Removable orthodontic appliances: new perspectives on capabilities and efficiency.

AIM: Removable appliances are a dependable choice for many patients but like all orthodontic appliances, they have some limitations in use. Patient selection and appropriate appliance design are two key factors for success. Many patients, especially adults, prefer intra-oral appliances to extra-oral devices. Sometimes a removable intra-oral appliance can solve a dental problem in a shorter period of time compared to fixed treatment, and this has also been repeatedly seen in molar distalisation. From the interceptive perspective, the appliance can prevent or alleviate an impending crowding for

Distribution of plaque and gingivitis and associated factors in 3- to 5-year-old Brazilian children.

This cross-sectional study investigated the distribution of plaque and gingivitis and its association with demographic, socioeconomic, and orthodontic variables (spacing in anterior teeth, anterior open bite, and crossbite), and visible plaque level (low, medium, high) in Brazilian preschoolers. The sample comprised 490 3- to 5-year-old children from nursery schools in Canoas, a city in southern Brazil. One single, trained observer examined children's oral cavities and determined visible plaque index (VPI) and gingival bleeding index (GBI). Results showed that 99% of the children had visible p

Alternative treatment for open bite Class III malocclusion in a child with Williams-Beuren syndrome.

Williams-Beuren syndrome (WBS) is a rare genetic condition that affects approximately 1 in every 20,000 - 50,000 live births. WBS children have specific skeletal deformities, dental malformations and rare lingual muscle dysfunction. The need for orthodontic and orthognathic therapy has arisen and has been considered a real clinical challenge even for experienced professionals, once it requires a complex and individualized treatment plan. This study reports a case of orthopedic expansion of the maxilla, in which a modified facial mask was used for protraction of the maxillary complex associated

Esthetic and functional rehabilitation for oligodontia in the mixed dentition: case report.

Tooth agenesis can be considered the most common anomaly of craniofacial development in humans, with an estimated prevalence reaching up to 20% worldwide. Treatments for tooth agenesis include orthodontic space closure, use of fixed or removable dentures, and placement of dental implants. Some of these options may not be applicable to children in mixed dentition because they can prevent the normal development of facial bones. A multidisciplinary, staged approach considering the number, size, and development of the remaining teeth in the long term would be the most suitable treatment. The purpo

Missing upper lateral incisors: orthodontic considerations in young patients.

Missing lateral incisors are usually discovered by age 9 when general orthodontic treatment needs are becoming apparent. Whether to close, open or maintain a space for a missing lateral incisor is an orthodontic decision that needs to fit within a total plan of dental management. Factors to consider include:- Skeletal base relationship, severity of crowding, overjet, overbite, position into which the canine erupts, crown sizes, crown shapes, crown colours, gingival contours, gingival display on smiling, patient and parent attitude toward options, availability of appropriate prosthetic services

Prevalence of malocclusion and orthodontic treatment need in 10-year-old New Zealand children.

The need for orthodontic treatment was determined in a random sample of 10-year-old Dunedin, New Zealand schoolchildren using the Dental Aesthetic Index (DAI); an index based on the social acceptability of dental appearance. When absent or unerupted teeth were disregarded, orthodontic treatment was considered to be "mandatory" for one-third of the children. The remaining children were almost equally divided into the three other treatment-need groups: "highly desirable", "elective", and "not necessary". The majority of the children had crowding and spacing in at least one incisor segment, and a

Analysis of bone resorption after secondary alveolar cleft bone grafts before and after canine eruption in connection with orthodontic gap closure or prosthodontic treatment.

PURPOSE: We sought to analyze the success rate of secondary alveolar cleft bone grafts before and after canine eruption in connection with orthodontic gap closure or gap opening. PATIENTS AND METHODS: Sixty-eight secondary alveolar cleft bone grafts with iliac crest spongiosa were carried out in 57 patients (mean age, 9 years; age range, 8 to 11 years) with 11 bilateral and 46 unilateral clefts of the lip, alveolus, or palate. Gap closures were carried out after 53 bone grafts (78%), and gap openings with subsequent dental implants were carried out with 15 bone grafts (22%). The parameters ac

The use of miniscrew as orthodontic anchorage in correction of maxillary protrusion with occlusal cant, spaced arch, and midline deviation without surgery.

This report describes the use of the miniscrew as orthodontic anchorage in maxillary protrusion with a spaced arch and midline deviation in a 16-year-old female patient. In cases with midline deviation, a cant in the maxillary occlusal plane is often observed. Thus, the authors used the miniscrew to control the vertical dimension, thereby flattening the maxillary occlusal plane, and to close and retract the incisors to improve her convex profile. Effective incisor intrusion to correct the deep overbite was also observed. In addition, functional evaluation by a 6-degrees-of-freedom jaw movement

Multidisciplinary approach to a nonsyndromic oligodontia patient using advanced surgical techniques.

Treatment of a woman suffering from oligodontia and multiple diastemata with insufficient alveolar ridges in both the maxilla and mandible is described in this clinical report. The total number of missing teeth was 11 (excluding the wisdom teeth). The treatment strategy consisted of an interdisciplinary team approach of orthodontic, surgical, and prosthodontic phases. Sinus floor augmentation, alveolar ridge augmentation via vertical alveolar distraction, and lateral augmentation with ramus graft procedures were performed after fixed orthodontic treatment and prior to dental implant placement.

Risk indicators of temporomandibular disorder incidences in early adolescence.

PURPOSE: Although temporomandibular disorder (TMD) has been presumed to be a condition affecting adults, studies have reported the presence of signs and symptoms in children. The purpose of this paper was to verify the influence of gender, mother's gestational behavior, malocclusion, and oral habits as risk indicators for TMD in 12-year-old adolescents. METHODS: TMD pain was assessed by research diagnostic criteria for temporomandibular disorders (RDC/TMD; axis I). Five-hundred fifty-eight subjects (330 girls and 228 boys) underwent examinations. Bivariate analyses were performed using the ch

Age changes in orthodontic treatment need: a longitudinal study of 10- and 13-year-old children, using the Dental Aesthetic Index.

The Dental Aesthetic Index (DAI) was used to assess the prevalence of unmet orthodontic treatment need in 150 13-year-old schoolchildren in Dunedin, New Zealand, and to compare the findings with those obtained in the same children at 10 years of age. Fewer 13-year-olds (27%) had a "mandatory" need of orthodontic treatment than when they were 10 years old (33%), 20% had "no/little" need for orthodontic treatment, 33% had an "elective" need for treatment and 20% had a "desirable" need for treatment. The fall in DAI scores is attributed to over-sensitivity of the Index to mixed dentition traits.

Esthetic Smile Design: Limited Orthodontic Therapy to Position Teeth for Minimally Invasive Veneer Preparation.

The standards of dentistry are being elevated, with a greater emphasis being placed on esthetics along with functionality. Minimally invasive dentistry has become an essential component in creating restorations that are functional and have increased longevity. In the case discussed in this article, the patient underwent 9 months of orthodontic therapy to correct her improper overbite and overjet, and the spacing of her dentition so the teeth could be positioned for future minimally invasive restorations. Orthodontic therapy was paramount in positioning the teeth so that the future restorations

Localized sequential use of resilient lining to generate orthodontic force in thermoformed active removable appliances.

A new modality of orthodontic treatment based on the thermoformed appliance was developed and trialled clinically. A light-cured resilient lining material commonly used for denture relining was placed locally and sequentially in thermoformed appliances to generate orthodontic forces. The new method appeared to be effective. All the presented cases showed substantial improvement in dental alignment. A number of orthodontic movements were demonstrated. Localized use of resilient lining in thermoformed orthodontic appliances appeared to be a promising alternative to other thermoformed active remo

[Incisor repositioning: a new approach in orthodontics].

Lower incisors axis has a "key" position in different cephalometric analysis. However, several critics are directed towards the cephalometric profile and cephalometric landmarks (point, line and angle). The published norms and the cephalometric standards recommended for the optimal positioning of incisors could only be used as general clinical guidelines. Incisor repositioning to achieve optimal facial aesthetics requires taking into consideration the hard and soft tissues of the face, the profile, the muscular dynamics as well as the facial growth. In this work, we propose a new approach of i

Interdisciplinary approach for increasing the vertical dimension of occlusion in an adult patient with several missing teeth.

This case report describes the interdisciplinary treatment of a 39-year-old man with several missing teeth (including both maxillary lateral incisors), Class II malocclusion, deep overbite, and significant mandibular midline deviation. The treatment plan included placement of endosseous dental implants early in the orthodontic treatment to increase the vertical dimension of the occlusion for deepbite correction, canine substitution for the missing lateral incisors, and distalization of the mandibular right buccal segment with the aid of a temporary anchorage device.

Oral rehabilitation of a hypohidrotic ectodermal dysplasia patient: a 6-year follow-up.

This case report describes the oral rehabilitation of a female child with hypohidrotic ectodermal dysplasia over a 6-year time period. It demonstrates the need for periodic modification and replacement of a prosthesis, an orthodontic appliance, and a gingivoplasty. Although the initial treatment plan was considered to be a compromise due to limited cooperation, an improvement was observed in the patient's social behavior as a consequence of her dental treatment. The effects of unavoidable changes in the dental team over 6 years are also discussed.
