The role of sonographic phenotyping in delivering an efficient noninvasive prenatal diagnosis service for FGFR3-related skeletal dysplasias.

OBJECTIVES: To evaluate the diagnostic yield of noninvasive prenatal diagnosis (NIPD) for FGFR3-related skeletal dysplasias and assess the accuracy of referrals based on sonographic findings to inform guidelines for referral. METHODS: We retrospectively reviewed laboratory and referral records from 2012 to 2018 to ascertain all NIPD tests performed using our next generation sequencing panel to detect FGFR3 mutations. We calculated the diagnostic yield of the test overall and when sub-divided according to the phenotypic features identified on ultrasound before testing. Pregnancy outcomes were

Screening for major aneuploidies at 10 weeks of gestation in an unselected population: comparing the combined test to the non-invasive prenatal testing with rolling-circle replication (SAPERER study).

OBJECTIVE: Non-invasive prenatal testing (NIPT) is the most accurate method for screening major trisomies. However, its diffusion is limited by high costs, and in many countries, the first-tier combined test remains the standard of care. This study compared the performance and outcomes of the Vanadis NIPT platform at 10 weeks' gestation with the combined first-trimester test. METHODS: This prospective cohort study included unselected women undergoing both first-tier NIPT and combined tests. Positive results led to genetic counselling and invasive procedures. Diagnostic accuracy was compared.

The association between non-invasive prenatal testing and first-trimester preeclampsia screening.

OBJECTIVE: The non-invasive prenatal testing (NIPT) and preeclampsia screening test (PES) has been widely used to detect fetal chromosomal abnormalities and maternal preeclampsia during early pregnancy. Our aim was to investigate the correlation between PES parameters and NIPT aneuploidy results. MATERIALS AND METHODS: We retrospectively analyzed pregnancies that underwent both PES and NIPT. PES included multiples of the median (MoM) of PlGF, PAPP-A, MAP, and calculated risks for preeclampsia, intrauterine growth restriction (IUGR), and preterm delivery, while NIPT assessed risks for common a

Non-Invasive Determination of the Paternal Inheritance in Pregnancies at Risk for β-Thalassaemia by Analyzing Cell-Free Fetal DNA Using Targeted Next-Generation Sequencing.

Non-invasive prenatal testing (NIPT) has been widely adopted for the screening of chromosomal abnormalities; however, its adoption for monogenic disorders, such as β-thalassaemia, has proven challenging. Haemoglobinopathies are the most common monogenic disorders globally, with β-thalassaemia being particularly prevalent in Cyprus. This study introduces a non-invasive prenatal haplotyping (NIPH) assay for β-thalassaemia, utilizing cell-free DNA (cfDNA) from maternal plasma. The assay determines paternal inheritance by analyzing highly heterozygous single-nucleotide variants (SNVs) in the β-glo

Artifacts, Not Differences in Sex Development, Are the Predominant Cause of Phenotypic Sex Discordance With Prenatal Cell-Free DNA Screening: A Review of 33 Cases.

OBJECTIVE: Although fetal sex chromosome complement prediction by prenatal cell-free DNA (cfDNA) screening is highly accurate, discordance between the cfDNA-inferred chromosomal sex and phenotypic sex observed via ultrasonography or at birth, that is, "fetal sex discordance", does rarely occur. Such discordance may be caused by differences in sex development (DSD) or technical or biological artifacts. This study aims to provide insights into the proportion of fetal sex discordance cases caused by DSD versus non-DSD artifacts. METHOD: We retrospectively reviewed 122,748 cases that underwent di

Prenatal cfDNA Screening for Emanuel Syndrome and Other Unbalanced Products of Conception in Carriers of the Recurrent Balanced Translocation t(11;22): One Laboratory's Retrospective Experience.

Prenatal cell-free DNA screening (cfDNA) can identify fetal chromosome abnormalities beyond common trisomies. Emanuel syndrome (ES), caused by an unbalanced translocation between chromosomes 11 and 22, has lacked a reliable prenatal screening option for families with a carrier parent. A cohort of cases (n = 46) sent for cfDNA screening with indications and/or results related to ES was queried; diagnostic testing and pregnancy outcomes were requested and analyzed. No discordant results were reported or suspected; there were ten true positives with diagnostic confirmation, six likely concordant

Targeted antenatal anti-D prophylaxis for RhD-negative pregnant women: a systematic review.

BACKGROUND: All non-sensitized Rhesus D (RhD)-negative pregnant women in Germany receive antenatal anti-D prophylaxis without knowledge of fetal RhD status. Non-invasive prenatal testing (NIPT) of cell-free fetal DNA in maternal plasma could avoid unnecessary anti-D administration. In this paper, we systematically reviewed the evidence on the benefit of NIPT for fetal RhD status in RhD-negative pregnant women. METHODS: We systematically searched several bibliographic databases, trial registries, and other sources (up to October 2019) for controlled intervention studies investigating NIPT for

Trends in the Prevalence of Births with Chromosomal Abnormalities - Haidian District, Beijing Municipality, China, 2013-2022.

WHAT IS ALREADY KNOWN ABOUT THIS TOPIC?: The primary causes of early miscarriage and stillbirth are chromosomal abnormalities (CAs) whose prevalence has been observed to increase in recent years. WHAT IS ADDED BY THIS REPORT?: According to data received from a hospital-based birth-defect surveillance system in the Haidian District, Beijing, there was a significant increase in the prevalence of CAs along with most subtypes from 2013 to 2022. This noted increase in the reported prevalence is potentially attributable to factors such as a rise in maternal age, alongside the enhanced detection eff

Discordant congenital heart defects in monochorionic twins: Risk factors and proposed pathophysiology.

A six-fold increase in congenital heart defects (CHD) exists among monochorionic (MC) twins compared to singleton or dichorionic twin pregnancies. Though MC twins share an identical genotype, discordant phenotypes related to CHD and other malformations have been described, with reported rates of concordance for various congenital anomalies at less than 20%. Our objective was to characterize the frequency and spectrum of CHD in a contemporary cohort of MC twins, coupled with genetic and clinical variables to provide insight into risk factors and pathophysiology of discordant CHD in MC twins.
