Climate variability, poverty and substance use disorders: Exploring maladaptive coping mechanisms in climate-stressed communities of Northwest Nigeria.

BACKGROUND: Northwest Nigeria is highly vulnerable to climate variability and heavily dependent on rain-fed agriculture, which exacerbates existing socioeconomic vulnerabilities. AIM: This study aimed to explore the link among climate-induced stress, poverty, and mental health outcomes, particularly substance use as a maladaptive coping mechanism, in this region. SETTING: The study was conducted in Kano, Kaduna, and Katsina in Northwest Nigeria. METHODS: A cross-sectional study was conducted among 20 000 adults aged ≥ 18 years from 40 randomly selected communities. Data were collected using

Single-Cell Immune Atlases to Map Small Extracellular Vesicle Cargo in Tuberculosis-Diabetes Comorbidity: A Narrative Review and Conceptual Roadmap.

Tuberculosis-diabetes mellitus (TB-DM) is increasingly recognized as a syndemic in which chronic metabolic dysregulation amplifies tuberculosis severity, delays treatment response, and increases relapse and mortality. However, conventional systemic correlates soluble cytokines and bulk whole-blood transcriptomic signatures often appear broadly similar between TB and TB-DM. This highlights a key gap: clinically meaningful immune dysfunction in TB-DM likely resides in specific lung and blood cell states that are poorly resolved by bulk assays. Small extracellular vesicles (EVs) in plasma and bro

EAT-Lancet Recommendations and Their Viability in Chile (2014-2023): A Decade-Long Cost Comparison Between a Healthy and Sustainable Basket and the Basic Food Basket.

BACKGROUND/OBJECTIVES: Addressing the global syndemic of obesity, undernutrition, and climate change requires a shift toward healthy and sustainable diets. This study examines the feasibility and cost implications of implementing a Healthy and Sustainable Basic Food Basket in Chile that aligns with the EAT-Lancet diet recommendations, through its comparison with the current Basic Food Basket used for the poverty line definition. METHODS: The Healthy and Sustainable Basic Food Basket was constructed based on the EAT-Lancet dietary model and was uniquely adapted to reflect the observed consumpt

Socioeconomic status correlates with COVID-19 vaccination coverage among primary and secondary students in the most populated city of Chile.

The burden of COVID-19 was heterogeneous, indicating that the effects of this disease are synergistic with both other non-communicable diseases and socioeconomic status (SES), highlighting its syndemic character. While the appearance of vaccines moderated the pandemic effects, their coverage was heterogeneous too, both when comparing different countries, and when comparing different populations within countries. Of note, once again SES appears to be a correlated factor. We analyzed publicly available data detailing the percentage of school-aged, vaccinated children in different municipalities

Prevalence of obesity, hypertension and diabetes among people living with HIV in South Africa: a systematic review and meta-analysis.

BACKGROUND: HIV has become a manageable chronic condition due to the success and scale-up of antiretroviral therapy (ART). Globally, South Africa has the highest number of people living with HIV (PLHIV) and research evidence indicates that countries with the highest burden of PLHIV have a substantial burden of obesity, hypertension (HPT) and type 2 diabetes (T2D). We sought to summarize the burden of these three common NCDs among PLHIV in South Africa. METHODS: In this systematic review, multiple databases were searched for articles reporting on the prevalence of obesity, HPT, and T2D among P

Within-prison drug injection among HIV-infected Ukrainian prisoners: prevalence and correlates of an extremely high-risk behaviour.

BACKGROUND: In Ukraine, HIV-infection, injection drug use, and incarceration are syndemic; however, few services are available to incarcerated people who inject drugs (PWIDs). While data are limited internationally, within-prison drug injection (WP-DI) appears widespread and may pose significant challenges in countries like Ukraine, where PWIDs contribute heavily to HIV incidence. To date, WP-DI has not been specifically examined among HIV-infected prisoners, the only persons that can transmit HIV. METHODS: A convenience sample of 97 HIV-infected adults recently released from prison within 1-

Epidemiology of hepatitis C in Alberta, Saskatchewan, and Manitoba compared to Canada, 1980-2023.

BACKGROUND: Canada aims to eliminate hepatitis C (HCV) by 2030. While national rates have dropped, regional disparities persist. In 2021, Manitoba had the highest new HCV rates, and Saskatchewan's rates were double the national average. This study aimed to describe the incidence and risk factors associated with HCV infections in Alberta, Saskatchewan, and Manitoba, compared to Canada,1980-2023. METHODS: This ecological study used publicly available government reports from Alberta, Saskatchewan, Manitoba, and Canada (1980-2023) on HCV cases and rates by year and disaggregated by sex (female/ma

Availability of Screening for Blood-Borne Viruses, Sexually Transmitted Infections, and HIV Pre-Exposure Prophylaxis at US Substance Use Treatment Facilities Treating Stimulant Use Disorder.

OBJECTIVE: Stimulant misuse is associated with greater incidence of HIV, Hepatitis C virus (HCV), and sexually transmitted infections (STIs). Substance use treatment facilities are an important site of care for people with stimulant use disorder (StUD). Accessible testing for HIV, HCV, and STIs, and HIV treatment, pre-exposure prophylaxis (PrEP), and HCV treatment are essential for controlling these syndemics. METHODS: We conducted a cross-sectional analysis of the 2024 National Substance Use and Mental Health Services Survey (N-SUMHSS) to describe availability of HIV, HCV, and STI testing, a

Prevalence and Correlates of Sexual Violence Experienced by Young Adult Black Men Who Have Sex with Men.

Men who have sex with men (MSM) experience high prevalence of sexual violence (SV), and SV has well-documented effects on health. Research gaps are especially evident for young Black MSM (YBMSM), who experience significant HIV disparities and syndemics, including multiple forms of violence victimization. We examined lifetime prevalence of SV (having been forced or frightened into sexual activity) in a cross-sectional sample of YBMSM (N = 1732), and tested associations of demographic, psychosocial, and structural factors using multivariable regression. YBMSM were recruited between 2013 and 2015

Prevalence of Tuberculosis in HIV-positive Prisoners: A Systematic Review and Meta-analysis.

UNLABELLED: Tuberculosis (TB) and HIV constitute the main burden of infectious diseases worldwide. Imprisonment is an important risk factor for contracting TB, especially among those living with HIV. This systematic review summarizes the available data on the prevalence of TB among HIV-positive prisoners; which may support improved targeted TB/HIV prevention plans. We electronically examined published studies up to December 2017 with the aim of finding articles that investigated the prevalence of TB in HIV-positive prisoners. Methadone treatment improves tuberculosis treatment among hospitalized opioid dependent patients in Ukraine.

BACKGROUND: Ukraine's volatile syndemics of tuberculosis (TB) and HIV among people who inject drugs (PWIDs) introduces numerous treatment challenges for each condition, including high mortality and development of multi-drug resistant TB (MDR-TB). METHODS: A prospective, non-randomized 90-day observational study was conducted in six Ukrainian TB treatment sites to assess the effectiveness of integrating methadone maintenance (MMT) with TB treatment using: (1) 90-day TB treatment retention; (2) time to treatment discontinuation; (3) TB medication adherence; and (4) subject disposition, includin

The impact of the Covid-19 pandemic on the incidence of diseases and the provision of primary care: A registry-based study.

INTRODUCTION: The Covid-19 pandemic had a tremendous impact on healthcare but uncertainty remains about the extent to which primary care provision was affected. Therefore, this paper aims to assess the impact on primary care provision and the evolution of the incidence of disease during the first year of the Covid-19 pandemic in Flanders (Belgium). METHODS: Care provision was defined as the number of new entries added to a patient's medical history. Pre-pandemic care provision (February 1, 2018-January 31, 2020) was compared with care provision during the pandemic (February 1, 2020-January 31

Chemsex among gay, bisexual, and other men who have sex with men in Singapore and the challenges ahead: A qualitative study.

BACKGROUND: Sexualised substance use, or 'chemsex' has been shown to be a major factor driving the syndemic of HIV/AIDS in communities of gay, bisexual, and other men who have sex with men (GBMSM) around the world. However, there is a paucity of research on chemsex among GBMSM in Singapore due to punitive drug laws and the criminalisation of sexual behaviour between men. This qualitative descriptive study is the first to explore perceptions towards, motivators to engaging in, and the barriers to addressing the harms associated with chemsex among GBMSM in Singapore. METHODS: We conducted 30 se

From individual coping to collective resilience: reframing mental health responses to global crises through geopsychiatry.

Global crises are reshaping the conditions under which mental health is produced, threatened, protected, and restored. Pandemics, climate change, armed conflict, forced migration, political instability, economic insecurity, disasters, and digital disruption do not affect mental health only through individual stress reactions or discrete traumatic exposures. They also transform the social, territorial, institutional, ecological, and symbolic environments in which people interpret threat, seek support, access care, maintain hope, and imagine the future. This Conceptual Analysis reframes mental h

Coproduction of Low-Barrier Hepatitis C Virus and HIV Care for People Who Use Drugs in a Rural Community: Brief Qualitative Report.

BACKGROUND: People who inject drugs are experiencing syndemic conditions with increasing risk of infection with hepatitis C (HCV) and HIV. However, rates of accessing HCV and HIV testing and treatment among people who inject drugs are low for various reasons, including the criminalization of drug use, which leads to a focus on treating drug use rather than caring for drug users. For many people who inject drugs, health care becomes a form of structural violence, resulting in traumatic experiences, fear of police violence, unmet needs, and avoidance of medical care. There is a clear need for no

An Evaluation of Local Implementing Partner Performance During the First 2 Years of the USAID/PEPFAR Transition.

INTRODUCTION: Locally led and owned development is considered the best practice for international aid. As an implementing agency for the U.S. President's Emergency Plan for AIDS Relief (PEPFAR), the U.S. Agency for International Development (USAID) supported the goal of transitioning 70% of its portfolio funding directly to local organizations by 2020, including partner country governments. However, limited evidence or evaluation exists on how such a transition can help achieve HIV-related health outcomes. METHODS: We evaluated monitoring, evaluation, and reporting performance; calculated ind

Global prevalence of hepatitis B or hepatitis C infection among patients with tuberculosis disease: systematic review and meta-analysis.

BACKGROUND: There is a substantial overlap in the epidemiology of chronic hepatitis B (HBV), hepatitis C (HCV) and tuberculosis (TB) due to overlapping risk factors. Testing for viral hepatitis is not widely recommended for patients with TB. The aim of this systematic review was to evaluate the global prevalence of chronic viral hepatitis infection among patients with TB. METHODS: MEDLINE, EMBASE, Web of Science, Cochrane Library, African Journals Online, LILACS, and country TB reports were searched for studies published between January 1st, 2011 and June 17th 2021. Cervical Cancer Screening in Women Living with HIV in South Africa: Addressing Health Disparities Through Integrated Prevention Strategies.

BACKGROUND: South Africa confronts a syndemic triple burden of HIV, tuberculosis, and cervical cancer. Women living with HIV (WLHIV) carry a three- to six-fold elevated cervical cancer risk-a biological synergy reflected in persistently high mortality despite progressive screening policies. This review synthesises evidence for why policy has not achieved population-level impact and proposes an integrated prevention model anchored in existing HIV infrastructure. METHODS: Following PRISMA-ScR guidelines and the Population-Exposure-Outcome (PEO) framework (Population: WLHIV in South Africa; Expo
