Immediate reverse shoulder arthroplasty demonstrates better outcomes than salvage reverse shoulder arthroplasty for proximal humerus fractures in the elderly: a meta-analysis.

BACKGROUND: Reverse shoulder arthroplasty (RSA) is a promising option for proximal humerus fractures (PHFs) and also used as a salvage procedure for failed treatment. Whether the timing of RSA would influence the clinical outcomes is controversial. This meta-analysis compared outcomes between immediate RSA (iRSA) and salvage RSA (sRSA) in geriatric patients with PHFs. METHODS: PubMed, Embase, Web of Science and Cochrane Library databases were searched up to 1st April 2024. Comparative studies reporting outcomes of iRSA and sRSA in geriatric patients with PHFs were included. A random-effects m

External fixation versus reverse shoulder arthroplasty for proximal humerus fractures in the elderly: a retrospective comparative study.

BACKGROUND: The optimal management of displaced proximal humerus fractures (PHFs) in the elderly remains a subject of ongoing debate. This study aims to compare the clinical and functional outcomes of biologically driven external fixation (EF) versus functional joint replacement via reverse shoulder arthroplasty (RSA) in patients aged 65-80 years. METHODS: A retrospective comparative study was conducted on patients with displaced Neer two- or three-part PHFs treated between 2015 and 2022. The final analysis included 67 patients: Group A (EF; n=34) and Group B (RSA; n=33). Clinical and function

Gender- and age-specific patterns of tennis-related injuries in pediatric populations: Insights from emergency room data (2014-2023).

INTRODUCTION: Pediatric tennis participation has grown steadily, yet limited research exists on sex- and age-based injury differences in this population. Understanding these patterns can inform tailored injury prevention efforts. MATERIALS AND METHODS: We queried the National Electronic Injury Surveillance System (NEISS) for tennis-related emergency department (ED) visits in patients under 18 years old from 2014 to 2023. Injuries were analyzed by sex, age group, injury type, and body region. National estimates (NEs) were calculated using NEISS sample weights. Statistical comparisons were perf

Comparing the exposure of the ulna coronoid using the anterior or medial surgical approaches: a cadaveric study.

BACKGROUND: Surgical fixation of isolated coronoid process fractures while maintaining the soft tissue integrity of the elbow joint is challenging. Several surgical approaches are described, each with their own benefits and risks. The purpose of this cadaveric study was to compare the extent of visualization and access to the coronoid process using the medial flexor carpi ulnaris (FCU)-split and anterior brachial artery and median nerve (B-M) approaches. METHODS: A total of 33 fresh frozen cadaveric upper limb specimens from 17 cadavers were dissected. Two surgical exposures, a FCU-split and

A preoperative decision algorithm for reverse shoulder arthroplasty in complex proximal humerus fractures in the elderly.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) has become the standard surgical option for managing complex proximal humerus fractures (PHFs) in elderly patients. Despite its widespread use, postoperative functional outcomes remain inconsistent, and patient selection criteria are not well defined. METHODS: A retrospective cohort study was conducted including 117 patients ≥75 years who underwent rTSA for Neer 3- or 4-part PHFs between 2012 and 2023 at a single tertiary hospital. Preoperative clinical and epidemiological variables were recorded, including Charlson Comorbidity Index, Ame

Evaluating Outcomes of Step-Cut Osteotomy for Pediatric Cubitus Deformities.

INTRODUCTION: Supracondylar fractures in children often result in malunion and subsequent cubitus varus or valgus deformity. While often considered cosmetic, these deformities can lead to pain, functional impairment, and other complications. Corrective osteotomy is a common treatment option, with step-cut osteotomy being a preferred method due to its effectiveness and relative simplicity. This study aims to evaluate the outcomes of step-cut osteotomy in correcting post-traumatic cubitus deformities. MATERIALS AND METHODS: A prospective cohort study was conducted on patients presenting with po

Does age affect patient outcomes after humeral head replacement in the treatment of acute proximal humeral fractures? A comparative cohort study with a minimum 10 years long-term follow-up.

BACKGROUND: Humeral head replacement (HHR) is now rarely recommended for complex proximal humeral fractures (PHFs) in older patients. However, in relatively young and active patients with unreconstructable complex PHFs, controversy still exists regarding the treatment options of reverse shoulder arthroplasty and HHR. The goal of this study was to compare the survival, functional, and radiographic outcomes of HHR in patients aged <70 years and those aged ≥70 years after a minimum 10 years follow-up. METHODS: Eighty-seven out of 135 patients undergoing primary HHR were enrolled and then divided

The influence of a continuum of care model on the rehabilitation of compensation claimants with soft tissue disorders.

STUDY DESIGN: Population-based before-and-after design with concurrent control group. OBJECTIVE: As continuums of care have been little studied, we evaluated the impact of the Workers' Compensation Board of Alberta (WCB-Alberta) model on sustained return to work, satisfaction with care, and cost. SUMMARY OF BACKGROUND DATA: Musculoskeletal conditions, such as back pain, continue to be leading causes of disability and work loss. From 1996 through 1997, the WCB-Alberta implemented a continuum of care model to guide rehabilitation service delivery for claimants with soft tissue injury. The mode

Long-term results of remodeling of lateral condylar prominence after lateral closed-wedge osteotomy for cubitus varus.

BACKGROUND: Despite the ability of lateral closed-wedge osteotomy for cubitus varus deformity to restore carrying angle and preserve elbow motion, there have been reports of poor cosmetic appearance of the elbow because of lateral condylar prominence. METHOD: We evaluated long-term results of remodeling of lateral condylar prominence after osteotomy in 11 patients (7 of the prepuberty group, 4 of the postpuberty group). The follow-up period ranged from 4.7 to 14.2 years (average, 10). RESULTS: The mean preoperative and postoperative lateral condylar prominence index (LCPI) of the affected el

Severe tardy ulnar nerve palsy caused by traumatic cubitus valgus deformity: functional outcome of subcutaneous anterior transposition.

Ten male patients with McGowan's grade III ulnar neuropathy due to traumatic cubitus valgus deformity underwent anterior subcutaneous ulnar transposition. Evaluation was performed using subjective and objective measures, and a modified Bishop score. After operation, subjective sensory and motor disturbances were improved or resolved in most of the patients, while objective measures improved less well. Improvement in two-point discrimination (2PD) was consistently associated with symptom relief. All of the patients reported satisfaction with the operation. There were no complications or recurre

Effect of prior rotator cuff repair technique and timing on clinical outcomes of reverse shoulder arthroplasty.

AIMS: Reverse shoulder arthroplasty (RSA) after prior rotator cuff repair (RCR) is commonly performed for recurrence of pain and arthritic progression; however, the influence of prior RCR timing and technique on outcomes of RSA has not been adequately explored. Our primary aim was to evaluate the relationship between the postoperative American Shoulder and Elbow Surgeons (ASES) score, whether prior RCR was performed open or arthroscopic, and the time interval between RSA and prior RCR. Secondary outcomes were forward elevation (FE) and external rotation (ER). METHODS: A prospectively collecte
