[Functional results of homodigital pedicle island flaps for finger-pulp defects. Evaluation of 15 cases].

OBJECTIVES: Fingertip amputations are very common. The aim of the treatment is to restore the sensibility of the finger pulp, with adequate pulp padding. The homodigital pedicle island flaps are used in zone 2 or 3 of Allen's classification. This study evaluates the functional results of this type of flap. METHODS: Fifteen patients were reviewed. The clinical evaluation noted complications, satisfaction level, use of the finger, cold intolerance and increased sensibility signs. The Weber test and the Semmes monofilaments were used for sensory evaluation. The joint mobility was measured and th

Results of heterodigital flexor digitorum profundus hemi-tendon transfer for 23 flexor tendon injuries in zones 1 or 2.

UNLABELLED: We report outcomes of reconstruction of zone 1 or 2 flexor tendon injuries using a heterodigital hemi-tendon transfer of the flexor digitorum profundus in 23 fingers of 23 patients. At mean follow-up of 57 months, the mean total active motion of the three finger joints including the metacarpophalangeal joint was 128 degrees preoperatively and 229 degrees at final follow up. According to Strickland criteria, the function was excellent for 14 fingers, good for seven fingers and poor for two fingers. The subgroup analysis showed that the results were better in cases of primary surgery

Posttraumatic proximal interphalangeal joint flexion contractures.

Normal motion of the proximal interphalangeal joint requires bony support, intact articular surfaces, unimpeded tendon gliding, and uncompromised integrity of the collateral ligaments and volar plate. Deficiency in any one of these structural requirements can lead to a loss of finger joint motion and decreased hand function. Once finger extension is lost, options include nonsurgical or surgical treatment. Nonsurgical treatment such as splinting or serial casting should be tried before attempting surgical intervention. When severe flexion deformity exists or the vascular status of the finger ha

A new surgical treatment for mallet finger deformity: deepithelialised pedicled skin flap technique.

INTRODUCTION: Mallet finger, well-known also as drop finger or baseball finger, is a frequent deformity after extensor tendons injury in the fingers. Although numerous nonoperative or operative techniques have been used in managing this deformity, the treatment still remains a debated subject. PATIENTS AND METHODS: Starting from 1996, 121 fingers in 118 patients with neglected deformity or unsuccessful splinting older than 10 days underwent surgical treatment. In 101 patients a tendinous mallet finger was present, and in 20 patients a bony mallet finger. After immobilising the distal interpha

The superficialis minus deformity and its operative treatment.

On removal of the superficial finger flexor the normal sequence of flexion is greatly disturbed if the proximal interphalangeal joint is hypermobile, and a zig-zag deformity results. The pathomechanics of this problem as described by Landsmeer (1958) is reviewed. A modification of Littler's lateral band re-routing operation for correction of the intrinsic plus finger deformity has been developed to correct the superficialis minus deformity. This procedure has been successful in correcting the proximal interphalangeal joint hyperextension in five cases and in preventing its development in three

Dysplasia epiphysealis hemimelica at the metacarpophalangeal joint.

Dysplasia epiphysealis hemimelica (DEH) or Trevor's disease is a rare developmental disorder of asymmetric epiphyseal cartilage overgrowth accompanied by an accessory, epiphyseal ossification center. No published reports exist of metacarpal or phalangeal DEH. We report a case of bilateral DEH of the index finger: right-sided distal index metacarpal and left-sided index proximal phalanx. Both were surgically corrected with excision of the accessory ossification center; a transepiphyseal cartilaginous wedge was excised, and the articular surface was maintained. Follow-up shows maintenance of ali

Functional assessment of ray transfer for central digital loss.

Nineteen patients who had ray transposition for central digital loss were evaluated retrospectively with regard to postoperative function and appearance. Pinch and grip recoveries were better in patients with small-to-ring finger transfers than in those with index-to-long finger transfers. Total average pinch recovery measured 83.3% of the nonoperated side. Total grip recovery was 80.2% of the nonoperated side. Average ranges of motion for all transferred digits were 77.6 degrees (metacarpophalangeal joint), 87.6 degrees (proximal interphalangeal joint), and 80.8 degrees (distal interphalangea

Salvage of failed proximal interphalangeal arthroplasty with vascularised toe joint transfer: Report of two cases.

Failed implants of the proximal interphalangeal joint may result in bone and soft tissue deficits and joint instability with limited reliable options for reconstruction besides an arthrodesis procedure. The purpose of this report is to illustrate the use of vascularized second toe joint for salvage of failed, multi-operated proximal interphalangeal joint in two active patients. Pre-existing scars are used to define the approach and choice of donor site. Flow through anastomosis was performed on the finger digital artery. At final follow-up, the transplanted joints achieved 80-90 degrees of mot

Osteomyelitis variolosa.

We present a patient with osteomyelitis variolosa in both elbow joints with short middle fingers-sequelae of smallpox. Both elbow joints showed sclerosis of bones around elbow joint, enlargement of olecranon process of ulna, and head of radius with intact joint space. Hands showed shortening of both middle fingers following dactylitis leading to short metacarpals. The purpose of this report is to remind those who seldom see one of the most important and easily recognized complications of smallpox.

Inside-out Volar Portal of Arthroscopy of Proximal Interphalangeal Joints: A Cadaver Study.

PURPOSE: The application of arthroscopy in the proximal interphalangeal joints can avoid scar development and joint contracture. However, conventional arthroscopic techniques are difficult to apply to the palmar side of the joint. This study aimed to explore the feasibility and safety of the inside-out volar portal arthroscopic technique for the proximal interphalangeal joint, elucidating a new portal technique for the inspection and treatment of small joints. METHODS: We performed the inside-out palmar portal arthroscopic technique on 10 cadavers. The distances between portals and important

Change in the collateral and accessory collateral ligament lengths of the proximal interphalangeal joint using cadaveric model three-dimensional laser scanning.

UNLABELLED: The purpose of this study was to assess the lengths of the index and middle finger proximal interphalangeal joint ligaments and determine the relative changes in the collateral and accessory collateral ligament lengths at 0°, 45° and 90° flexion. We generated three-dimensional scans of 16 finger (eight index and eight middle) proximal interphalangeal joints to assess relative changes in ligament length. Significant changes were found between 45°-90° and 0°-90° for the ulnar collateral ligament of the index finger and both collateral ligaments of the middle finger between 45°-90° an

Radiographic signs in the temporomandibular and hand joints in patients with psoriatic arthritis.

The temporomandibular (TMJ) and hand joints of 64 patients with psoriatic arthritis (PA) were examined radiographically with panoramic tomography and dorsovolar projection. The associations between radiographic signs in the condyle of the TMJ and finger joints were analyzed with Pearson's product-moment correlation coefficient. Twenty-six patients (41%) had radiographic signs in their TMJs. Arthritic signs were seen in 14 (22%) and degenerative signs in 12 (19%) patients. Fifty-five patients (86%) had radiographic signs in their hands; 33 patients (52%) had arthritic and 22 (34%) degenerative

The neglected rupture of deep transverse metacarpal ligament.

The deep transverse metacarpal ligament (DTML) extends in radio-ulnar direction between the palmar plates of the metacarpophalangeal (MCP) joints of the second through fifth finger rays. On the radial aspect of the index and the ulnar aspect of the small fingers, the ligament merges with the collateral ligament of the MCP joints. The ligament has palmar grooves for the flexor tendons at the MCP joints and act as a support for the metacarpal arch. Closed DTML injury is an unusual case, but the physical findings of DTML injury are a decrease in grip strength, deviation of the finger in flexion a

Tendon displacements during voluntary and involuntary finger movements.

In the human hand, independent movement control of individual fingers is limited. One potential cause for this is mechanical connections between the tendons and muscle bellies corresponding to the different fingers. The aim of this study was to determine the tendon displacement of the flexor digitorum superficialis (FDS) of both the instructed and the neighboring, non-instructed fingers during single finger flexion movements. In nine healthy subjects (age 22-29 years), instructed and non-instructed FDS finger tendon displacement of the index, middle and ring finger was measured using 2D ultras

Clinical outcomes of unstable metacarpal and phalangeal fractures treated with a locking plate system: a prospective study.

We prospectively assessed clinical and radiological outcomes of locking plate fixation in treating unstable fractures in 11 metacarpals, 15 proximal phalanges, and eight middle phalanges in 34 consecutive patients from October 2011 to December 2016. Median length of follow-up was 14 months (range 12-24). The motion of finger joints, bony union, and complication rates were recorded. The median postoperative range of motion of the two interphalangeal joints and the metacarpophalangeal joint was 82% of the contralateral hands. Fractures in the three locations had significantly different recovery

Can collagenase effectiveness in Dupuytren's contracture be improved by using ultrasound-guided Injection? A comparative study.

The objective of this study was to compare the effectiveness of ultrasound-guided injection of collagenase Clostridium histolyticum (CCH) in patients with Dupuytren's contracture (DC), with the standard injection. We hypothesised that the ultrasound-guided Injection of CCH is more effective than the standard injection. A prospective cohorts study in patients with DC was done. We treated consecutively 47 fingers with the standard injection and 43 with the ultrasound-guided. Patients in both groups had the same inclusion criteria. The degrees of contracture of the metacarpophalangeal (MCP) and p

A Modified Pull-out Wire Technique for Acute Mallet Fracture of the Finger.

BACKGROUND: A variety of surgical techniques for treating mallet fracture finger has been reported with different outcomes and complications. However, the optimal procedure remains controversial. This study describes surgical outcomes of mallet fractures of the finger with distal phalanx treated by modified pull-out wire fixation with Kirschner wire (K-wire) stabilization of the DIP joint in hyperextension. MATERIALS AND METHODS: 30 patients who had mallet fracture finger injuries (Doyle's classification type IVC) with DIP joint subluxation between January 2009 and January 2015 were included

Morphologic, dynamic and high-resolution microscopy MRI in early-onset spondyloarthritis finger dactylitis.

OBJECTIVE: Up to now, the pathophysiology of SpA dactylitis has not been entirely clarified. It is not clear which are the involved tissues and which is the primary lesion of the "sausage-like" digit. The aim of our study was to examine the finger structures in early-onset finger dactylitis using high-resolution microscopy MRI together with morphologic and dynamic MRI. SUBJECTS AND METHODS: In a 6-month period, 13 SpA patients (7 females and 6 males), mean age 54.07 years (range 37-73 years) and mean disease duration 7.07 years (range 1-44 years) with early-onset finger dactylitis (less than
