The Evaluation of Equine Allogeneic Tenogenic Primed Mesenchymal Stem Cells in a Surgically Induced Superficial Digital Flexor Tendon Lesion Model.

Background: Tendon injuries are very common in horses and jeopardize the athletic performance, and due to the high risk of reinjury may lead to early retirement. The use of mesenchymal stem cells for the treatment of equine tendon disease is widely investigated because of their regenerative potential. The objective of this study is to investigate the safety and efficacy of equine allogeneic tenogenic primed mesenchymal stem cells (tpMSCs) for the management of tendinitis in horses. Methods: A core lesion was surgically induced in the superficial digital flexor tendon of both forelimbs of eight

Evidence for Rehabilitation Interventions After Acute Lateral Ankle Sprains in Athletes: A Scoping Review.

CONTEXT: Acute lateral ankle sprain (LAS) is a common injury in athletes and is often associated with decreased athletic performance and, if treated poorly, can result in chronic ankle issues, such as instability. Physical performance demands, such as cutting, hopping, and landing, involved with certain sport participation suggests that the rehabilitation needs of an athlete after LAS may differ from those of the general population. OBJECTIVE: To review the literature to determine the most effective rehabilitation interventions reported for athletes returning to sport after acute LAS. EVIDEN

Musculotendinous Junction Injuries of the Proximal Biceps Femoris: A Prospective Study of 64 Patients Treated Surgically.

BACKGROUND: Injuries to the hamstring complex most commonly involve the proximal musculotendinous junction of the long head of the biceps femoris (MTJ-BFlh). Nonoperative management of these injuries is associated with prolonged rehabilitation and high risk of recurrence. To our knowledge, the surgical management of acute MTJ-BFlh injuries has not been previously reported. HYPOTHESIS: Surgical repair of acute MTJ-BFlh injuries enables return to sporting activity with low risk of recurrence. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: A total of 64 patients (42 male and 22 fema

Biomechanical measures during landing and postural stability predict second anterior cruciate ligament injury after anterior cruciate ligament reconstruction and return to sport.

BACKGROUND: Athletes who return to sport participation after anterior cruciate ligament reconstruction (ACLR) have a higher risk of a second anterior cruciate ligament injury (either reinjury or contralateral injury) compared with non-anterior cruciate ligament-injured athletes. HYPOTHESES: Prospective measures of neuromuscular control and postural stability after ACLR will predict relative increased risk for a second anterior cruciate ligament injury. STUDY DESIGN: Cohort study (prognosis); Level of evidence, 2. METHODS: Fifty-six athletes underwent a prospective biomechanical screening af

Differences in kinematics of single leg squatting between anterior cruciate ligament-injured patients and healthy controls.

Seventy to eighty percent of all anterior cruciate ligament (ACL) injuries are due to non-contact injury mechanisms. It has been reported that the majority of injuries due to single leg landing come from valgus positioning of the lower leg. Preventing valgus positioning during single leg landing is expected to help reduce the number of ACL injuries. We found that many ACL-deficient patients cannot perform stable single leg squatting. Therefore, we performed 3D motion analysis of the single-legged half squat for ACL-injured patients to evaluate its significance as a risk factor for ACL injuries

Treatment selection in acute anterior cruciate ligament tears.

Traumatic hemarthrosis associated with rupture of the anterior cruciate ligament is one of the most common ligament injuries encountered in sports medicine today. Its management is difficult because of the combination of presenting pathology as well as the multitude of patient profiles encountered. Crucial to the decision on proper management of a particular patient is the accumulation of data concerning the patient's social and sporting life as well as a detailed physical examination. When doubt exists, the physician can use other tests, including arthrograms, examination under anesthesia, or

Comparative Analysis of Iliotibial Band Graft Length in Pediatric Anterior Cruciate Ligament Reconstruction: MacIntosh Versus All-Epiphyseal Femoral Drilling Technique.

BACKGROUND: Iliotibial band (ITB) grafts yield excellent outcomes for anterior cruciate ligament reconstruction (ACLR) in skeletally immature patients. Because closure of the ITB harvest interval is often not feasible, techniques that allow for shorter ITB grafts may be beneficial. HYPOTHESIS: That the all-epiphyseal femur ITB graft would be significantly shorter than the over-the-top ITB graft. STUDY DESIGN: Controlled laboratory study. METHODS: Six fresh-frozen pediatric knees (mean age, 9.7 years; range, 9-11 years) were used. Surgeons simulated the modified MacIntosh and all-epiphyseal

Efficacy of cycloid vibration therapy for arthrogenic muscle inhibition after anterior cruciate ligament reconstruction: a study protocol for a prospective, randomised controlled trial.

INTRODUCTION: Arthrogenic muscle inhibition (AMI) is a neuromuscular control disorder that occurs following joint injuries such as ligament tears, joint inflammation or postoperative conditions. It is characterised primarily by reflexive inhibition of the muscles surrounding the affected joint, substantially impeding rehabilitation. After anterior cruciate ligament reconstruction (ACLR), effective rehabilitation is essential to restore joint function, facilitate a return to preinjury activity levels and reduce the risk of reinjury. Vibration therapy may mitigate AMI by modulating somatosensory

London International Consensus and Delphi study on hamstring injuries part 2: operative management.

The key indications for surgical repair of hamstring injuries (HSIs) remain unclear in the literature due to a lack of high-level evidence and expert knowledge. The 2020 London International Hamstring Consensus meeting aimed to highlight clear surgical indications and to create a foundation for future research. A literature review was conducted followed by a modified Delphi process, with an international expert panel. Purposive sampling was used with two rounds of online questionnaires and an intermediate round involving a consensus meeting. The initial information gathering (round 1) question

ACL Reconstruction Rehabilitation: Clinical Data, Biologic Healing, and Criterion-Based Milestones to Inform a Return-to-Sport Guideline.

CONTEXT: Anterior cruciate ligament (ACL) reconstruction (ACLR) and postoperative rehabilitation continues to be a multidisciplinary focus in both research and clinical environments. Recent research on ACLR warrants a reexamination of clinicians' current rehabilitation practices to optimize the strikingly variable clinical outcomes after ACLR and return to sport. The purpose of the article and updated guidelines is to use contemporary evidence to systematically revisit our practice guidelines and validate our clinical milestones with data from our university-based practice. Single-Joint and Whole-Body Movement Changes in Anterior Cruciate Ligament Athletes Returning to Sport.

INTRODUCTION: Athletes returning to sport after anterior cruciate ligament reconstruction (ACLR) demonstrate prolonged changes in landing kinematics, kinetics, and muscle activation, predisposing them for reinjury, knee osteoarthritis, and/or knee instability. So far, researchers have been focusing on how kinematics and kinetics change in every joint separately. However, as the human body operates within a kinetic chain, we will assess whether single-joint changes are associated with whole-body changes. METHODS: Twenty-one athletes who had an ACLR and 21 uninjured controls performed five unil
