Efficacy of Operculectomy in the Treatment of 145 Cases with Unerupted Second Molars: A Retrospective Case-Control Study.

The aim of this study is to assess whether operculectomy in patients with retained second molars eases spontaneous tooth eruption in respect to untreated controls. Two hundred and twenty-two patients with delayed eruption of at least one second molar were selected from the archives of the Department of Orthodontics, Milan, Italy. Eighty-eight patients, 40 males and 48 females (mean age 14.8 ± 1.3 years), met the inclusion criteria. Records were then divided into case and control groups. The case group consisted of patients that underwent removal of the overlaying mucosa over second molars (i.e

Rehabilitation of a Head and Neck Cancer Survivor With a Functionally and Biomechanically Challenged Dentition.

Radiation therapy for patients with head and neck cancer can have damaging effects on oral health, including reduced salivary flow. In this case report, a 58-year-old patient had previously undergone radiation therapy for stage II squamous cell carcinoma of the larynx. Symptoms of xerostomia persisted, and her dentition suffered as a result. Problems included moderate to severe loss of tooth structure; erosive pitting lesions on the cusp tips of posterior teeth; and attrition throughout her dentition, particularly in the anterior. To achieve the esthetic, functional, and biomechanical goals of

Repair of root resorption 4 and 8 weeks after application of continuous light and heavy forces on premolars for 4 weeks: a histology study.

INTRODUCTION: Repair of root resorption cavities has been studied under light microscopy, scanning electron microscopy, and transmission electron microscopy. The aim of this investigation was to demonstrate the use of microcomputed tomography (micro-CT) to assist in the identification of the region of interest for light microscopy preparation. This study also qualitatively illustrated the root resorption craters with 4 or 8 weeks of retention after 4 weeks of continuous light or heavy orthodontic force application. METHODS: Four patients who required bilateral extractions of maxillary first p

Autotransplantation of a completely developed impacted maxillary canine: A 7-year follow-up case report.

BACKGROUND: Impacted maxillary canines are likely to be extracted in adult patients once orthodontic treatment is ruled out. The missing teeth are generally replaced with dental implants. CASE DESCRIPTION: The authors report the clinical case of a 45-year-old man who was missing the maxillary left canine for 30 years. Radiographic examination revealed that the maxillary permanent left canine was palatally impacted. The tooth was transplanted into the prepared socket on the missing tooth site and secured in the desired position with a flexible wire splint. During the 7-year follow-up, the toot

Maxillary canine-to-maxillary incisor transposition.

Dental transposition is the positional interchange of two adjacent teeth. Canine transpositions are usually accompanied by other dental anomalies, such as: impaction of the incisors; missing teeth; peg-shaped lateral incisors; severe rotation or malposition of adjacent teeth; dilacerations; and malformations. Local pathologic processes, such as tumors, cysts, retained primary canines, and supernumerary teeth, might be responsible for canine transposition. The purpose of this paper was to present a rare case of maxillary canine-to-maxillary incisor transposition in an 8-year-old girl. The patie

Orthodontic management of mandibular canine-incisor transposition.

This article presents five cases of an unusual dental anomaly--transposition of mandibular canine and lateral incisor. While every one of the presented cases had its individual characteristics, there are certain common traits. For instance, deciduous canines are usually retained, and the anomaly is found concurrent with other dental problems, such as congenital absence of teeth or peg-shaped lateral incisors. Many transposition cases also have a severe arch length discrepancy, which in itself may be an etiologic factor. In two of our cases orthodontic management included extraction of the tran

[Patients with low socio-economic status: is orthodontic treatment more complex?].

INTRODUCTION: Patients from low socioeconomic backgrounds are empirically considered more difficult to treat. The objectives were to evaluate low socioeconomic status of patients as a factor associated with increased treatment need and the correlation between socioeconomic status and dysmorphosis on one hand, and cooperation level on the other. MATERIAL AND METHODS: This was a retrospective comparative monocentric observational cohort study. Included subjects were under 16 years old at the time of orthodontic braces placement and were beneficiaries of the complémentaire santé solidaire (C2S)

Incidence of Canine Impaction in Tribal Population of Jharkhand.

INTRODUCTION: The position of the canine has a key role in facial harmony, dental aesthetics, functional occlusion, and health of temperomandibular joint. Maxillary canine impaction is the second most common tooth after third molars impaction. Maxillary canines are more prone to impaction due to longer path of eruption. SETTINGS AND DESIGN: All the patients were examined in the Dental Chair light and referred to the department of Oral Radiology for Orthopantomogram. Yamamoto's classification was used to classify maxillary canine impaction. Position of mandibular canine from cement-enamel junc

Cranio-cervical posture and rapid palatal expansion therapy.

OBJECTIVE: Connections between craniocervical posture (CCP) and changes in teeth position have already been demonstrated; however, the defined pathway of relationship is still not clear. The aim of this study was to evaluate CCP modifications after rapid maxillary expansion therapy using rapid palatal expansion therapy (REP)/McNamara appliances. MATERIALS AND METHODS: A consecutive series of 35 subjects, aged between 6 and 14 years, with no prior history of orthodontic treatment, and requiring skeletal expansion of the upper arch, were selected and analyzed. All patients were treated with REP

Orthodontic Treatment for Bloch-Sulzberger Syndrome in Patient with Cleft Lip and Palate.

We performed orthodontic treatment, fitted prostheses, and provided restorative treatment in a patient with Bloch-Sulzberger syndrome and cleft lip and palate during the early mixed dentition period. We report the case after a subsequent 6-year retention phase including the period of pubertal growth. A girl aged 8 years 4 months visited our hospital with the chief complaint of crowding of the anterior teeth and anterior crossbite. She had bilateral cleft lip, alveolus, and palate; a Class II molar relationship; winging of both the maxillary bilateral central incisors; and spacing in the mandib

A retrospective overview of treatment choice and outcome in 126 cases with arrested eruption of mandibular second molars.

The purpose of the present retrospective study was to analyze treatment choice and outcome in patients with retention/impaction of the mandibular second molar. Radiographic material, from three large clinics, from 106 patients (60 males and 46 females) with 126 retained/impacted permanent mandibular second molars treated during the years 1985-2005 was evaluated for treatment choice and treatment outcome. Follow-up questionnaires were sent to dentists in cases where treatment outcome could not be determined from the radiographic material. Clinical evaluation was not possible as the patients wer

Iliac apophyseal cartilage augmentation of the deficient piriform rim and maxilla in alveolar cleft grafting.

This article presents a new approach to cleft lip and cleft palate repair, where the retained bulk of alveolar cleft grafts is augmented by simultaneous cartilage augmentation of the deficient maxilla. Nine patients who provided 10 cleft sites underwent secondary bone grafting of the clefted alveolus by this technique. This study evaluates the following: (1) the postoperative esthetic results, (2) the quantity of bone in the grafted area, and (3) the provision of bone support for unerupted teeth, for teeth adjacent to the cleft, and for orthodontic tooth movement when the secondary bone grafti

Modified Interim Mandibular Advancement (MIMA) Appliance for Symptomatic Correction of Obstructive Sleep Apnea.

Obstructive Sleep Apnea (OSA) is a chronic, progressive, multifactorial, life-threatening disorder that causes significant impact on patient's life. Patients with OSA [Apnea/Hypopnea Index (AHI)>30] who cannot tolerate Continuous Positive Airway Pressure (CPAP) therapy or are not surgical candidates may benefit from oral appliances. This paper describes interim appliance devised from existing Hawley's retainer in patients with OSA. A 38-year-old man of athletic built with history of orthodontic treatment six months back due to esthetic concerns and wearing upper Hawley's retainer, reported wit

Risk management in clinical practice. Part 7. Dento-legal aspects of orthodontic practice.

Orthodontic treatment, like any aspect of general dentistry, exposes the clinician to the risk of malpractice and litigation. While for orthodontists this issue has not been a major concern over the last century, risk management has, over the last decade, become a significant issue in orthodontic practice. The patient-orthodontist contact may be classified into pre-treatment, active treatment and post-treatment periods. Risk management issues pertinent to each time period are discussed in this article with the intention of increasing the clinician's awareness of potential problems. Armed with

Unintentionally Retained Elevator Blade Left for 8 Years Causes Mild Chronic Pain.

Fracture of surgical instruments in dental practice is not usually reported in the literature. Management involves searching for and retrieving the fragment to avoid issues such as infection, swallowing, or aspiration. Although foreign bodies may not cause symptoms for years, some can cause chronic pain. The authors describe herein the unusual case of an unintentionally retained broken elevator blade left for 8 years before being found during an orthodontic evaluation. Panoramic radiography revealed a foreign body in the alveolar bone of the right mandibular third molar that was causing mild c

Diagnosis and treatment planning for unerupted premolars.

Premolars rank third in frequency after third molars and maxillary canines in impacted or unerupted teeth. Failure to detect and analyze the problem may lead to unnecessary space loss, crowding, or collapse of the dental arch. A diagnostic scheme is presented to facilitate diagnosing and treating unerupted premolars. Important observations include: Diagnosing congenitally missing permanent teeth. Whether the condition is generalized or localized. Whether the succedaneous tooth has a viable form, eruptive potential, and viable orientation Whether the delayed eruption is due to over-retained pri

Eruption of impacted permanent upper incisors after removal of of supernumerary teeth.

In a retrospective study of 63 patients, an attempt is made to determine in how many cases retained permanent upper incisors erupt after the removal of supernumerary teeth, and what predisposing and etiological factors are of importance. It proved that the eruption of a supernumerary tooth positively contributed to the chance of a spontaneous eruption of the permanent incisors. No relationship could be demonstrated between the age of a patient from whom supernumeraries were extracted, the number of supernumeraries, the extent of root formation, marked apical displacement and the chance of spon

Hopeless tooth? Prognosis and comprehensive treatment. A case report.

A hopeless tooth from a periodontal point of view, with severe bone resorption, mobility and abnormal tooth migration, is often extracted. In advanced cases, function and esthetics are impaired, and an interdisciplinary treatment is requested. Retaining or not these teeth is based on clinician judgment. A growing body of evidence claims that prognosis has great potential to be improved in a motivated patient with good oral hygiene and regular maintenance. This case report aims to present a periodontal regenerative technique combining enamel matrix protein derivatives and a particulated xenogra
