model of penetrating ballistic brain injury.

Penetrating brain injury (PBI) is a leading cause of mortality and morbidity in modern warfare and accounts for a significant number of traumatic brain injuries worldwide. Here we characterize the pathophysiology of a new rat model of PBI that simulates the large temporary cavity caused by energy dissipation from a penetrating bullet round. Male Sprague-Dawley rats (250-300 g) were subjected to a simulated ballistic wound to the right frontal hemisphere implemented by an inflatable penetrating probe. Three levels of injury severity were compared to control animals. Neurological and physiologic

An injury-induced diffuse slow potential from brain.

Three different slow potential (SP) changes resulting from focal brain injury are described. The first is an immediate, high amplitude (in excess of 25 mV)negative shift at the site of injury. The second is a biphasic negative-positive SP wave which spreads throughout the cortex ipsilateral to injury and is similar to spreading depression (SD). The third SP change, called here the injury-induced diffuse slow potential (IDSP)is a prolonged (lasting approx. 2h) negative shift occurring simultaneously in many brain areas, also in those far removed from the injured focus. The SD can be separated f

Convulsant action of naloxone in the rat amygdala.

Increasing doses of naloxone hydrochloride (100-1000 nmol) were micro-injected unilaterally into the rat amygdala and the behavioral, neuropathological and electrographic responses were studied. Microinjections of low doses of naloxone (100-250 nmol) produced staring, gustatory automatisms and wet shakes whereas higher doses additionally resulted in motor limbic seizures and status epilepticus. The electroencephalogram showed a sequence of alterations characterised by high voltage fast activity, spiking, bursts of polyspiking, electrographic seizures and postictal depression which first appear

P2X7 Receptor Activation Triggers a Neuroinflammatory Cascade Driving Migraine Pathophysiology: Implications for Glial Modulation and Symptom Management.

Migraine, a disabling neurological disorder, is increasingly recognized as being driven by neuroinflammatory processes. Purinergic receptor P2X ligand-gated ion channel 7 (P2X7R), an ATP-gated ion channel highly expressed in microglia, astrocytes, and neurons, has emerged as a pivotal regulator of these neuroimmune responses. This narrative review synthesizes the literature on P2X7R-mediated neuroinflammation and glial modulation in migraine. Evidence indicates that P2X7R activation triggers a cascade of pro-inflammatory events, including Pannexin-1 (PANX1) channel opening, NOD-like receptor f

The Potential Role of Glutamate Receptors and their Antagonists or Modulators in Migraine Therapy.

BACKGROUND: Glutamate is implicated in playing a crucial role in modulating the complex pathophysiological mechanisms of migraines, including central or peripheral sensitization, cortical spreading depression, and pain transmission. With expanding knowledge over the last three decades, glutamate receptors have become focal points in neurological drug research. Altered plasma glutamate levels during migraines suggest a potential avenue for effective therapies targeting glutamate reduction. Furthermore, glutamate is believed to play a vital role in modulating the complex pathophysiological mecha

Malnutrition/exercise interaction in the rat mother-pup dyad: a behavioral and electrophysiological analysis.

OBJECTIVES: Maternal protein malnutrition alters brain functioning, impairing fetal development. Physical exercise during gestation benefits the fetal organism from maternal adaptive changes that may be neuroprotective. This study evaluated the effect of a low-protein diet associated with maternal voluntary physical activity (VPA) on rats' behavioral and brain electrophysiological parameters in the mother-pup dyad. METHODS: Female Wistar rats (n = 40) performed VPA on running wheels during a 30-day pre-mating period. Daily running distance, activity duration, and caloric expenditure enabled u

The emerging role of epigenetic regulation in pain sensitization associated with headache disorders.

BACKGROUND: Headache disorders, including migraine, tension-type headache, and cluster headache, are among the most prevalent and disabling neurological conditions. Although genetic factors contribute to their pathogenesis, they fail to fully account for their clinical heterogeneity, episodic nature, and varied treatment responses. This review synthesizes current evidence on the role of epigenetic mechanisms—including DNA methylation, histone modifications, non-coding RNAs, and RNA modifications—in the pathophysiology of headache disorders. PRINCIPAL FINDINGS: Evidence from both human studies

Dural Immune Cells, CGRP, and Migraine.

Migraine is the most common neurological disorder in the world, affecting 12% of the population. Migraine involves the central nervous system, trigeminal nerves and meninges. Recent advances have shown that targeting calcitonin gene-related peptide (CGRP) through either antibodies or small molecule receptor antagonists is effective at reducing episodic and chronic migraine episodes, but these therapeutics are not effective in all patients. This suggests that migraine does not have a singular molecular cause but is likely due to dysregulated physiology of multiple mechanisms. An often-overlooke

Current perspectives on mitochondrial dysfunction in migraine.

Mitochondria are an autonomous organelle that plays a crucial role in the metabolic aspects of a cell. Cortical spreading depression (CSD) and fluctuations in the cerebral blood flow have for long been mechanisms underlying migraine. It is a neurovascular disorder with a unilateral manifestation of disturbing, throbbing and pulsating head pain. Migraine affects 2.6% and 21.7% of the general population and is the major cause of partial disability in the age group 15-49. Higher mutation rates, imbalance in concentration of physiologically relevant molecules and oxidative stress biomarkers have b

Stereotypical patterns of epileptiform calcium signal in hippocampal CA1, CA3, dentate gyrus and entorhinal cortex in freely moving mice.

Epilepsy is a multi-etiological brain dysfunction syndrome. Hippocampal neuronal damage induced by seizures may be one of the causes leading to cognitive impairment, but the underlying mechanism remains to be further elucidated. The kainic acid (KA) model of temporal lobe epilepsy is widely used in understanding of the epileptogenesis. Fiber photometry is a signal detection technology suitable for recording calcium activity of neurons in the deep brain of freely moving animal. Here, we used the optical fiber-based method to monitor the real-time neuronal population activities of freely moving

Continuous Monitoring of Spreading Depolarization and Cerebrovascular Autoregulation after Aneurysmal Subarachnoid Hemorrhage.

Delayed cerebral ischemia (DCI) is a prominent complication after aneurysmal subarachnoid hemorrhage (aSAH). Although vasospasm of proximal cerebral arteries has been regarded as the main cause of DCI, vasospasm of distal arteries, microthrombosis, impaired autoregulation, cortical spreading depolarization (CSD), and spreading ischemia are thought to be involved in DCI after aSAH. Here, we describe a patient with aSAH in whom CSD and cerebrovascular autoregulation were evaluated using simultaneous electrocorticography and monitoring of the pressure reactivity index (PRx) after surgical clippin

The science of migraine.

The cardinal symptom of migraine is headache pain. In this paper we review the neurobiology of this pain as it is currently understood. In recent years, we discovered that the network of neurons that sense pain signals from the dura changes rapidly during the course of a single migraine attack and that the treatment of an attack is a moving target. We found that if the pain is not stopped within 10-20 minutes after it starts, the first set of neurons in the network, those located in the trigeminal ganglion, undergo molecular changes that make them hypersensitive to the changing pressure inside

A-type K+ channels impede supralinear summation of clustered glutamatergic inputs in layer 3 neocortical pyramidal neurons.

A-type K+ channels restrain the spread of incoming signals in tufted and apical dendrites of pyramidal neurons resulting in strong compartmentalization. However, the exact subunit composition and functional significance of K+ channels expressed in small diameter proximal dendrites remain poorly understood. We focus on A-type K+ channels expressed in basal and oblique dendrites of cortical layer 3 pyramidal neurons, in ex vivo brain slices from young adult mice. Blocking putative Kv4 subunits with phrixotoxin-2 enhances depolarizing potentials elicited by uncaging RuBi-glutamate at single dendr

Case report: Diazepam in severe tetanus treatment.

The causes of death in tetanus are muscle spasms and spasm of the larynx, which are caused by blocking the release of inhibitory neurotransmitters in the spinal synapses, causing the uncontrolled spread of impulses. Diazepam controls the spasms by blocking the polysynaptic reflexes, working peripherally, without depressing the cortical center and has no cardiovascular or endocrine effects. High dose diazepam had been used and proved to be a good muscle relaxant. Diazepam seems to work better with tetanus than pancuronium bromide, but both drugs need mechanical ventilation. In cases where the d

Bridging the gap: molecular mechanisms, regional activity and connectivity in headache disorders.

Chronic headache disorders have a tremendous impact on psychosocial functioning. Despite the availability of various treatment options, suboptimal management remains present in a subset of patients, leading to persistent suffering. Molecular mechanisms, regional activity patterns and connectivity pathways are crucial for understanding the pathophysiology, serving as a foundation for developing novel treatments, refining existing therapies, and ultimately optimizing the management of headache disorders. Nevertheless, articles combining fundamental and clinical aspects of the pathophysiology and

Treatment of hemiplegic migraine with anti-calcitonin gene-related peptide monoclonal antibodies: A case series in a tertiary-care headache center.

Hemiplegic migraine (HM) is a subtype of migraine with aura that includes motor weakness; such headaches can be excruciating. The presence of not only headache but also aura symptoms of HM increase the burden on patients, and the treatment of HM is sometimes challenging. Monoclonal antibodies (mAbs) targeting the calcitonin gene-related peptide (CGRP) pathway are novel migraine preventive treatments that have shown promising efficacy in patients with migraine; however, there have been no reports regarding their efficacy in HM to date. Six patients with HM were treated with galcanezumab in a te

A comparative study on the effectiveness of topiramate and propranolol in patients with migraine with aura.

Background: Migraine is considered as one of the most common and disabling diseases of the nervous system that has a great impact on quality of life (QOL) and a little risk of neurologic complications such as stroke. Migraine aura is known to be the result of cortical spreading depression and is associated with higher risk of this complication. Thus, the present study was conducted with the aim to compare the effects of topiramate as an antiepileptic, and propranolol in patients with migraine with aura. Methods: The present randomized clinical trial was conducted on patients with migraine with

Migraine-like Visual Aura Rriggered by a Large Aneurysm in the Left Extracranial Internal Carotid Artery with Successful Prevention of Recurrence by the new Anticoagulant Dabigatran: First Case Report.

PURPOSE: We present a rare case of late-onset migraine-like visual aura triggered by a large aneurysm in the left extracranial internal carotid artery. To the best of our knowledge, this is the first case of migraine-like visual aura triggered by an extracranial internal carotid artery (ICA) aneurysm. This is also the first case of migraine-like visual aura with probable thromboembolic trigger being successfully treated with a new anticoagulant, dabigatran. CASE REPORT: A 61-year-old woman complained about stereotypical episodes of a short-lasting flickering light in the right visual field fo
