End-to-side anastomosis and autologous pulmonary artery patch for aortic coarctation and hypoplastic aortic arch.

BACKGROUND: Aortic coarctation (CoA) is a congenital heart disease that usually combines various cardiovascular malformations, including hypoplastic aortic arch (HAA). We explore end-to-side anastomosis (ESA) and autologous pulmonary artery patch (APAP) in CoA with HAA, to provide more clinical experience. METHODS: This is a single-center retrospective study. The target population is patients with aortic stenosis and HAA who underwent surgery between 2015 and 2021. All cases included were consecutive, and no instances were omitted due to absent data, lack of consent, or other factors. Collect

Different approach, similar outcomes: the impact of surgical access routes in minimally invasive cardiac surgery on enhanced recovery after surgery.

OBJECTIVES: Enhanced recovery after surgery (ERAS) is a growing phenomenon in all surgical disciplines and aims to achieve a faster functional recovery after major operations. Minimally invasive cardiac surgery (MICS) therefore integrates well into core ERAS values. Surgical access routes in MICS include right anterolateral mini-thoracotomy (MT) as well as partial upper mini-sternotomy (PS). We seek to compare outcomes in these two cohorts, both of which were enrolled in an ERAS scheme. METHODS: 358 consecutive patients underwent MICS and perioperative ERAS at our institution between 01/2021

The impact of thymectomy in subgroups of Myasthenia gravis patients: a single center longitudinal observation.

BACKGROUND: Myasthenia gravis (MG) is a rare neuromuscular disorder. Symptoms can range from ptosis only to life threatening myasthenic crisis. Thymectomy is recommended for anti-acetylcholine receptor-antibody positive patients with early-onset MG. Here, we investigated prognostic factors shaping therapeutic outcomes of thymectomy to improve patient stratification. METHODS: We retrospectively collected single-center data from a specialized center for MG from all consecutive adult patients that underwent thymectomy from 01/2012 to 12/2020. We selected patients with thymoma-associated and non-

Robotically assisted mitral valve surgery-experience during the restart of a robotic program in Germany.

BACKGROUND: Following the first robotic-assisted mitral valve operations in Paris and Leipzig, the era of this innovative technique expired after a few years in Germany. At that time, the main arguments against robotic surgery within the German cardiac surgical community were low cost effectiveness and operative time utilization. Encouraged by favorable results, we re-started our robotic-assisted cardiac program as the first and only center in Germany in 2019. METHODS: Between July 2019 and December 2021, 329 patients underwent robotic-assisted operations using the daVinci Xi system, includin

Active clearance vs conventional management of chest tubes after cardiac surgery: a randomized controlled study.

BACKGROUND: Chest tubes are routinely used after cardiac surgery to evacuate shed mediastinal blood. Incomplete chest drainage due to chest tube clogging can lead to retained blood after cardiac surgery. This can include cardiac tamponade, hemothorax, bloody effusions and postoperative atrial fibrillation (POAF). Prior published non randomized studies have demonstrated that active tube clearance (ATC) of chest tubes can reduce retained blood complications prompting the ERAS Cardiac Society guidelines to recommend this modality. OBJECTIVE: A randomized prospective trial to evaluate whether an

Aortic valve-sparing root replacement with Tirone E. David's reimplantation technique: single-centre 25-year experience.

OBJECTIVES: Tirone E. David described aortic valve-sparing root reimplantion (AVSRR) almost 30 years ago. At our centre, we adopted this operation in 1993, and since then, we have performed >700 operations over a time period of >25 years. In this report, we present our single-centre experience. METHODS: Between 1993 and 2019, a total of 732 patients underwent AVSRR at our centre. The mean age was 53 ± 15 and 522 (71%) were male. Marfan syndrome was present in 117 (16%) patients and bicuspid aortic valve in 81 (11%). The indication for surgery was aortic root aneurysm in 588 (80%) patients and

Neonatal intra-atrial baffle repair for isolated ventricular inversion with left isomerism: a case report.

BACKGROUND: Discordant atrioventricular connection with concordant ventriculoarterial connection, otherwise known as isolated ventricular inversion (IVI), is an extremely rare congenital cardiac malformation. Reports on the corrective surgery for this anomaly in neonates are few, and the procedure is difficult and complicated. Herein, we report our use of atrial septostomy as a palliative procedure followed by corrective surgery for the repair of neonatal IVI with situs ambiguous(inversus) morphology. CASE PRESENTATION: A 2-day-old girl weighing 3.5 kg was admitted to our hospital with a low

Mitral valve operations through standard and smaller incisions.

OBJECTIVE: Evaluate the operative results of mitral valve repair (MVV) and mitral valve replacement (MVR) performed through standard and smaller incisions. METHODS: From January 1997 through December 2002, 821 consecutive patients underwent mitral valve operation. Of these procedures, 475 were MVV and 346 were MVR. A logistic regression model was developed to identify the risk factors for early mortality and to evaluate the effect of replacement versus repair and standard versus small incision. RESULTS: Replacement patients were older, more likely New York Heart Association (NYHA) class III

Minimally invasive videoscopic mitral valve surgery: the current role of surgical robotics.

OBJECTIVE: Recently, the efficacy of video-assisted mitral valve surgery has been demonstrated. The evolution of this technology has been relatively rapid. In this article we review this development and predict the future of endoscopic and robotic-enabling technology for cardiac valve operations. METHODS: A new video-assisted mitral valve operation is described and results discussed. The majority of each valve operation was done through assisted vision and near endoscopically. Cardiopulmonary bypass was established via femoral cannulation, and blood cardioplegic arrest induced using a new per

The subclavian and axillary arteries as inflow vessels for coronary artery bypass grafts--combined experience from three cardiac surgery centers.

BACKGROUND: The subclavian and axillary arteries represent reliable inflow vessels in peripheral vascular surgery. During recent years they have also been used for special situations in coronary artery bypass grafting. We report on a preliminary, triple center experience with subclavian/axillary artery to coronary artery bypass grafting. METHODS: Twenty-one patients (11 male, 10 female, median age 70 years) received subclavian artery/axillary artery to coronary artery bypass grafts. Indications for application of this bypass variation were internal mammary artery problems during minimally inv

The Use of Negative Pressure Wound Therapy as an Adjunct Treatment in Patients Undergoing Sternal Wound Reconstruction With Pectoralis Major Flaps: Effects on Postoperative Outcomes.

BACKGROUND: Deep sternal wound infection and dehiscence following median sternotomy remain challenging clinical problems with high morbidity. Negative pressure wound therapy (NPWT) has been reported to improve wound healing outcomes in various types of wounds. However, the reported efficacy of NPWT in sternal wound reconstruction varies widely in the literature. There are few large-scale single-institution studies examining the preoperative factors associated with NPWT and associated postoperative outcomes. METHODS: Records of 584 sternal wound reconstructions with pectoralis major flaps from

Postoperative outcomes of combined interpectoral-pectoserratus versus combined deep-superficial serratus anterior plane blocks in minimally invasive coronary artery bypass surgery: A prospective randomized study.

Minimally invasive coronary artery bypass grafting (MICS-CABG) and minimally invasive direct coronary artery bypass (MIDCAB) reduce surgical trauma compared with sternotomy but remain associated with significant postoperative pain that may delay recovery and contribute to complications. Although regional analgesic techniques are increasingly used to reduce opioid requirements, comparative data on different block strategies are limited. This study evaluated the effects of combined interpectoral-pectoserratus and combined deep-superficial serratus anterior plane blocks on postoperative pain, ana

Primary Pericardial Angiosarcoma Demonstrating Initial Response Followed by Progression on First-Line Chemotherapy: A Case Report.

Primary pericardial angiosarcoma is an exceedingly rare and highly aggressive malignancy arising from pericardial endothelial cells, with a reported median survival of approximately 7 months. Owing to its non-specific clinical presentation, diagnosis is frequently delayed, particularly in tuberculosis-endemic regions, where symptoms may be attributed to tuberculous pericarditis. We report the case of a 25-year-old Nepalese male who presented with a six-month history of progressively worsening dyspnea (MMRC grade 1 to 4) and an ECOG performance status of 3. Initial evaluation revealed a large p
